Aspen Ridge Transitional Rehab sits about 2.4 miles from Midvale, Utah, and has 38 beds in a skilled nursing and rehabilitation facility that mostly focuses on short-term care, helping people recover after hospital stays or health setbacks, and the place has private suites with nice touches like private bathrooms, adjustable beds with pressure-relieving mattresses, televisions, private phones, and fast internet for people who like or need to stay connected during recovery, and if you need help with daily activities or medication management, trained staff is there around the clock and licensed nurses are on site 24/7, giving medical care and personal support. Besides its clinical services, Aspen Ridge offers physical, occupational, and speech therapy with individualized rehab programs set by a team that includes doctors, nurses, therapists, dietitians, and social workers who work together to check what each patient needs and then make a care plan that's just for them, aiming to help everyone regain as much independence as possible. There's a focus on mental health too, with programs for mental wellness, group and individual therapy, classes, life skills learning, and even relapse prevention if you're dealing with a substance use problem, along with special nutrition plans made by staff chefs and registered dietitians, including options for cardiac or diabetic diets. The dining rooms feel more like a restaurant, with fresh, seasonal meals, and a few other homey touches, like a fireplace lounge, library with a reading area and saltwater fish tank, beauty salon, and a courtyard patio with roomy seating where you or your visitors can relax a bit outside of therapy. Aspen Ridge is for-profit and part of Advanced Health Care, and is seen as an alternative to a typical nursing home, with all private rooms and a high priority on patient satisfaction and clinical excellence, and Medicare is accepted to help with costs. In government ratings, Aspen Ridge holds a 5-star score from the Centers for Medicare & Medicaid Services and is the third highest rated facility in Murray, with an average daily census of 36 residents and a nurse turnover rate of 29.3%, offering 5.75 nurse hours per resident per day. The place has had 6 documented deficiencies, including 3 related to infection control and some issues managing psychotropic medicines and following care orders, as shown in inspection reports. Short-term residents and those needing longer care both have access to family and alumni support programs, as well as inpatient and outpatient rehab options, and although Aspen Ridge aims to keep a supportive, comfortable environment that encourages recovery and independence, it's still a facility that does regular business, with staff changes and periodic challenges like most others in this field.

Excellent therapy, dangerous nursing care
I placed my mom at Aspen Ridge because the building, rooms, courtyard and therapy staff (Rob, Nole and the PT/OT team) were excellent and helped her mobility - the facility can feel clean, peaceful and well run. Unfortunately nursing and aide care was inconsistent to dangerous: my mom was left alone much of the day, sat in urine, wasn't changed, suffered bedsores and a bleeding ulcer, had oxygen and call-button problems, and was taken to the ER/ICU without family notification. Communication with family about timelines and incidents (including vomiting blood) was poor, overnight checks were lacking, and disability accommodations (cutting food, assistance eating) were ignored. Some nurses and aides were compassionate and knowledgeable (Dora and several therapists stood out), but there were multiple neglectful, unhelpful or lazy staff and worrying hygiene/medication practices. I'm grateful for the therapists' progress, but overall I cannot recommend the facility without major management and safety improvements.
