    Compassionate knowledgeable clean peaceful care

I'm very pleased with this facility - compassionate, knowledgeable nursing and therapy teams, attentive leadership and social services, helpful admissions staff, clean bright rooms, lovely gardens and koi pond, on-site dialysis, and meals tailored to diets. Residents seem happy, staff treat families like family, and I felt genuine peace of mind. Overall a solid, trustworthy choice for skilled nursing and rehab.

    Summary of reviews

Reviews for Fairfax Nursing Center are highly polarized. Many families and some clinical visitors praise individual caregivers, therapists, and the recreation/social services teams for compassionate, handsโ€‘on care and effective shortโ€‘term rehabilitation. At the same time a substantial number of reviewers describe recurring operational problems that affect clinical care, safety, and family communication.

Care quality appears variable across units and shifts. Positive accounts emphasize attentive nursing staff, knowledgeable therapists (PT/OT/ST), and successful shortโ€‘term rehab outcomes. Conversely, other accounts describe prolonged waits for assistance with hygiene and toileting, delayed medication administration, inconsistent implementation of physician orders, and examples of pressureโ€‘injury concerns. These contrastive patterns suggest the facility can deliver good care when staffing and coordination align, but that reliability is uneven.

Staffing and staff conduct are common themes. Many reviewers single out individual nurses, CNAs, and therapists as caring and competent; however, chronic understaffing, high turnover, and low morale are also repeatedly cited. Staff shortages are linked to long response times to call lights, reduced frequency of bathing and meal assistance, and difficulty scheduling specialty appointments. Several reviewers noted managerial communication failures and instances where family requests went unanswered for extended periods.

Rehabilitation and therapy receive mixed feedback. The facility has a number of positive reports about effective physical, occupational, and speech therapy and rapid functional gains for some residents. At the same time critics report short therapy sessions, limited weekend availability, and coordination problems between therapy and nursing that reduced the perceived benefit for certain patients.

Dining, housekeeping, and facility condition are areas of concern for many families. Numerous comments describe inconsistent meal temperature and quality, limited accommodation of dietary requests, and small portions. Sanitation concerns, localized odor issues, delayed maintenance responses, aging rooms (including basement rooms with limited natural light), and occasional pestโ€‘control concerns were reported. Other reviewers, however, noted clean and pleasant common spaces, attractive outdoor areas, and an overall orderly environment on some units.

Management and administrative practices are another source of variability. Positive notes point to helpful admissions staff, an effective socialโ€‘work presence, and examples of administrators resolving issues. Negative feedback centers on poor communication, weak discharge planning and careโ€‘transition coordination, billing and financialโ€‘administration concerns (including at least one allegation of improper account activity), and inconsistent followโ€‘through on complaints. A few reviewers flagged discrepancies between external ratings/marketing and onโ€‘theโ€‘ground experience, suggesting prospective families verify claims directly.

Activities and qualityโ€‘ofโ€‘life offerings are frequently praised. The recreation department, gardens, koi pond, social events, and petโ€‘friendly policies were repeatedly mentioned as strengths that contribute to resident engagement and morale.

Taken together, the pattern suggests Fairfax Nursing Center can provide highโ€‘quality, rehabilitative, and compassionate care in some circumstances, largely driven by strong individual staff members and therapy teams. However, systemic issues โ€” chiefly staffing shortages, inconsistent clinical responsiveness, facility maintenance needs, dining variability, and uneven management responsiveness โ€” create variability in the resident experience. Prospective families should conduct an inโ€‘person visit, ask specific questions about current staffing levels and turnover, review recent care plans and incident logs, confirm therapy schedules (including weekend availability), observe cleanliness and meal service, and request references from recent discharges to assess current operational reliability.

    About Fairfax Nursing Center

Fairfax Nursing Center sits in a comfortable spot for people needing both long-term and short-term care, and for over 50 years, family ownership has set a steady hand here, with Mr. Wayne Pflasterer, II, serving as the dedicated administrator; the facility runs with a total of 198 and 2 licensed beds, focusing on skilled nursing care and strong rehabilitation programs. The experienced staff-registered nurses, licensed practical nurses, and certified nursing assistants-handle care around the clock, covering needs for those who've had recent falls, brain bleeds, or big surgeries, and the physicians themselves run the place, which means medical care's always at the center. People get help with daily activities and specialized therapies like speech, physical, occupational, and even respiratory therapy, with personalized rehab programs adjusting as people get better or need more help. There's an emphasis on making folks feel honestly cared for, with the staff working in a supportive, warm environment, while amenities such as air conditioning, wheelchair accessibility, onsite parking, and restrooms make visits and living easier for both residents and families. Residents have access to social services, medication management, and special diets if needed, and the daily activities and recreation programs help keep people engaged and connected. The staff-to-resident ratio is strong, with care taken to match each person's needs, whether for short-term rehab with modern equipment or long-term supervision, and there's always someone there to guide new residents through admissions in a friendly way. Fairfax Nursing Center keeps track of its care quality with efficiency indicators, and the whole approach centers on making sure people feel supported, secure, and respected every day.
