    Spotless facility, excellent food, caring

I'm very happy with how my aunt is cared for here - the facility is spotless and well-kept, the food is excellent, and the staff (Mandy Koenig and Amy Tunnell included) are warm, professional and go the extra mile. Great activities, responsive administration, and genuine, compassionate care - I'd recommend this place for a loved one.

    Summary of reviews

The reviews for Mineral Wells Nursing & Rehabilitation present a mixed picture. Many families and visitors praise the facility’s physical environment and programs: the building is frequently described as clean, bright, and well maintained, with comfortable common areas such as a family room and front porch. The activities program is consistently highlighted as robust and varied (bingo, karaoke, outings, games, music and other social engagements), and rehabilitation services receive strong positive comments from those using them.

Clinical care and staffing elicit polarized impressions. Numerous comments commend nursing staff, CNAs, and specific leaders for compassionate, family-oriented care, prompt assistance, and professionalism; reviewers also note strong leadership in clinical and administrative roles. Several families described positive experiences with hospice and end-of-life care. At the same time, a distinct set of concerns recurs: inconsistent responsiveness from staff, variable conduct and tone during interactions, and communication gaps between the facility and families. There are specific operational concerns about safety practices (including fall-prevention) and about timely clinical follow-up after changes in condition.

Sanitation and direct-care consistency appear mixed. Many reviewers emphasize an overall clean and well-kept environment and praise housekeeping and dietary services, but others raise incontinence-care delays and room-level cleanliness inconsistencies, indicating variation across shifts or units. Dietary services are often singled out positively for quality and accommodation of specific needs.

Management and admissions processes are similarly mixed. Admissions and referral handling are described as efficient and helpful in several accounts, with named staff receiving praise for being responsive and attentive. Conversely, some families experienced abrupt or unfriendly interactions during admissions or visits and noted irregular phone responsiveness and difficulties getting timely answers to questions.

Notable patterns: the overall sentiment is polarized — a substantial number of reviewers report very positive, family-like care and excellent programming, while a smaller but serious subset describe operational failures that relate to safety, communication, and incontinence-care practices. There are also mentions of serious individual allegations and concerns following a resident’s death; these are isolated in the dataset but significant for prospective families to review further. Prospective residents and families should weigh the facility’s strengths in rehabilitation, activities, and dining against the noted variability in responsiveness and some operational practices, and consider meeting with clinical and administrative leadership to discuss fall-prevention, communication protocols, and routine care procedures before placement.

About Mineral Wells Nursing & Rehabilitation

Mineral Wells Nursing & Rehabilitation is a nursing and rehabilitation center with 109 certified beds and usually around 85 residents each day, and it offers both skilled nursing and long-term care for those who need help over a longer period, and sometimes people come for hospice or respite care if a family needs a break or someone's nearing the end of life, and the team there can handle special needs like ostomy care and dementia, and wound management is a service they provide along with therapies such as occupational, physical, respiratory, and speech to help with recovery and daily function, and they try to keep residents active with different recreational activities, plus the dining room has a gazebo, and the lobbies and resident rooms are decorated, and the facility provides a secure area if someone tends to wander and needs extra safety so families can have peace of mind, and if someone requires a van with a lift for a wheelchair, they have a large one for transportation needs.

Mineral Wells Nursing & Rehabilitation belongs to both Daybreak Venture and Creative Solutions in Healthcare, so it's part of a bigger group of care centers that focus on helping seniors and adults who need extra support. The nurse turnover of 48.1% is lower than the state average, which shows some stability, though total nurse staffing hours at 2.94 per resident per day fall below the state average of 3.4 hours so it's worth looking at how that might affect care. In the last 36 months, the facility had 12 deficiencies in inspection reports, which included issues like not always getting food from approved sources, not following all professional standards, and one deficiency about protecting residents' belongings and money, but there wasn't actual harm-just a chance it could become a problem, and there has also been an infection-related deficiency, so infection control has room to improve according to the reports. They do stay connected with families through email and phone, and there's a customer care line for any concerns, with a contact form to reach staff, so communication is available if there's something you want to ask or share. Memory care services are available, and the staff works to improve quality of care over time, though there aren't more details about other services or particular features listed at this time.
