    Excellent rehab friendly attentive staff

I stayed here after a hip fracture and was very pleased with the care. From admissions to discharge the staff were friendly and caring - nurses, CNAs and aides did their best - and the PT/OT therapists were outstanding and helped me regain my independence. The facility is busy but clean, residents are well cared for, activities are nice, and I got regular updates from staff who remembered visitors by name. I would recommend this place.

    Summary of reviews

Reviews of Crossroads Care Center of Pewaukee are highly polarized. Many families and residents praise the facility's rehabilitation program and several front-line caregivers, but an extensive set of operational concerns appears repeatedly and affects perceptions of overall quality. Prospective families should weigh the strong therapy offerings and individual staff strengths against systemic issues identified in multiple areas.

Care quality: The facility's strengths are most apparent in its skilled physical and occupational therapy teams and in cases where individual nurses and CNAs provide attentive care. Reviewers describe measurable functional progress for some residents. At the same time, there are recurring concerns about basic personal care and clinical management: inconsistent bathing schedules, delays in addressing continence needs and linen changes, wound and pressureโ€‘injury management gaps, and medication-administration or documentation lapses. A small number of reviewers described serious clinical escalation issues; one review referenced an allegation of an unmonitored elopement with delayed staff awareness and subsequent concerns following a resident's death. These more severe items underscore the importance of asking about clinical protocols, incident reporting, and oversight during a tour.

Staff and responsiveness: Many comments highlight caring, personable staff members who provide meaningful support and clear communication to families. Conversely, chronic understaffing is a consistent theme and appears to drive slower call-light response times, episodes where residents wait for assistance, and reports of staff congregating away from resident areas. There are also multiple remarks about variable staff conduct and tone, including instances where families found communication unsatisfactory. Staffing levels and supervision seem to be a principal driver behind many operational shortcomings.

Dining and nutrition: Feedback on dining is mixed. Some residents receive appropriate accommodations (for example, lowโ€‘sodium diets), but others cite inconsistent food quality, cold or unappealing meals, and deviations from posted menus. Meal-service continuity and tray handling were also noted as problematic in several accounts. Families should ask about kitchen oversight, menu cycles, and meal-delivery procedures.

Facilities and housekeeping: Reviews indicate uneven facility maintenance and housekeeping performance. Specific operational concerns include sanitation inconsistencies in resident rooms and common areas, odor concerns, pest-control issues, malfunctioning hot-water or shower scheduling problems, and general building disrepair. Laundry handling (missing or shrunk clothing) and shortages of supplies such as towels were also mentioned. These items suggest variability in environmental services and supply-chain management.

Management and communication: Several reviewers expressed frustration with administrative responsiveness, including social-worker communication, discharge coordination, and billing or ambulance/insurance disputes. Ownership changes were referenced as a factor in perceived service decline by some families. While some staff and therapists were noted as excellent at keeping families informed, others found communication fragmented and difficult to navigate.

Notable patterns and takeaways: The strongest, most consistent positive is the rehabilitation program and individual caregivers who go above and beyond. The most consistent negatives are operational: understaffing, inconsistent personal-care routines, slow response to assistance requests, medication and woundโ€‘care process gaps, and environmental maintenance and sanitation variability. Prospective residents and families should conduct an in-person tour with targeted questions about staffing ratios, call-light average response times, woundโ€‘care protocols, medication administration procedures, incident/elopement prevention and monitoring, laundry processes, and recent state inspection or corrective-action history. If short-term rehab is the priority, the facility's therapy services may be a strong fit; for long-term placement, verify that housekeeping, nursing coverage, and clinical oversight meet your expectations before deciding.
