    Attentive compassionate staff, successful rehab

I was very pleased with the attentive, compassionate staff-nurses, CNAs and therapists who listened, pushed recovery, and made short-term rehab successful. The renovated single-story building is clean and sunny with a lovely garden and engaging activities; the director and resident council were proactive and basic services like laundry were prompt. My family felt supported (special thanks to Cindy and Romana, RN); I'd recommend Edenbrook and will miss the caring team.

    Summary of reviews

The reviews portray a facility with a clearly mixed profile: strong rehabilitation and therapy services and a number of staff members who are described as caring and attentive coexist with operational and clinical weaknesses that have meaningful impact on some residents and families.

Care quality: Therapy (physical and occupational) is repeatedly described as a strength — therapy teams are characterized as effective, goal-oriented and often instrumental in successful short-term rehab outcomes. Nursing and direct-care staff are frequently praised for compassionate interactions and dignity-preserving assistance. However, reviewers also describe significant variability in clinical practices. Concerns include medication-administration errors or inconsistencies, gaps in clinical oversight for complex conditions (for example, stroke and heart-failure monitoring), and inconsistent attention to routine personal-care tasks. These patterns suggest that clinical skill mix and supervision may be uneven between shifts or units.

Staff and training: Many families highlight specific caregivers and administrators who were helpful, compassionate, and proactive. At the same time, other accounts indicate training and accountability gaps — staff availability is inconsistent, some shifts appear understaffed, and there are described instances of poor responsiveness. Taken together, these comments indicate a workforce with committed individuals but uneven deployment, supervision, and training across the facility.

Dining and housekeeping: Comments about meals are mixed; several reviewers note small portions and disappointing food quality, while others describe acceptable dining experiences. Housekeeping and sanitation are another area of divergence: portions of the facility are described as recently remodeled and clean, yet other accounts raise sanitation concerns, pest-control issues, and inconsistent room-cleaning practices. These contrasts suggest variability in housekeeping standards across units or time periods.

Activities and environment: Activity programming and resident engagement receive consistent positive notice. The facility’s activities staff, resident council, and outdoor spaces (garden, sunny rooms, single-story layout) are seen as meaningful quality-of-life assets, especially for longer-stay residents.

Management, communication, and logistics: Communication with families and between clinical teams is a recurrent area of concern. Reviewers describe difficulty obtaining timely updates, inconsistent documentation practices, and a complaint process perceived as slow or unresponsive. Transportation and discharge logistics are also problematic for some families: limited transportation options, long waits for pickups, and unclear discharge/re-entry planning are mentioned frequently. Additionally, there are accounts of personal belongings being mishandled or lost, which points to weaknesses in property-management processes.

Notable patterns and guidance: The overall picture is polarized — many families report very positive, even exemplary, rehab experiences and caring staff, while others describe serious operational lapses including medication-management issues and post-acute care coordination failures. There are also isolated but serious concerns, including accounts linked to adverse outcomes and a facility response perceived as inadequate; such matters suggest prospective families should pursue direct, specific inquiries.

For families considering this facility, recommended questions include: how the facility manages medication safety and clinical oversight (especially for complex diagnoses), staffing levels by shift and unit, housekeeping and infection-control protocols, property-tracking procedures, transportation scheduling, and how discharge/re-entry planning is handled. Visiting in person and meeting therapy, nursing leadership, and the activities coordinator can help clarify whether the strengths highlighted by many reviewers align with your expectations and care needs.

    About Edenbrook Oshkosh

Edenbrook Oshkosh sits at 1850 Bowen St in Oshkosh, WI, and serves as a nursing home under the Eden Senior Care Network, offering both short-term rehab and long-term care for up to 110 residents, though the daily number usually averages around 69. Channie Lifsics, Dovie Mauer, and Pamela Rice manage the facility, while direct ownership includes Maxim Stesel, Mordechai Polstein, Dan Feinstein, Channie Lifsics, Dovie Mauer, and Rostislav Pukshansky. The place has a long list of specialized services-from 24/7 nursing and physician care to memory care, rehabilitation, physical and occupational therapy, and speech-language pathology, as well as family caregiving resources, all aimed at frail elders needing more support.

The rooms include private rehab suites, wheelchair and handicapped-accessible options, and daily comfort features like outdoor courtyard, patio access, complimentary cable TV, Wi-Fi, and an in-room dining service. Residents get help with daily needs and enjoy social and recreational activities such as arts and crafts, pet visits, on-site banking, beauty salon/spa services, family gatherings, and community outings using the facility's transportation. There's a well-equipped library and a communal dining area, with culinary services managed by registered dietitians who provide customized menus and wellness programs that fit each person's health requirements. Staff aim to build a community that feels safe and familiar, using personalized care plans and offering a say through a resident council that meets each month. A therapy team provides care with a firm but friendly approach, and leaders try to involve residents in different programs or activities as the months go by.

Edenbrook Oshkosh has amenities that make daily life easier while attending to comfort and emotional well-being, focusing on both primary and specialty medical services using holistic methods. However, state inspection and complaint reports, including a notable one from March 26, 2025, have pointed out many issues-including 21 total deficiencies, with 3 tied to infection control, and concerns involving resident rights, infection prevention, and failure to provide a safe and homelike setting or proper mobility care. Nurse turnover sits higher than state averages at 57.7%, and nurse staffing falls short at 3.42 hours per resident per day, below the state's average of 4.1. Even so, many find the staff knowledgeable and courteous, and the therapy workers professional. The facility averages a 4.0 out of 5 review score from residents and families who want a blend of care services, a sense of community, and a place where loved ones can join activities or relax in the courtyard. The environment is secure for memory care needs, and disposable medical supplies and nursing home specialties are part of their services, but families should know about past inspection citations related to resident safety and infection prevention.
