    Compassionate staff and excellent care

My mom lives here and we couldn't be happier. The compassionate, caring staff go above and beyond, the building is immaculate and cozy and truly feels like home, nursing and therapy are excellent, move-in was quick and organized (special thanks to Admissions Counselor Amber Robinson), and the family-oriented, welcoming vibe gives us real peace of mind.

    Summary of reviews

Overall impression: Reviews present a mixed but informative picture. The facility is repeatedly praised for its rehabilitation services, therapy staff, and a small, home-like environment that many families find comforting. Several reviewers highlighted strong teamwork, a welcoming culture, and an efficient admissions experience. At the same time, a number of families described operational inconsistencies that affected day-to-day care and family confidence.

Care quality and clinical issues: Strengths are most evident in the therapy/rehabilitation program and among individual caregivers who are described as compassionate and skilled. However, there are notable patterns of concern around responsiveness and clinical follow-through. Families described delays in attending to resident needs via call lights and inconsistencies in incontinence and hygiene assistance. Some reviews raised concerns about wound care and medication-management practices; these items point to gaps in clinical process adherence and oversight rather than uniformly poor care. There is also mention of pressure-injury concerns arising in specific cases, which suggests the need to review pressure-injury prevention and monitoring protocols.

Staff, management, and culture: Staff are frequently characterized as caring, friendly, and willing to go above and beyond; several comments specifically commend individual staff members and the admissions counselor for clear communication. At the same time, reviewers indicate considerable variability between employees — some nurses and aides receive high praise while others are described as less attentive. Management visibility and responsiveness appear uneven: some families noted improvements under new managers and better departmental coordination, while others cited difficulty reaching administrators or receiving timely follow-up.

Dining, activities, and facilities: The building and common areas are often described as clean, attractive, and well maintained; many reviewers appreciate the facility's small scale and community feel. Activities and social opportunities are viewed positively, with residents and families noting social engagement and a friendly atmosphere. Feedback on dining is mixed: several reviewers praised the food, but a subset reported issues with meal temperature and inconsistent meal quality.

Notable patterns and recommendations for visitors: The consistent positives to verify are the therapy program, staff friendliness, and the facility’s welcoming environment. Areas to investigate during a visit include call-light response times, incontinence-care routines, wound-care and medication-management protocols, recent staff turnover or training initiatives, and administrative/contact procedures for families. Observing mealtimes and common-area cleanliness during a tour, and meeting the therapy team and nursing supervisors, can help prospective families assess how the facility addresses the operational inconsistencies described.

    About Windsor of Bastrop

Windsor Nursing and Rehabilitation Center of Bastrop sits over in Bastrop, Texas, and operates as a nursing and rehabilitation facility with space for up to 96 certified beds, though you'll usually find about 46 residents staying there each day. People come here for short-term rehabilitation after surgery or illness, as well as long-term care if they need more help day-to-day, and the focus lands pretty square on therapies and skilled medical support with physical, occupational, and speech therapy available both for those staying in the facility and folks coming in from outside for outpatient visits. The center tends to those recovering after hospital stays, offering cardiac care, pulmonary support, stroke care, tube feeding, wound care, and therapy for folks who had joint replacement or orthopedic surgeries, and there's attention given to helping people stay as independent as they can.

Windsor provides care for several needs, including Alzheimer's, Dementia, and general Memory Support Services, along with palliative and hospice care at end of life, and they can admit new residents any day of the week. They set up specialized programs like nutritional support and incontinence management, and offer both private and semi-private rooms with amenities like WiFi, cable, recreational activities, transportation, and a general aim to make the place comfortable for getting better or living with daily challenges. Skilled nurses watch over the residents all day and night, though daily nurse staffing hours average about 2.75 hours per resident, which comes in under the state norm of 3.4 hours. Nurse turnover in a recent report was 40%, which is notably lower than the Texas average.

Windsor's had 21 total deficiencies reported during inspections, including issues with infection control and proper nutrition; in June 2024, six deficiencies got flagged, such as not getting food from approved sources and falling short in developing full care plans for some residents, though these were marked as causing no actual harm but showing potential for more than minimal risk. Management comes from Regency IHS of Bastrop LLC since October 2022, and OakBend Medical Center fully owns the facility, which is affiliated with Wellsential Health. Residents or their families can expect access to things like home health services if they'd rather recover at home, and there's a range of therapy options offered, so folks get support to regain strength or maintain function as needed. Amenities and services aim for comfort, but there's room for improvement based on recent inspection findings.
