    Caring staff, homey, highly recommended

I'm very happy with this facility - the staff are consistently kind and professional, nurses and CNAs are attentive, and the admin team (Thomas and activities director Marianne included) advocate for residents. Therapy and long-term care options are strong, Medicaid navigation was helpful, the building is clean, and the atmosphere feels like home with residents socializing and improving. I'd encourage a tour and I highly recommend this caring, trustworthy place.

    Summary of reviews

Reviews of Fairlawn Health and Rehabilitation of Cascadia present a strongly polarized picture: many families and residents describe attentive, compassionate caregivers, an active therapy program, and engaged administrative staff, while other accounts identify significant operational and clinical weaknesses. The favorable comments emphasize warm, family-like interactions, effective rehabilitation services that support return-to-function and weight gains, helpful front-office support (including Medicaid navigation), and an activities program that promotes socialization. Several reviewers also noted improvements tied to new ownership and visible managerial engagement.

Clinical-care feedback is mixed. Positive accounts praise attentive CNAs and therapists; however, a subset of reviews raises substantive concerns about medication administration, clinical oversight, wound and infection management, and the facilityโ€™s ability to follow through on special-diet or individualized care plans. These concerns cluster around inconsistent application of clinical protocols, delays in responding to resident needs, and variable follow-up on nursing or physician orders. There are also reports that have prompted questions about end-of-life coordination and timing of clinical escalation.

Staffing, communication, and conduct also show variability. Many reviews highlight supportive, professional nurses and an administrative team that advocates for residents. Conversely, other reviews describe delays in call-light responses, lapses in incontinence-care or hygiene-management, and instances of unprofessional communication style. Visitor-access issues โ€” such as restricted restroom assistance or locked doors that complicate visits โ€” were mentioned alongside praise for staff who are flexible and responsive when concerns are escalated.

Dining, activities, and facilities likewise produce mixed impressions. The activities program and opportunities for social engagement were consistently described as strengths; several families credited activities staff with improving appetite and mood. Meal quality was described as inconsistent, and the facilityโ€™s responsiveness to special-diet requests was sometimes slow. Facilities were often described as clean and well maintained in common areas, though there are isolated sanitation and cleanliness inconsistencies noted in some accounts.

Notable operational patterns that emergent reviewers should consider when evaluating this facility include: medication-management and clinical-oversight variability; potential delays in direct-care response times; gaps in wound, infection, and end-of-life care coordination; inconsistent meal and special-diet follow-through; visitor-accessibility concerns; and serious allegations about billing controls and unauthorized charges. Many reviewers who encountered problems described positive outcomes when issues were escalated to specific administrative staff members, which suggests that family advocacy and direct communication with leadership can influence resolution. Prospective residents and families may wish to tour the facility, ask about staffing levels and call-response metrics, review medication-administration and infection-control protocols, clarify visitation and restroom-access policies, and request written billing safeguards before admission.

About Fairlawn Health and Rehabilitation of Cascadia

Fairlawn Health and Rehabilitation of Cascadia sits at 3457 NE Division St in Gresham, Oregon, and it runs around the clock, every day, so folks can always get the care they need without worrying about schedules or missed help or anything slipping through the cracks, which sometimes brings peace to family members when someone needs support at odd hours. The place has 82 beds and works as a licensed Skilled Nursing Facility, focusing on long-term care, transitional care, and patient-centered rehabilitation, and the staff is experienced in geriatric care, so seniors get support and kindness whether it's for a short recovery or for living there longer. They offer skilled nursing care all day and night, plus rehab therapies like physical, occupational, and speech therapy, so those recovering from illness or injury have options, and if someone needs respite care or memory care, they've set programs for those needs, too, with plenty of medical support like nurses, wound care, and medication help, and they'll even bring in podiatry services on site. Residents get a choice between private rooms, costing about $8,000 to $11,000 per month, or semi-private rooms, ranging from $7,000 to $9,000 per month, and whether you pick private or shared, the rooms include typical amenities for skilled nursing like kitchens or kitchenettes, dining areas, WiFi, cable TV, washers and dryers, safety features like sprinklers, and easy-to-use bathrooms, which sometimes helps keep life feeling closer to home. The building was recently renovated, so living spaces and the common areas look modern and welcoming, and since there are spacious outdoor areas, residents can spend time in the fresh air when they feel like it, alone or socializing, which many people appreciate.

The staff, who speak English and sometimes other languages, help with daily needs like bathing, dressing, grooming, laundry, and toileting, plus they've got transportation for doctor visits or outings, and guests can park on site if they want to visit, so family doesn't need to worry about where to leave their car. There are plenty of activities, arts and crafts, fitness programs, educational opportunities, and social events, all aimed at keeping folks involved and engaged, and the game rooms or activity spaces are always available for those who want to join a group or spend quiet time with new friends. There are health and wellness programs, a fitness center, and a salon/barbershop, and meals are served in pleasant dining rooms, trying to keep things comfortable and more like you're at home instead of in a hospital. They accept long-term care insurance, along with Medicare and Medicaid, and have staff to help families understand financing options, which can make things easier when figuring out costs and payment. Regular scheduled assessments help make sure individual care needs are always up to date, and staff follow a clear Code of Conduct, staying focused on proper privacy practices and respect for every resident. People thinking about moving in or moving a loved one in can set up tours or virtual visits, read up on provider licensing and inspection records, or get answers about discharge planning, waiting lists, and how skilled nursing fits compared to things like assisted living or independent living, so no one's left confused. There's always skilled nursing staff on site, and the facility is designed with safety, comfort, and well-being in mind, so whether someone's there for recovery or planning for long-term support, the environment stays homelike and supportive, without being too fancy or too basic.
