    Attentive staff, warm memory-care atmosphere

I placed my loved one in the secured memory-care unit at this small, hometown-feel facility and have been very pleased. The staff are attentive, knowledgeable, and genuinely caring - activities (Cora/GREAT program) keep residents engaged and smiling, meals are good, and the rehab team is strong. Clean, updated building, prompt family communication, and a warm, supportive atmosphere - I'd recommend a visit.

    Summary of reviews

Overall impression: Reviews present Concho Health and Rehabilitation Center as a small, well-kept facility with a strong emphasis on a family-style atmosphere and personalized attention. Families and visitors consistently describe an environment with happy, engaged residents, a secure memory-care unit, and a compact campus that supports close staff–resident relationships. Facility upkeep and recent cosmetic updates are noted positively, and many comments recommend the center for both short-term rehabilitation and longer-term residency.

Care and staff: The dominant thread across reviews is the quality of direct care. Nurses, CNAs, and rehabilitation staff are repeatedly characterized as attentive, compassionate, and willing to go beyond basic duties to meet resident needs. Reviewers emphasize prompt responsiveness to questions and proactive family communication, with leadership described as accessible and supportive. This combination of clinical attention and open communication suggests consistent day-to-day care practices that families find reassuring.

Dining and services: Dining is frequently highlighted as a strength; meals are described as enjoyable and of good quality. The facility’s rehabilitation department also receives specific praise for its effectiveness in supporting recovery and discharge goals. The presence of a distinct memory-care unit and a secure, fenced perimeter are important programmatic features for families seeking dementia-specific safeguards.

Activities and social environment: Activity programming is noted as robust and engaging. Reviewers point to an active calendar, well-run events, and communal spaces such as a community room and gathering areas that encourage participation. The small resident census appears to facilitate social interaction and attentive activity staffing, resulting in visible resident engagement.

Facilities and management: The building is characterized as clean, well-maintained, and recently refreshed in places (new flooring, facelift). Management and administrative staff are described as approachable and helpful; communication channels to families are consistently cited as a positive. The combination of a small scale with attentive leadership contributes to the perceived ‘‘hometown’’ or family-like culture.

Notable patterns and considerations: The reviews are overwhelmingly positive with recurring praise for staff conduct, communication, meal quality, and activity engagement. Potential limitations inferred from the facility profile include constrained bed availability due to the small census (18 residents) and the likelihood that a smaller center will offer fewer on-site specialty services than larger institutions. Prospective families should confirm current bed availability, the scope and hours of on-site clinical specialties, and how the facility handles needs that require services beyond its scale. Overall, Concho HRC appears to deliver a high-touch, community-oriented model of care that many families find reassuring, particularly for rehabilitation stays and memory-care placement.

About Concho Health and Rehabilitation Center

Concho Health and Rehabilitation Center provides skilled nursing care, short-term stays, and long-term care, with a focus on health and rehabilitation services. The facility has 66 licensed certified beds and averages about 37 residents each day, so it's not too crowded, and people can usually get more personal care, though the nurse hours per resident each day are 3.01, which runs below the Texas average of 3.4, and the nurse turnover rate runs somewhat high at 66.7% compared to the state average of 51.8%. People in the facility can get services like physical, occupational, and speech therapy, wound care, and recovery help after surgery, as well as memory care, assisted living, vent care, in-house dialysis, podiatry, dentistry, optometry, hospice care, and support for heart and diabetes problems, with many specialists who come in to offer what folks need. They also have services to help veterans and folks needing stroke or neurological care, plus transportation, medication management, nutritional support, activities, laundry, housekeeping, beauty and barber care, and religious services, so residents have a good mix of essentials and comforts.

The center belongs to Creative Solutions In Healthcare and the direct owner is West Wharton County Hospital District, while Eden Ii Enterprises, LLC manages things since September 2022, with a variety of job openings and ways to give feedback through staff or online tools, though they require enabling JavaScript and cookies if using the website fully. Residents and families can get updates through an online portal, call in on a customer care line, or use the Care Portal and Payment Portal, and there's a confidential ombudsman service for handling any complaints about long-term care, which people might find reassuring.

People have found that the facility wants to create a home-like, caring atmosphere and tailor care to each person, but it hasn't been perfect, with 12 total deficiencies in their inspection reports, 2 involving infection control where there were issues with starting and following a proper infection prevention plan, and another about not always getting or storing food the right way, plus some quality of life and accident hazard issues in parts of the center. The inspection reports do track these things, which shows how the state tries to keep care accountable. People looking for details can visit the facility's website, use the FAQ or contact form, scroll through social media pages, or even look at other communities in the same network, so there's a good amount of information available for anyone who wants to look around before making a decision.
