    Caring staff and effective rehab

I'm very satisfied with this facility - admission and administrative staff were welcoming and helpful, and the RNs, CNAs, therapists and housekeeping showed genuine care and expertise. The rehab program helped my loved one regain independence, residents seem happy, the building is clean and welcoming, and staff gave strong support and guidance to our family. I would recommend this place for long-term and Alzheimer's care.

    Summary of reviews

Rockwood Care & Rehab Center elicits a mixed set of impressions. On the positive side, the facility has notable strengths in specialized dementia care and its rehabilitation program. The dementia/Alzheimer's unit (Bridges) is repeatedly described as having knowledgeable, patient staff who provide focused programming and family support. The interdisciplinary therapy team and rehab services receive consistent praise for helping residents regain mobility and independence, with several families noting successful returns to home and practical guidance on home modifications.

Clinical and direct-care staff are frequently characterized as compassionate and respectful. Many comments single out CNAs, nurses, therapists, and admissions/administrative personnel for their attentiveness, teamwork, and welcoming demeanor. Residents and families also appreciate social programming, community outings, seasonal activities, and varied meal choices; these elements contribute to a generally resident-centered atmosphere and positive daily experience for many occupants.

Counterbalancing those strengths are recurring operational concerns. Medication administration and pharmacy coordination weaknesses are an important pattern — reviewers describe delays in receiving medications, late medication rounds, and coordination problems between the facility and pharmacy or medical records. Relatedly, delays in staff responsiveness to call requests and inconsistencies in incontinence care were raised as care-quality issues that affect daily comfort and dignity.

Housekeeping and physical-condition issues appear uneven across the building. While some areas are described as clean and well-maintained, other accounts point to inconsistent room conditions, discrepancies between advertised room appearance and reality, and isolated pest-control concerns. Safety-process shortcomings were also cited: examples include gaps in fall documentation and maintenance problems that can affect resident safety. Additionally, a few reviews describe serious concerns following a resident's death and include allegations of dishonesty by staff; these comments suggest variability in conduct and communication that families should evaluate carefully.

Management and communication show variability. Several families praised effective leadership and clear communication, while others experienced inconsistent updates and limited responsiveness from clinical leadership during incidents, including infection events such as COVID-19. This unevenness in operational consistency and family communication emerges as a central pattern across reviews.

For prospective residents and families: Rockwood offers strong clinical programs in dementia care and rehabilitation and has many staff who provide compassionate, individualized care. At the same time, there are repeat concerns about medication processes, responsiveness, cleanliness consistency, safety documentation, and occasional staff-conduct issues. Visitors planning a tour should observe the specific unit they are considering, ask about medication and pharmacy procedures, infection-incident protocols, staffing patterns and ratios, housekeeping and pest-control practices, safety documentation (including fall-prevention and post-fall reporting), and how the facility handles family communication during clinical events. These targeted questions can help reconcile the divergent experiences described and clarify whether the facility’s strengths align with a particular resident’s needs.

About Rockwood Care & Rehab Center

Rockwood Care & Rehab Center, sometimes called Signature Healthcare Of Rockwood Rehab & Wellness or Bridge at Rockwood, sits at 5580 Roane State Hwy in Rockwood, Tennessee, close to Hwy. 70 and Hwy. 27. The center is part of Signature HealthCARE and runs as a for-profit limited liability company, with LP Cr Holdings LLC owning it completely and companies like Agemo Holdings LLC, Jjla LLC, Lpsnf II LLC, Wheaten LLC, Ira Smedra, and Elmer Steier III having indirect stakes. The building has 157 certified beds and averages about 81 residents a day. The business office opens from 8:00AM to 5:00PM, but the facility runs 24/7, so care never stops.

The center focuses on rehabilitation and wellness. Staff offer skilled nursing care, memory care, and assisted living. Long-term care, short-term respite stays, and home care services are also available. People come here for therapy after hospital stays or to recover from illness or injury, and there are dedicated programs for both physical and emotional wellness. Life enrichment programs help residents stay active and connected, even as they get older. They also have interfaith spirituality programs and take a family-based approach to caring for residents, with personal care plans for each person. There's an effort to serve a mix of cultures, with programs that try to honor spiritual needs and different customs.

The nursing staff spends about 3.75 hours a day with each resident, though there's a high nurse turnover rate at 29.1%. Rockwood Care & Rehab Center is led by Olivia Strickland, the Office Manager, along with administrators Ray Allen, Allison McKenney (Administrator in Training), and Roger Parker. There's a focus on quality of life and care navigation to help families and residents understand the care options.

Recent inspection reports note some serious problems. As of January 11, 2023, the center had a deficiency related to protecting residents from abuse, neglect, and exploitation, with potential for more than minimal harm. An infection control deficiency appeared on May 17, 2023, affecting residents' health and safety. Regular inspections haven't happened in over two years, and the inspection history shows repeated issues with compliance.

Many people look at reviews and see different ratings; Signature HealthCARE of Rockwood Rehab & Wellness Center, for example, has a 9.2/10, while Signature Healthcare Of Rockwood Rehab And Wellness Center rates at 8.5/10, and other nearby facilities like Harriman Care & Rehabilitation Center and Spring City Care & Rehabilitation Center have ratings from around 6.8 to 8.6.

Rockwood Care & Rehab Center's main purpose matches its mission, to try and improve each resident's health care experience with personal care, family involvement, and programs that support dignity. Anyone wanting more details about inspections or care provided can find extra information on the facility's Medicare page.
