    Caring, responsive staff and therapy

I placed my mother, who has a debilitating terminal illness, in this facility and was very pleased with the caring, responsive staff. Cheryl (administrator), Nurse Mike, Alex and Jennifer in Social Services went above and beyond, kept me informed, and provided professional nursing and therapy that truly helped and comforted her. The building was clean, staff were friendly and upbeat, therapy was proactive with sensible safety recommendations, and little touches like daily puzzles boosted morale. Overall I felt supported and would recommend their compassionate, knowledgeable team.

    Summary of reviews

Reviewer feedback for this facility is mixed, with a clear split between families who praise individual caregivers and therapy outcomes and others who identify persistent operational problems. Positive commentary centers on the clinical and relational strengths of particular staff members and teams: several families highlight compassionate nursing, effective physical and occupational therapy that supported recovery, and social-services and reception staff who facilitate family communication and logistics. Management accessibility and timely family updates were called out as strengths in multiple accounts, and an activities program and engaged activities staff were appreciated for contributing to resident morale.

At the same time, a number of reviews describe recurring operational weaknesses that affect day-to-day resident experience. Cleanliness and housekeeping practices were frequently characterized as inconsistent, and reviewers raised concerns about incontinence-care processes and uneven bathing schedules. Call-light response times and delayed bedside assistance were also common themes, with several accounts describing prolonged waits for help. Medication handling and timeliness of pain management were described as unreliable in some reports, and a subset of comments raised serious concerns, including allegations of missing medications and taken meals.

Staffing and workforce issues appear to underlie many of the negative observations. Reviewers referenced short staffing, high turnover, and apparent burnout, which they linked to reduced service hours, slower responses, and uneven care coverageโ€”particularly during nights and shift changes. Several families also reported inconsistent infection-control experiences during COVID-related events and noted that facility ventilation and odor control could be improved.

Dining and amenities drew mixed remarks: some found the food adequate, while others cited cold or dry meals and water with an off-taste. The building itself was described as older in places, with variable odor and ventilation concerns that may reflect maintenance priorities. On balance, prospective residents and families should weigh the facilityโ€™s evident strengths in individualized clinical care and family communication against operational gaps in housekeeping, staffing, responsiveness, medication controls, and infection prevention. When investigating this facility, visitors should ask for current staffing ratios, housekeeping and infection-control protocols, medication-safety procedures, and recent inspection or incident records, and should seek direct conversations with therapy, nursing leadership, and social services to verify how the facility addresses the issues raised in reviews.

    About Sapphire at Tigard Rehab

Sapphire at Tigard Rehab is a 112-bed nursing facility where seniors get support and care around the clock, and staff work hard to make sure everyone feels safe and respected, whether residents need skilled nursing, short-term rehabilitation, memory care, or assisted-living. This for-profit facility is part of the Sapphire Health Services Community and is known among clinicians for quality care, with high clinician ratings based on their reviews, and they make individualized care plans for each person so that everyone's different needs are met whether someone needs help for dementia, Alzheimer's, or recovery from an injury or surgery, and sometimes people just stay for respite care if family caregivers need a break. At Sapphire at Tigard Rehab, licensed nurses and staff are on hand 24 hours a day, so residents get help when they need it, and there's always a focus on comfort, safety, and keeping folks as independent as possible, with physical therapy and rehabilitation services to help people regain strength and skills. The facility is smaller, which lets the staff focus closely on daily needs and give attention to things like personal care, dignity, and respect, while keeping a family-like atmosphere that feels warm and friendly, with regular activities, both social and recreational, plus chances for spiritual engagement, which many people appreciate. Common areas are comfortable, there are clean outdoor spaces, and meals are served three times a day with specialized dietary services to fit everyone's health concerns-plus housekeeping and laundry are taken care of. Residents who need memory care have access to skilled nurses and doctors, and some services are available for seniors who want to stay in their own homes with familiar belongings, thanks to in-home care options. Sapphire at Tigard Rehab accepts Medicaid, Medicare, and many managed care plans, and the staff tries to help not just residents but their families too, working to promote the highest possible quality of life for everyone involved.
