    Compassionate staff, excellent rehab care

I'm very pleased with Meadow Peak. The staff are compassionate and responsive - Nurse Esther and Director Lizzy went above and beyond - and the management is approachable. The facility is modern, spotless, bright with lovely mountain views, outdoor patios and great amenities. Therapy and nursing care drove real rehab progress, daily activities and family-focused support gave us peace of mind. Highly recommended.

    Summary of reviews

Overall impression: Meadow Peak presents as a modern, well-appointed assisted-living, memory care, and rehabilitation campus with many families praising its physical environment, therapy resources, and a number of individual staff members. Reviewers commonly highlight a bright, clean building with spacious private rooms, abundant natural light, multiple therapy gyms, and a range of amenities (movie theater, salon, library, outdoor spaces) that support social engagement and rehabilitation goals. For many residents โ€” particularly short-term rehab and memory-care residents โ€” therapy teams and caregiving staff produced measurable improvement and a strong sense of individualized attention.

Care and staff: The dominant positive theme is staff compassion and clinician dedication. Several reviewers named nurses, CNAs, and therapists who went above and beyond, and memory-care teams were frequently described as patient-focused and dignity-oriented. At the same time, there is a recurring pattern of operational weaknesses: extended call-light response times, inconsistent medication administration or documentation, and uneven clinical training for some staff members. These issues manifest as delays in assistance, variable therapy consistency, and concerns about staff competence with certain medical equipment. Language barriers between predominantly Spanish-speaking aides and English-speaking families were also cited as a source of miscommunication.

Dining and housekeeping: Opinions about dining are mixed. Some families described warm, well-presented meals and consistent hydration practices; others raised concerns about food quality, lack of fresh fruit/vegetables, and cold or unappealing meals. Housekeeping and laundry processes earned praise for cleanliness overall, but reviewers also noted inconsistent linen changes and occasional lost clothing, indicating variability in routine support services.

Activities and amenities: Activity programming and communal life are strengths. The facilityโ€™s amenities and scheduled events โ€” exercise classes, daily activities, family events โ€” were frequently mentioned as contributors to quality of life. Therapy gyms and active PT/OT programs support rehabilitation goals for many residents; some families experienced accelerated progress and positive functional outcomes.

Management, admissions, and discharge planning: Administrative experiences are mixed. Admissions and social services staff received positive comments for responsiveness in numerous cases, and certain leaders were singled out for effective communication. Conversely, there are repeated concerns about administrative follow-through: delayed insurance paperwork, capacity decisions that disrupted planned moves, and gaps in discharge planning that left families without timely home-health arrangements or necessary supplies. In some cases these coordination gaps coincided with avoidable care interruptions.

Safety and end-of-life care: Several reviews raised safety-related concerns such as fall-risk exposures, rooms not always configured with recommended mobility aids, and inconsistent transfer support. End-of-life and hospice communication also showed variability: while some families described respectful, comfort-focused transitions, others raised serious concerns about clinical coordination and communication with power-of-attorney representatives.

Recommendation guidance: Meadow Peak offers many structural strengths โ€” environment, amenities, engaged therapy teams, and examples of highly compassionate staff โ€” and can be an excellent choice for rehabilitation and memory care when those strengths are realized. Prospective residents and families should weigh those positives against noted operational patterns by asking targeted questions about call-light response expectations, medication-administration protocols, staff language support, staffing ratios during the intended level of care, and discharge/home-health coordination before admission. Doing so can help set clearer expectations and identify whether the facilityโ€™s current operational practices align with the residentโ€™s clinical and communication needs.

    About Meadow Peak

Meadow Peak Assisted Living, Memory Care, and Rehabilitation sits within the Summit Vista campus in the Salt Lake Valley and offers several levels of senior care, so people can move in at different stages of life and get more help as they need it, which makes things simpler for families, and with services like assisted living, memory care, skilled nursing, and rehabilitation, you'll find programs set up for many different needs, whether it's help with daily tasks, more involved care for memory loss, or physical therapy and recovery after illness or surgery. The facility houses 54 assisted living apartments-22 studio, 20 one-bedroom, and 12 two-bedroom ones-and there are also 20 memory care suites meant for folks with Alzheimer's, dementia, or other memory impairments, where they've set up a secure and supervised environment with life-enriching activities and memory-enhancing programs, and then there are also 63 skilled nursing suites with 75 beds, including 51 private and 12 semi-private suites, so there are options for both short-term and long-term care for people who need full medical support, and you'll find amenities like chef-quality restaurants, beautician and barber services, pharmacy fulfillment, parking, mail service, and daily help like laundry and housekeeping, all part of the setup to make life easier and more comfortable. The building has unique features, using exposed timbers and tall roof lines, with a modern three-story wing made from steel and concrete for earthquake safety, and the whole place is designed to be both sturdy and pleasant to live in. Residents have access to inpatient and outpatient physical, occupational, and speech therapy, so those recovering from injuries or illnesses can get care right on site without needing to leave, and all residents can join in social activities and programs if they want a more active lifestyle in either the independent living, assisted living, or memory care sections. Staff provides 24-hour assistance with wellness and personal care needs, and there are tailored programs for people at all care levels, including folks who just need a little help as well as those whose needs are more complex. All the suites, whether for assisted living, memory care, or skilled nursing, come with private or shared bathrooms, and the neighborhoods sometimes feature residential homes to give a smaller, homier feel with daily care for a handful of people, and with everything gathered on one campus, seniors have a chance to stay in one community even as their needs change, in line with the facility's goal to support dignity, comfort, and independence for all who live there.
