    Compassionate, clean, well-managed care, recommended

I'm very grateful for the compassionate, attentive staff - long-tenured nurses and an excellent PT/OT team helped my dad regain strength and settle in. The facility is clean and well-managed, with engaging activities, pleasant courtyards, good meals, and staff who keep families connected via video chats and responsive communication. Overall a very positive, reassuring experience; I highly recommend them.

    Summary of reviews

Overall impression of Salem Center is a facility with strong front-line clinical and rehabilitation strengths alongside operational inconsistencies that affect family experience. Nursing and therapy teams receive repeated positive comments for compassion, attentiveness and clinical skill; families attribute measurable functional improvement to the physical and occupational therapy programs and describe successful short-term rehabilitation episodes. Direct-care staff are often characterized as helpful, respectful and team-oriented, and many families reported that bedside nurses keep them informed and provide considerate hands-on care.

Despite these clinical strengths, staffing patterns emerge as a persistent operational concern. Night shifts and aide coverage are described as thin at times, which families link to stress for residents and loved ones. Relatedly, reviewers describe occasional miscommunication between shifts or departments that can delay routine tasks or family updates. Administrative communication processes — including email responsiveness, scheduling of family conferences, and the handling of escalation concerns tied to power-of-attorney decisions — are described as uneven. These issues appear to drive the most intense dissatisfaction when they occur, even where clinical care is otherwise rated positively.

Dining and activities evoke mixed feedback. The facility’s activities program and outdoor amenities — including a large atrium and pleasant courtyards — are consistently praised for contributing to resident quality of life. However, dining experiences are described as variable; some families find meals well-presented while others characterize the food as bland or inconsistently seasoned. There are also practical gaps noted in virtual programming contingency (for example, limited backup plans for remote activities), which may reduce engagement options during staff absences or visitor-restriction periods.

Facility upkeep and infection-control practices show both strengths and areas for improvement. Many reviews indicate a clean, well-managed environment and appreciation for COVID-era safety measures that permitted visitation under controlled conditions. At the same time, isolated reports of illness and related communication gaps suggest that infection-control outcomes and family notification protocols could be strengthened and made more consistent.

In sum, Salem Center’s clear strengths lie in direct resident care and rehabilitation services, with many families expressing gratitude for compassionate staff and meaningful therapy outcomes. Prospective residents and their families should weigh those strengths against recurring operational issues — primarily staffing consistency, administrative communication, dining variability, and transfer/ escalation handling — and seek specific assurances from facility leadership about staffing ratios, family communication protocols, and contingency plans for activities and virtual engagement before placement.

    About Salem Center

Salem Center Genesis sits at 255 Sunbridge Dr in Salem, WV, and is run by Genesis HealthCare, which also oversees many centers across several states, so you get that kind of bigger company experience but on a smaller, community level. The place offers skilled nursing, short-term rehab, and long-term care, along with independent and assisted living for folks who can do some things themselves but need help with cooking, showering, or housework, and they've got a transitional care unit for people moving from hospital back to home. Doctors, nurse practitioners, and registered nurses stay on site, and they've got an attending physician, so people who need 24-hour medical care-including wound care, dialysis, pain management, or rehabilitation after surgery-get steady attention. Salem Center features a Pulmonary Program, neurorehabilitation, mental health and recovery programs, and special help for folks with dementia or Alzheimer's through a memory support service, and there are specialized units for different health conditions to match care with what the resident needs. For therapy, there's occupational, speech, and physical services to help folks recover skills, and programs like ACCELerate Rehabilitation and Powerback Rehabilitation to You. The center offers palliative care, hospice care, respite stays, and home care services, and folks looking for social connections find scheduled social, educational, and religious activities, along with activity rooms, a library, and a rehab gym. The staff pays attention to wellness and recreation, and there's an outdoor garden, patios, and even scenic views, and residents can get their hair cut in the beauty salon or barbershop, watch cable TV, and enjoy private or semi-private rooms-all fully air-conditioned with individual controls. Some rooms have fireplaces in the living area, and the whole place is smoke-free, which goes well with the open, homelike feeling. There's also a communal dining hall, private dining for families, in-room dining, and housekeeping and laundry come with the room, plus mail and newspaper delivery for those who want to stay up-to-date. Safety measures include an alarm system for residents with memory problems, and staff handle medication, dietary needs, and help with planning discharges. Residents get case management, podiatry, and vision care, too, and the center offers things like audiology and psychiatric services, which means families can feel a bit more at ease if someone has special medical needs. The administrator, Eilish Burner, leads the team that does the day-to-day, and pets are allowed, so people don't have to give up a companion. For transportation, the facility coordinates rides for residents, which helps with outings or appointments. Salem Center runs as a privately owned nursing home and keeps a friendly, community feel, supporting recovery, wellness, and long-term living, and uses some unique program names and setups to match care with each person's situation-and there are a lot of health and comfort services here without too much fuss about appearances.
