    Warm, clean, dignified, responsive care

I'm very pleased with Southampton - a warm, clean, odor-free facility where staff treated my loved one with dignity and compassion. Nursing, therapy, and admissions were professional and kept our family informed; activities and attentive CNAs helped my parent settle in and make real progress. Administration and front-desk staff were responsive and supportive, giving us much-needed peace of mind.

    Summary of reviews

The review corpus for Southampton Rehabilitation & Healthcare Center is highly polarized, with many families describing strong positive experiences alongside accounts of significant operational concerns. Positive accounts emphasize compassionate bedside care, an effective therapy program that produces measurable recovery for short-term rehabilitation, attentive admissions and front-desk teams, and a robust activities program that supports social engagement. Multiple reviewers also note clean and welcoming common areas, active renovation efforts, and efficient transportation and discharge coordination.

Care quality appears uneven. Numerous reviews praise individual nurses, CNAs, therapists, and wound-care specialists for attentive, personalized care and good communication with families. At the same time, others describe delayed clinical response, missed changes in condition, and inconsistencies in medication administration and clinical documentation. Related operational themes include insufficient RN coverage at times, delays in assistance with activities of daily living, and variable follow-through on wound and skin-care plans. Families seeking placement should specifically inquire about current staffing ratios, wound-care protocols, and medication-safety procedures.

Dining, activities, and therapy are frequent strengths. Many families applaud varied menus, enjoyable special events (proms, balloon volleyball, life-enrichment outings), and a therapy team that is described as motivating and result-oriented. Conversely, food temperature and meal consistency are recurring complaints, and some reviewers cited missed or delayed meal service. Activities and social programming are consistently reported as engaging and helpful to resident morale.

Facility condition and management present mixed signals. Several reviewers describe a clean, well-maintained facility with visible improvement and renovation work; others cite odor concerns in certain units, maintenance delays (including elevator and room infrastructure issues), and inconsistent laundry or personal-belongings handling. Administrative leadership is likewise described variably: some families credit visible, hands-on directors and improved direction under new management, while others report unresponsiveness, departmental miscommunication, billing delays, and allegations of theft or mishandling of belongings. A small number of reviews reference regulatory inquiries and concerns following a resident's death; these are serious items that merit direct discussion with facility leadership and review of any public regulatory records.

Notable patterns: (1) Frontline staff and therapy receive frequent, specific praise and are the facility's primary strength; (2) operational consistency—staffing, meal service, sanitation, documentation, and property management—is the principal area of concern; (3) leadership and quality-improvement efforts appear to be changing over time, with both reports of improvement and reports of ongoing issues. For prospective residents and families, recommended due diligence includes an in-person tour, targeted questions about current staffing and clinical oversight, review of incident and regulatory histories, clarification of meal and laundry processes, and periodic checks on documentation and belongings. That approach will help balance the facility's clear strengths in rehabilitation and engagement with the operational risks highlighted in the review set.

About Southampton Rehabilitation & Healthcare Center

Southampton Rehabilitation & Healthcare Center sits in Richmond, Virginia, and you'll find it's a skilled nursing facility with about 195 licensed beds, though numbers given range from 72 up to 196 beds depending on where you look, and most days you'll see around 175 residents. The center provides many types of healthcare and rehabilitation services, including subacute rehab, pulmonary rehab with tracheostomy care, oxygen therapy, and other clinical care services like stroke recovery, cardiac care, wound care, diabetes care, orthopedic rehab, pain management, and bariatric care. Folks who need memory care, respite care, urgent Skilled Nursing Facility (Urgent SNF™), hospice, palliative care, or long-term care can get help here. They offer individualized care plans, post-discharge support, and a care navigation system so recovery is easier when coming from the hospital. Their staff includes specialty-program physicians such as pulmonologists and physiatrists, and they have registered nurses on-site around the clock. There's a 24-hour hospitality line for residents and families, and the staff encourages visits and staying connected.

If you're exploring activities and amenities, the place runs art and music therapy, sensory activities, gardening and horticultural therapy, aromatherapy, and family programs, including something they call the Family Matters Program and intergenerational events. Southampton uses technology like LifeLoop to help residents stay engaged and manage their daily experience. The building itself has an expansive gym for therapy, a beauty and barber salon, flexible dining, and family support resources. The Memory Care area stays secure, and staff provide treatments tailored to dementia.

They're associated with Marquis Health Services, but you can also find them listed as EnvoyCare, with websites at both www.southamptonhc.com and www.envoycare.com. Facilities like this one have to meet certain staffing standards, and Southampton reports about 3.36 nursing hours per resident per day, which is somewhat below Virginia's average of 3.8 hours, with a higher nurse turnover rate (about 63%) compared to the state average. Nurse staffing rules say at least 0.55 hours from registered nurses and 2.45 hours from aides per patient daily. Daily room rates for semi-private rooms run about $310, and there's no private room rate listed.

Recent inspection records show Southampton has had 50 deficiencies, including four related to infection control, some findings around safety and cleanliness, and a few deficiencies about resident assessment, care planning, and actual harm under the abuse and neglect category. There's a reported fine of $12,106 from a February 2024 complaint report documenting six deficiencies. Standard inspections have been behind schedule, with the last one over two years ago.

The administrator's name is given as Mr. Ron Tealakh on one listing and Raven Powel on another. Despite challenges, the center offers a broad range of rehab, nursing, and memory care services along with many special programs and therapies designed for older adults needing extra help.
