    Compassionate care clean welcoming facility

I had a very positive experience - nursing and admissions staff were knowledgeable and caring, therapists helped my loved one regain mobility, and CNAs and housekeeping were attentive and respectful. The private rooms and common areas were spotless, the garden and activities (bingo, church, music) made the place feel welcoming, and administration is making helpful improvements. I'm grateful for the compassionate, professional team and would recommend this facility.

    Summary of reviews

The reviews present a highly mixed picture of Hallmark Healthcare Center, with distinct positive experiences alongside serious operational concerns. A recurring strength is the clinical and therapeutic team: many families and residents praised therapists, nurses, and admission/social-work staff for being compassionate, attentive, and effective in rehabilitation. Several accounts describe meaningful functional recovery, individualized therapy plans, and supportive hand-holding during difficult transitions. Activity programming and family-oriented initiatives (including group activities, religious services, music, and a guardian-style program) are frequently mentioned as bright spots that contribute to resident engagement and morale.

At the same time, a substantial set of operational weaknesses appears repeatedly. Staffing inconsistency is a prominent theme โ€” reviewers describe delays in responding to call lights, long wait times for assistance, and variability in staff conduct and tone. Related to that, medication-management processes and communication with families are described as uneven: missed or delayed medication delivery, insufficient notification about medication changes, and general communication gaps between staff and families. Several reviews also raise sanitation and incontinence-care concerns in particular areas, and note maintenance issues such as broken beds, plumbing problems, mold, and rooms in need of renovation.

Dining and housekeeping impressions are mixed. Some families report satisfying meals and improvements to laundry services; others describe inconsistent food quality, incorrect meal delivery, and ongoing cleanliness problems in parts of the facility. The physical environment draws similarly mixed comments: positive notes about clean units, a pleasant garden and gazebo, and rooms that are roomy for two contrast with reports of dim, cramped rooms, dated dining spaces, and infrastructure deterioration. Multiple reviewers commented on improvements under new or proactive maintenance leadership, suggesting that some cosmetic and repair work has been underway.

Management and communication receive polarized feedback. Several reviewers praised a responsive, caring administration and specific staff members by name, and some noted visible improvements after leadership changes. Conversely, others described limited phone responsiveness, a perception of disorganized or revolving management, and inconsistent enforcement of visitation or infection-control policies โ€” including concerns about COVID-era testing and visitation restrictions. A small number of reviews expressed serious concerns following clinical incidents, including end-of-life situations; these emphasize the facilityโ€™s need for clearer incident-response protocols and family communication.

Overall, the pattern suggests that Hallmark can deliver high-quality rehabilitative care and compassionate support under certain staffing and leadership conditions, but operational variability โ€” particularly around staffing levels, medication processes, sanitation in some areas, and facility maintenance โ€” creates a risk of uneven resident experience. Prospective residents and families should weigh the positive reports of therapy outcomes and engaged staff against the operational inconsistencies described, and consider direct questions about current staffing ratios, medication-management procedures, cleaning and maintenance plans, and recent administrative changes during any tour or intake conversation.

    About Hallmark Healthcare Center

Hallmark Healthcare Center sits at 255 Midland Parkway in Summerville, SC and is managed by Kellie Jellyman, and you find a range of healthcare and senior services under one roof, including skilled nursing, assisted living, support for people living with Alzheimer's or dementia, and palliative care when that's needed, with a licensed capacity for 88 certified beds, so it's not too big, not too small, and the staff-who go by names like "nursing care associates"-set up programs and therapies like physical, occupational, and speech therapy, with the nurse turnover rate at 42.5% and an average of 3.12 nurse hours per resident per day, which has its challenges, but the team still delivers care for all kinds of personal and medical needs, covering short-term recovery and longer stays too, and they even have specific programs for stroke rehabilitation, post-surgical recovery, and cardiac rehab, using newer treatment techniques and always trying to follow a patient-centered approach, meaning each resident gets a plan built around their health and comfort. Hallmark Healthcare Center offers amenities and conveniences that focus on comfort, trying to make the environment feel inviting for people who stay there, and they say the facility supports everyone equally, regardless of race, color, national origin, religion, gender, orientation, age, or disability status, and that includes people with AIDS, HIV, or related conditions. Now, inspections have found some problems in the past, with 17 documented deficiencies-including issues with drug storage and labeling, equipment safety, and infection control, specifically with two infection-related deficiencies-which are serious areas that they've had to address, and every three years inspectors check for complaints and make sure infection-control rules are being followed, so those reports are always worth looking at if you're worried about compliance. Hallmark's staff uses distinct program names and specialized terms for many of their services, and the healthcare center is affiliated with Fundamental Healthcare, which helps guide care standards and policies. People looking for care can use their facility finder tool online, and their programs aim to help seniors improve their quality of life, with staff that many recognize for long-term dedication. The focus is on providing care that fits the individual, keeping services clear and simple, and making sure support covers both health and everyday comfort for all residents.
