    Caring staff, clean facility, recommended

I'm very pleased with Beaumont Nursing & Rehabilitation - the staff were consistently caring, professional and welcoming (special thanks to nurses Ariana, Shon and Joy and admissions support from Kaci and Brittany). The facility felt clean and home-like, with flavorful meals, in-house PT/OT/speech, attentive nursing, smooth admissions and compassionate communication; I recommend it.

    Summary of reviews

Overall impression is predominantly positive: many families praise Beaumont Nursing and Rehabilitation for a compassionate, family-oriented culture, skilled therapy services, and a clean, home-like environment. Admissions and administrative staff receive frequent commendation for being responsive and helpful during placements, and the facility's location and small-unit options are seen as conveniences for families. In-house rehabilitation (physical, occupational, and speech therapy), daily clinical follow-up for some residents, and post-discharge assistance are highlighted as strengths that support recovery and continuity of care.

Care quality is generally described as attentive and skilled, with multiple mentions of nurses and therapists who are personable and effective. Families report meaningful clinical progress for some residents and appreciate individualized attention, bedside nursing presence, and organized therapy programming. That said, a subset of reviewers raised concerns about inconsistent staff conduct, variability in responsiveness, and episodic monitoring lapses; these comments point to uneven performance across shifts or units rather than uniformly poor care. There are also specific concerns that touch operational controls—most notably around medication handling and administration—which suggest the facility should reinforce medication-administration protocols and auditing practices.

Dining and activities are consistently noted as positives. Reviewers cite flavorful meals, routine meal service, and an active schedule of recreational opportunities that contribute to a home-like atmosphere. The activities program is described as organized, and staff engagement in events is seen as a contributor to resident morale. The facility is characterized as clean and well-kept, with pleasant rooms and communal areas; however, the building is described as older and lacks a dedicated outdoor garden area, which may matter to families seeking outdoor access.

Notable patterns include strong individual praise for specific staff and leaders, balanced by isolated but serious operational concerns. Several families referenced issues involving personal-property handling and one instance of an unauthorized documentation/notarization matter that prompted an investigation; these items raise questions about property safeguards and documentation/privacy controls. Combined with the medication-related concerns and occasional accounts of short staffing or limited check-ins, the pattern indicates that while relational care and therapy services are strengths, certain administrative and clinical controls need attention.

For prospective residents and families: verify current staffing ratios and shift coverage, ask about the facility's medication-administration safeguards and audit procedures, review privacy/documentation policies and incident-reporting processes, and request a tour that includes any outdoor spaces. Also consider speaking with recent family contacts and asking how the facility handles follow-up after incidents and transitions home. Beaumont appears to offer strong therapeutic services, engaged staff, and a welcoming environment for many residents; assessing the facility's response to the operational issues noted above will help determine fit for an individual resident's needs.

    About Beaumont Nursing and Rehabilitation

Beaumont Nursing and Rehabilitation holds 120 certified beds and is part of the Creative Solutions In Healthcare family network, managed by Creative Solutions In Healthcare Inc since October 2011, and Gary Blake and Malisa Blake also play key ownership and management roles through Honor X Enterprises, LLC. The facility serves an average of 42 residents each day and offers around-the-clock skilled nursing care, memory care services, and a range of therapies including physical, occupational, and speech therapy. The team uses personalized care plans, taking into consideration each person's unique needs, and provides specific programs like wound care, tracheotomy care, respiratory therapy, vent care, post-surgical stabilization, intravenous therapy, enteral and parenteral care, and in-house dialysis. Assisted living services, rehabilitation services, and short-term or long-term care options are available, along with programs for Veterans Affairs contract services. Residents can expect dental, podiatry, and optometry services on site, and there's care for people recovering from surgery or needing respite care as well as specialized programs for stroke, diabetes, cardiac, and other neurological conditions.

The facility places an emphasis on safety, dignity, and a home-like environment, aiming to help people stay as independent and active as possible. There's a focus on nutritious meals from culinary staff, daily activities for mental and physical well-being, and ways for family to stay informed using an online health portal. Laundry, housekeeping, beauty, and barber services are available for everyday comfort, and spiritual and religious activities are offered. However, the nurse turnover rate of 66.7% is notably higher than the state average of 51.8%, and official inspections reported 15 deficiencies, including infection control and failure to address accident hazards. The facility was fined $14,020 after a 2025 complaint and cited for issues related to preventing, reporting, and investigating abuse, neglect, or theft.

Despite these challenges, a dedicated care team works to create a welcoming and safe place for residents, offering memory care, medication management, restorative nursing, and a choice of rehabilitation services designed to help people return home after illness or injury. The community supports its residents and their families through transparent health updates, a long-term care ombudsman for confidential complaints, and a customer care line for any concerns or questions. While Beaumont Nursing and Rehabilitation has many services and amenities to support resident comfort and well-being, it operates under regular oversight and faces areas noted by regulators for improvement.
