    Compassionate, responsive, clean, family-owned community

I placed my mom here and I'm very pleased - the family-owned staff (owner on-site) are compassionate, well-educated, polite and responsive, and genuinely receptive to care suggestions. The community is always clean, offers great meals, activities and a pet-friendly atmosphere my mom enjoys, and communication and personalized attention have been excellent; I would recommend it.

    Summary of reviews

Feedback for Crestview Manor reflects a mix of clear strengths and recurring operational concerns. Many accounts emphasize warm, compassionate frontline caregivers — particularly CNAs and some nursing staff — and describe a clean facility with respectful resident interactions. The facility's family-owned character and an on-site owner are noted as positives by families who experienced responsive attention, and front-desk staff and reception are frequently described as helpful. Dining and activities are definite strengths in multiple accounts: meals were characterized positively and the activity program, including pet-friendly opportunities, is viewed as engaging and beneficial to residents' quality of life.

Clinical quality and day-to-day care present a more mixed picture. Several descriptions highlight knowledgeable clinical staff and willingness to incorporate family suggestions, but there are repeated concerns about understaffing and staff workload that appear to affect responsiveness. Reviewers describe issues consistent with gaps in medication-management processes, delays in therapy and rehabilitation initiation, and variability in monitoring and safety practices. These operational weaknesses have been associated with adverse events in specific accounts, and they suggest the need for clearer protocols and oversight to ensure consistent clinical performance.

Management and communication are another area of contrast. Some families experienced polite, helpful administrators and direct access to management, while others described inconsistent administrative professionalism, turnover in care-coordination roles (including social work), and inadequate follow-up after incidents. Communication lapses between staff and families — particularly around incident notification and post-event updates — recur as a theme and have contributed to perceptions of inadequate incident response. A few families also indicated discrepancies between the facility's advertised rating and their personal experience.

For prospective residents and families, the facility appears to offer a warm, activity-rich environment with compassionate direct-care staff and solid housekeeping and dining services. At the same time, it would be prudent to evaluate clinical oversight, staffing levels, medication-safety practices, and the facility's incident-notification procedures during a visit. Specific questions to consider include typical staffing ratios for the unit, protocols for medication administration and reconciliation, expected timelines for therapy services, and examples of how families are notified and involved after clinical incidents. Observing staff–resident interactions during mealtimes and activities can help corroborate the positive aspects while targeted inquiries to management can clarify the facility’s approach to the operational concerns noted above.

    About Crestview Manor Healthcare Center

Crestview Manor Healthcare Center, sometimes called Crestview Nursing Home, sits out in Jane Lew, West Virginia, and folks will find that it's a family-owned place with space for 72 residents, offering many types of care for seniors and folks with serious medical needs. The center provides assisted living support, like help with daily chores, taking medicine, meal preparation, and general personal care, which helps people keep up with their routines when things get hard. Skilled nursing staff give round-the-clock care for people needing special attention after surgery or illness, including wound care, short- or long-term rehabilitation, and even occupational, physical, or speech therapy, all with the right equipment on hand. For those dealing with memory loss, including Alzheimer's or dementia, there's a safe memory care section, where staff offer specialized programs, activities to boost memory, and 24-hour supervision, and families can rely on the place to keep their loved ones cared for throughout the day and night.

Anyone recovering from substance use will notice there are also drug and alcohol rehabilitation programs, including withdrawal support and both inpatient and outpatient care for people who want to address addiction issues, which not all places have, and that kind of full care continues with options for hospice and palliative care, too. Crestview Manor also takes care of diverse needs in the community, with services for residents of many backgrounds, and both men, women, and non-binary people, with staff who speak a wide range of languages from English to Spanish, Russian, Chinese, and even sign language, which helps many people feel understood and safe. The center is linked with several big hospitals, so if someone needs medical specialties, or needs to see a provider connected with Adventist Health, Asante Rogue, or Providence Portland, they already have those relationships set up. There's often a focus on comfort and connection through worship services, Sunday preaching, Friday encouragement, and a steady stream of outreach activities and scheduled events-like one coming up on July 14, 2025-so days have both help and a bit of community spirit.

Crestview Manor handles primary care, therapy, and both mental health and substance use clinics, and they've got beds for people who need care for the long term or who are just there to recover and head home. Rooms and common spaces are set up to make daily life easier and more comfortable, and there are specialized programs and equipment aimed at helping residents keep or regain abilities, all while keeping everything straightforward, steady, and safe. They can't take new patients now, but the staff still work to offer what's needed for those who live there, and the wide variety of support means people can get different types of help all under one roof, in a way that feels settled and sincere, without fuss or show.
