    Moved mom to Sandy Lake

I moved my mom to Sandy Lake and have been very pleased. The staff - nurses, CNAs, therapists, housekeeping (Star), maintenance and a standout activities director (Grace) - are compassionate, professional and welcoming. The building is exceptionally clean, bright and homey, therapy is effective, and administration keeps us informed. My family feels comforted and supported; she's settling in and making real progress. I highly recommend it.

    Summary of reviews

Overall impression: Reviews of Sandy Lake Rehabilitation and Care Center are mixed and polarized. Many families describe strong rehabilitation outcomes, an active activities program, and caring individual staff members; other accounts raise facility-level concerns about consistency of nursing care, housekeeping, communication, and property management. Prospective residents and families should weigh the facility's rehabilitative strengths against variability in day-to-day nursing and support services.

Care quality and clinical services: The facility's inpatient rehabilitation services — particularly physical, occupational, and speech therapy — are commonly described as effective and outcome-oriented. Multiple families credited therapy staff with measurable progress and timely discharge home. Nursing and direct care quality is uneven: reviews identify standout CNAs and nurses who provide attentive care, but also frequent accounts of delayed medication administration, inconsistent monitoring, and variability in night-shift responsiveness. Several reviews describe skin-integrity and incontinence-care concerns, suggesting risk areas where consistent hygiene and surveillance practices are important.

Staff and conduct: Many reviewers praise individual caregivers for compassion, responsiveness, and professionalism; named staff members are frequently singled out positively. At the same time there are repeated comments about understaffing, high turnover, reliance on agency staff, language barriers, and uneven training. Those operational patterns appear to underlie issues with response times, care consistency, and occasional problems with staff conduct and communication tone. Leadership is described as changing frequently in some accounts; families cite both engaged management and periods when administrators were slow to respond to concerns.

Dining and nutrition: Dining impressions are mixed. The food is characterized by some as cafeteria-style but acceptable, with a few families noting helpful dietary coordination and personalized meals. Other accounts cite cold meals, limited choices, and a desire for more nutritious or varied options. Nutrition coordination and same-day accommodations are mentioned positively in several reviews, indicating capability when meals are individualized.

Activities and environment: The activities program is a clear strength: reviewers describe creative, varied, and inclusive programming (bowling, baking, exercise, arts, trivia, bingo, etc.) and praise enthusiastic activities staff. Common areas, landscaping, and the rehab gym are frequently noted as clean, pleasant, and well-maintained. Room quality and cleanliness, however, vary by unit — some rooms are described as bright and modern while others are described as dated or small (many double-occupancy rooms).

Facilities, housekeeping, and maintenance: Common areas and grounds receive positive comments for cleanliness and upkeep. Conversely, households of reviewers describe inconsistent housekeeping in resident rooms and bathrooms, broken furniture or equipment (call buttons, chairs, drawers), and occasional sanitation or odor concerns in specific units. Property-management weaknesses are repeatedly mentioned, including lost clothing or personal items and problems retrieving dentures or hearing aids.

Management and notable patterns: The pattern across reviews is one of contrast: effective rehabilitation and strong individual caregivers coexist with operational weaknesses in staffing, medication administration, housekeeping, and communication. A few reviewers raised serious allegations such as theft and referenced bankruptcy or regulatory complaints; there are also mentions of concerns following a resident's death. These more severe claims are less frequent but salient and warrant direct inquiry with facility leadership and the local survey agency.

Recommendation for families: Sandy Lake appears to offer solid rehabilitative care and an active social program, and many families report positive, even excellent, experiences. However, the facility also exhibits variability in nursing coverage, housekeeping, and communication. Families who choose Sandy Lake should ask specific questions about current staffing levels, medication administration protocols, property-accountability procedures, and how the facility manages night coverage and agency staff. Families who are not able to monitor care closely or advocate proactively may wish to verify recent state surveys and speak directly with the therapy and nursing leadership before placement.
