    Compassionate attentive care for mother

My mother stayed here for six years and I'm very grateful - the staff (especially nurses Kevin, Jaymie and Ruben) were attentive, compassionate and knowledgeable, therapy and rehab were excellent, meals and activities kept her engaged, and the facility was always clean and welcoming. Communication was timely, care felt personalized and faith-based options and social worker Corey impressed us; I felt Mom was safe, valued and improving, and I highly recommend this place.

    Summary of reviews

Reviews of River Hills Health and Rehabilitation present a mixed but consistent pattern: the facility receives repeated praise for its rehabilitation program, several therapy disciplines, and many individual staff members who provide compassionate, person-centered care. Physical, occupational, and speech therapists are frequently described as effective and encouraging; families report good outcomes from short-term rehab stays. The facility also offers a range of ancillary services that families value, including on-site psychiatric support, grooming services, faith-based programming, and a variety of recreational activities that promote engagement.

At the same time, multiple reviews describe operational and clinical inconsistencies that affect resident experience and safety. Staffing levels are a recurring concern: reviewers mention inconsistent nurse and aide coverage, dependence on agency/travel staffing, and reduced staffing or services on weekends and holidays. These staffing patterns are linked to delayed personal care (including bathing), missed or late medication administration, and limited weekend therapy availability. Several comments raise questions about medication administration and documentation practices, and a subset of reviews describe documentation irregularities that prompted regulatory scrutiny and monitoring.

Dining and nutrition are also described unevenly. Some families find meals adequate or above average, while others report repetitive menus, temperature and quality issues, and an inability to reliably accommodate specific diets. Laundry and personal-property handling are an operational gap identified across reviews; missing or unreturned clothing and inconsistent management of residents' belongings are noted concerns. Cleanliness perceptions vary: many visitors note clean, well-maintained common areas and a pleasant admission experience, while other reports describe sanitation and odor concerns in particular resident areas and problems with linens and room upkeep.

Management and communication elicit polarized views. Several reviews single out administrators and specific staff (including social work and admissions personnel) for helpfulness, clear updates, and effective discharge planning; those same reviews contrast with accounts of spotty communication, delayed callbacks, and conflicts between staff and family advocates. Regulatory and documentation issues mentioned in feedback indicate that some reviewers experienced or observed lapses significant enough to prompt external oversight, which prospective families should clarify during their evaluation.

In summary, River Hills appears to offer strong rehabilitative services, many compassionate caregivers, and a range of resident-centered programs and amenities. However, the facility demonstrates operational variability in staffing, medication/documentation controls, personal-care routines, dining consistency, and property management. Prospective residents and families would benefit from focused questions at tour and admission: ask about current staffing ratios (including weekend coverage), weekend therapy availability, medication-administration and documentation practices, processes for laundry and valuables, and how dietary needs are accommodated. Those steps can help align expectations and identify whether the facilityโ€™s strengths match a prospective residentโ€™s clinical and daily-living needs.

About River Hills Health and Rehabilitation

River Hills Health and Rehabilitation sits on Bandera Highway in Kerrville, Texas, and offers assisted living, independent living, and continuing care all under one roof, plus skilled nursing care in a dedicated wing designed to make long-term residents feel right at home like family would, and you'll find a facility that's open every day, always with staff on hand, caring for people who may need help with things like bathing, therapy, or memory care, since there are programs in place for stroke, dementia, and people who need a lot of support, and the staff includes RNs, LVNs, CNAs, and NPs, working all through the day and night, with experienced hands in high acuity needs and long-term care, and they have both private and semi-private rooms so people can have space, along with things like a gym for rehabilitation, a beauty salon, wireless internet, and pets allowed, plus opportunities for folks to be around plants, animals, and children if they want, and there's always housekeeping and laundry done, meals served up in a fine dining way, and assistance with self-care tasks so people stay comfortable, but it's important to mention that the facility has had several citations, including immediate jeopardy-level deficiencies such as medication errors, lacking a licensed pharmacist, and failing to follow residents' care preferences, which led to a $74,915 fine in early 2025 for four deficiencies, and the most recent reports list a total of 54 deficiencies, including problems with infection control, so people considering River Hills should be aware that, while it's Joint Commission accredited and has some strong programs like wound care, pain management, podiatry, respite care, joint replacement rehab, and lots of therapies-physical, occupational, speech, and IV-it's seen more nursing staff turnover than the state average, and nurse hours per resident run a bit low, and yet there are many positive reviews from families who've toured or stayed, appreciating the personalized care approach and that the staff treat residents with respect, and it's nice too that people can get Medicare, Medicaid, and insurance to help with costs, and the management is by River Hills Chm LLC and the Uvalde County Hospital Authority, with oversight changing hands as recently as 2020, but there's no public info on direct owners, and families looking at this place should probably visit, since you're welcome to take a tour, and see for themselves whether the warm, inviting environment, comprehensive care, and rehabilitation options outweigh the concerns documented in inspections.
