    Clean facility with caring staff

I'm very pleased with Cedar Manor - the staff are caring, experienced, and the administrator clearly has a big heart. Since Creative Solutions took over the facility has stayed clean and well-kept, with ongoing renovations, hot fresh meals, engaging activities, and a knowledgeable therapy team that made a real difference. Staff consistently go above and beyond; I'd recommend touring the place and feel confident placing a loved one here.

    Summary of reviews

Cedar Manor presents a mixed but overall serviceable profile. The most consistent strengths across reviews are the caregiving and therapy teams: many families and residents described staff as compassionate, attentive, and willing to provide individualized support. The therapy program is frequently cited as effective at facilitating short-term rehabilitation and helping residents return home. Activities and social programming appear active and well-received, contributing to a welcoming atmosphere for residents.

Dining and facility condition are areas of notable variability. Several accounts praise hot, freshly prepared meals and an improved menu, while others describe inconsistent food quality. Renovations and facility upgrades are underway under new ownership and have been highlighted positively; common areas and rooms are described as clean and spacious in many reports, though some reviews pointed to intermittent room-maintenance and cleanliness issues.

Operational weaknesses recur in several themes. Communication and responsiveness are commonly mentioned concerns: families described ineffective front-office and business-office communication, unreturned phone calls, and delays in responding to resident requests or call lights. Staffing levels and staff availability were also raised as ongoing challenges, with reviewers linking coverage gaps to slower response times. Relatedly, some reviewers raised serious concerns about medication management and about staff conduct and the tone of interactions; a small number of allegations described use of medications to calm residents and unprofessional behavior that prompted family intervention. Safety patterns were also noted, including concerns about fall-prevention and transfer-safety practices.

Management and ownership change is a prominent pattern. Multiple reviews reference new ownership and a visible management presence, with several families reporting improvements since the transition and crediting leadership for renovation work and operational changes. At the same time, some reviewers described inconsistent issue resolution and concerns about staff morale and compensation, which may reflect transitional staffing and administrative challenges.

For prospective residents and families: the facility shows clear strengths in caregiving, therapy, and social engagement, and is actively investing in physical upgrades. However, variability in communication, staffing consistency, dining quality, medication oversight, and safety processes suggest areas to probe during a tour. Recommended questions include current staffing ratios, protocols for responding to call lights, medication-management policies, fall-prevention strategies, recent inspection or quality metrics, and examples of how complaint-resolution is handled post-ownership change. A direct tour and conversations with the administrator and therapy staff can help verify whether recent improvements have addressed the documented concerns.

About Cedar Manor Nursing & Rehab

Cedar Manor Nursing & Rehab sits in San Angelo, Texas, and has 166 certified beds, so it's a pretty good size place for skilled nursing, long-term care, and rehab services, and they do things like wound care, memory care, in-house dialysis, vent care, and veterans affair services, too, and you'll also find help with physical, speech, and occupational therapy for folks dealing with surgery recovery, neurological conditions, or other chronic health needs. Cedar Manor is part of the Creative Solutions In Healthcare network, so you might notice it's connected to other facilities, and the Ballinger Memorial Hospital District owns it, with San Angelo II Enterprises LLC managing things since February 2024, and their team aims to provide a home-like feeling, and they say they offer a lot of support and personalized care plans made for each resident, which sounds like they try to address unique needs room by room and person by person, including memory care. Now, the place has had 14 total deficiencies from inspections, including two about infection control, one of which got flagged as an F0880, with the inspectors saying there was a program problem but no actual harm, just the potential for some harm to a small group. They also had a deficiency about not fully honoring all resident rights, especially about dignified living and communication, so these are areas that might concern some families. When looking at staff, they average 3.35 nurse hours per resident per day and have a nurse turnover rate of 56.3%, so staff changes are something families might want to keep in mind, but folks have said the team is dedicated and tries to be compassionate. There's a handful of things residents can use, like transportation, religious services, barber and beauty shop, laundry and housekeeping, meals planned by culinary staff, and both a Care Portal and Payment Portal online for families, along with customer care lines, and options to get information, leave feedback, or keep up with a loved one's health. Cedar Manor Nursing & Rehab also stays connected with families through social media, helps with engaging activities, tries to make spaces feel like home, and has a 2.6-star rating from fifteen reviews, which might help inform someone looking for honest opinions about the experience there.
