    Caring staff, excellent therapy, home

I had a very positive stay - the staff were outstanding and genuinely caring, especially Cherika. Therapy and nursing were top-notch (my mobility improved), meals were delicious, rooms clean, and activities and transport were reliable. I felt at home and would highly recommend this facility.

    Summary of reviews

Overall sentiment across the reviews is mixed but leans positive regarding personal interactions, therapy outcomes, and dining, while raising significant concerns about safety and staffing.

Care quality and therapy are consistently praised. Multiple reviewers highlighted effective rehabilitation services, specifically occupational and physical therapy, and described the therapy team as a major strength. Reviewers reported good functional outcomes from rehab and reliable follow-through for scheduled care. Nursing and caregiving staff are repeatedly described as caring, compassionate, and easy to relate to; several comments single out the social worker positively. This suggests that when staff are present and engaged, residents receive attentive, person-centered care.

At the same time, safety-related issues are a recurring and serious theme. Multiple falls with resultant bruising were reported, and reviewers explicitly linked these incidents to an insufficient staff-to-patient ratio. There are also reports of delayed transfers to the emergency room in urgent situations. These safety incidents and delays suggest operational vulnerabilities that can undermine otherwise good one-on-one care and good rehabilitation outcomes. The combination of staffing shortfalls and emergent-care delays is the most significant negative pattern in the reviews.

Facilities and environment receive mostly neutral-to-positive remarks. Physical features such as tall ceilings and hospital-style double rooms were noted, which may contribute to a more open, clinical atmosphere. However, the facility does not have a dedicated Alzheimerโ€™s or memory care unit, a limitation for families seeking specialized dementia services.

Dining and activities are clear strengths. The kitchen staff and a dedicated cook are repeatedly lauded, and food quality is described as excellent. Recreational offerings are mentioned (for example, puzzles), indicating some level of engagement and social activity for residents. Friendly residents and staff contribute to a welcoming social environment according to reviewers.

Management and coordination show mixed signals. On routine schedules and planned care, reviewers noted reliable follow-up and coordination. Conversely, the organization appears to struggle under emergency conditions and with adequate staffing levels, pointing to unmet needs in staffing strategy and incident response protocols.

In summary, Sage Rehabilitation Hospital SNF is viewed favorably for its rehabilitation programming, compassionate staff, and strong dining/activities offerings. However, safety and staffing concernsโ€”specifically multiple fall incidents, insufficient staffing ratios, and delayed emergency responsesโ€”are material issues raised by reviewers and represent the primary risks to overall quality. Prospective residents and families should weigh the strong therapeutic and interpersonal strengths against these operational safety concerns, and inquire specifically about current staffing levels, fall-prevention measures, and emergency transfer procedures before deciding.

    About Sage Rehabilitation Hospital SNF

Sage Rehabilitation Hospital SNF is a skilled nursing facility that helps people recover from illness or injury with a focus on getting them back to their homes, work, and communities, and you'll find their team of doctors, nurses, therapists, dietitians, and case managers work together to provide care that fits each patient's needs, and they've got services for rehabilitation, medical care, wound care, and long-term acute care all under one roof at different sites in Baton Rouge and Lafayette, with inpatient and outpatient options depending on what your situation calls for. Sage's therapy departments offer physical, occupational, and speech therapy in both one-on-one sessions and group settings, using methods like Neuro-Developmental Treatment for stroke patients and the LSVT programs for Parkinson's disease, and if you need help with things like cognitive skills, swallowing, speech, memory, or even driving after a brain injury, they've got programs set up for that, including assessments and therapy for voice and language problems with tools like AmpCare and VitalStim. People who need more time to heal can stay at their 25-bed skilled nursing facility or the 20-bed inpatient rehab hospital, with a choice of private or semi-private rooms, and folks who are able to participate in three hours of therapy a day, 5-6 days a week, get specialized inpatient therapy, while those needing less intensive support can get up to 2.5 hours a day, and all of these patients get round-the-clock nursing care and daily checks by physiatrists and internal medicine specialists. There's also a Day Neuro Program for folks recovering from brain problems, with therapy, community outings, leisure, and education groups, and they provide transportation for eligible patients to certain programs. People struggling with hip fractures, joint replacements, arthritis, amputations, burns, strokes, Parkinson's, brain injuries, vision issues, spinal cord injuries, and other issues can get treatment here, as well as wound care, nutritional services with dietitian-approved menus, and activity therapy, and Sage also supports emotional, spiritual, and physical healing with their programs, doing their best to treat people with dignity and respect. They help plan for returning home or moving to a less intensive care setting, and their admission office can help you figure out what level of care or program is right for your situation; you should know they accept Medicare, Medicaid, most managed care, and commercial plans. Facilities are located at several addresses in Baton Rouge and Lafayette, with Sage Outpatient Services at 8313 Picardy Ave, inpatient long-term care at 8225 Summa Ave Building B, and inpatient rehabilitation at 8000 Summa Ave in Baton Rouge, and they also have outpatient wound care and case management services, so if you need therapy after an accident, illness, or surgery, or if you need ongoing skilled care while you work toward getting stronger, Sage tries to cover a lot of ground with options for both short-term recovery and longer-term needs.
