    Clean, caring, professional pet-friendly facility

I was very pleased with my experience at this senior living facility. The place is clean and smells pleasant, reception is welcoming, and the staff-from nurses and CNAs to therapists and admissions-are friendly, professional and genuinely caring. Therapy and rehab were outstanding, communication was responsive (they even used FaceTime to check in), and the team worked together to solve issues quickly; the pet-friendly, family-like atmosphere made my loved one comfortable. I would feel confident placing a family member here.

    Summary of reviews

The reviews present a mixed picture of Johns Island Post Acute. Clinical rehabilitation services and many members of the therapy team receive repeated praise for producing meaningful functional gains; families and patients frequently highlighted effective PT/OT and individual therapists who contributed to mobility, self-care, and communication improvements. Admissions, social work, and some nursing staff are commonly described as knowledgeable and welcoming, and several reviewers noted proactive care planning and timely outreach to families.

At the same time, operational inconsistencies are a recurring theme. Meal service and food quality are described as uneven, with interruptions in supply and variability in preparation. Staffing stability is a concern: reviewers mentioned short staffing, the use of agency personnel, and limited therapeutic coverage on weekends. These staffing patterns are linked to delays in medication administration and pain management, variable bedside assistance, and inconsistent participation in scheduled wellness activities.

Facility upkeep shows both strengths and weaknesses. Many common areas and courtyards are described as clean and pleasant, with responsive maintenance and outdoor spaces that allow pet visits. Conversely, some reviews raised sanitation and pest-control concerns in specific areas, and there were multiple mentions of laundry or linen issues and damaged or poorly maintained fixtures. Housekeeping performance appears variable across shifts or units, creating an uneven impression of cleanliness.

Staff conduct and communication are polarized. Numerous reviews praise compassionate, attentive caregivers and leadership efforts to improve processes; multiple staff members were singled out for helpfulness and teamwork. However, other accounts describe unprofessional interactions, poor front-desk communication, staff distraction with personal devices, and gaps in family notification after incidents. One serious allegation of racially offensive language and multiple descriptions of staff-to-staff confrontations indicate areas needing attention to workplace culture and training.

Management and leadership appear to be in transition in some areas, with several reviewers noting positive changes, increased oversight, and responsiveness from administrators. At the same time, continuing supply-management gaps (for example, linens or basic supplies) and inconsistent execution of policies suggest that improvements are still in progress. Prospective families should ask specific questions about current staffing ratios, weekend therapy availability, medication administration protocols, infection-control and pest-control measures, and how the facility communicates incidents to families.

In summary, Johns Island Post Acute offers strong rehabilitative capability and has many staff who provide compassionate, effective care. Those strengths are tempered by operational and consistency issuesโ€”particularly around dining, staffing, medication timeliness, housekeeping, and communicationโ€”that prospective residents and families should evaluate during tours and admissions discussions. Clear, specific questions about current practices and recent management actions will help determine if the facilityโ€™s recent improvements align with a familyโ€™s priorities.

    About Johns Island Post Acute

Johns Island Post Acute sits about 10 miles from historic downtown Charleston, SC, over at 3647 Maybank Highway, and it's a skilled nursing facility where folks can get both long-term and short-term care, especially if they're dealing with tough medical needs, things like brain injuries, trachs, and feeding tubes, and while the place hasn't shared a lot about its services or amenities, people do say the staff knows everyone by name and cares for patients in a friendly way. The focus leans pretty hard on personalized care, especially for seniors, and they've got therapy services such as physical, occupational, and speech therapy, plus some dining options and various social activities, though nobody gets too specific about what those activities look like or what the dining includes, and you won't find much about onsite rehab facilities or religious services. The facility's got an administrator named Jared Leavitt and offers post-acute care, which covers a range of recoveries and care needs, and if folks want to learn more details, heading over to https://johnsislandpa.com would be the next step, because there really isn't much else in the way of public information about what the place is like day to day.
