    Caring staff, clean comfortable facility

My mother moved in recently and I've been very pleased with the attentive, compassionate staff-CNAs, nursing and rehab therapists all go above and beyond. The facility is clean, secure and comfortable with nice outdoor spaces, an on-site beauty shop, rec room, library and two-bedroom options; meals, snacks and activities are well run and staff helped with memory-care placement and financing. Overall a warm, family-like atmosphere I would recommend.

    Summary of reviews

Reviews paint a mixed but informative picture of Adams Farm Living & Rehabilitation. Strengths center on direct-care services: many families and residents characterize nursing, CNA, and therapy teams as compassionate and hardworking, with physical and occupational therapy repeatedly identified as a strong point. Reviewers praise rehabilitation outcomes and the facilityโ€™s willingness to assist with transitions such as memory-care placement and financing. The overall atmosphere is often described as family-like and supportive.

Staffing and care responsiveness are the most consistent areas of concern. Multiple accounts indicate the facility operates with limited staffing at times, producing delays in routine care and response to call bells. These operational gaps have translated into intermittent lapses in incontinence care and inconsistent bathing schedules for some residents. There is also variability in staff professionalism and communication tone: while many staff members are described as attentive and respectful, other reports describe unhelpful or brusque interactions from nursing personnel. In addition, reviewers note occasional delays in medication delivery and uneven clinical follow-through.

Dining, activities, and amenities receive generally positive comments. Food quality is described as good or decent, and the facility appears responsive to dietary restrictions. A variety of activities โ€” music, library visits, and recreational programming โ€” are available and many residents participate. The campus and common spaces are viewed positively: reviewers highlight a large, attractive campus, a pleasant courtyard with benches and a gazebo, and on-site amenities such as a cafeteria, recreation room, and beauty shop.

Facility condition and administration show a split impression. Many visitors find the environment clean and comfortable despite an older physical plant; several reviewers describe the building as dated but well-maintained. Administrative follow-through shows room for improvement: transfer paperwork and other records have, on occasion, not been sent as requested, and communication can be a problem for residents with hearing loss or when families seek timely updates.

Overall, prospective residents and families should weigh the facilityโ€™s strong rehabilitation services, caring staff members, comprehensive activities, and campus amenities against recurring operational challenges related to staffing levels, responsiveness, and administrative consistency. For those prioritizing therapy, a supportive community atmosphere, and campus amenities, Adams Farm appears to offer clear strengths; for those for whom consistently timely daily-care assistance and administrative reliability are critical, the patterns in reviews suggest areas to probe further during a visit and in pre-admission discussions.

About Adams Farm Living & Rehabilitation

Adams Farm Living & Rehabilitation sits at 5100 Mackay Road in Jamestown, North Carolina, and this place is a 120-bed skilled nursing facility that provides care for people with complex medical needs, those recovering from surgery or illness, and those who need extended nursing care, and they've also got a Physical Medicine and Rehabilitation Clinic right there, which means people can get their therapy and rehab without having to go far, and you might notice how things here revolve around supporting residents' recovery, with a team of physicians, nurses, physical therapists, occupational therapists, speech therapists, nurse technicians, and registered dietitians, all working together to help each person get better, and, you know, Adams Farm's rehabilitation program is pretty thorough-rehab's available every day of the week for folks trying to bounce back from surgery, illness, or injury, and they've got services for short-term stays, long-term care, assisted living, respite care for folks who normally live at home but whose caregivers just need a bit of a break, and end-of-life care when that's needed, so there are options whatever someone's situation might be.

This place falls under Century Care Management, and you'll find that the administrator-Sherri Ingram-Bass, LNHA-manages things day to day, with the admissions office overseen by Nikki Tsujii, and you can tell they've got a focus on making sure residents feel comfortable, with private rooms for folks doing transitional rehab, a Rehabilitation Suite for therapy, and a general atmosphere that's got a home-like feeling, warm and inviting rather than cold or clinical. There's a multidisciplinary team keeping up their licenses and certifications with regular training. Residents can join therapeutic activities and social programs that aim to encourage personal growth, social connections, and time spent in nature, since the staff here seems to believe that staying active and engaged helps with healing and overall happiness.

Adams Farm Living & Rehabilitation is Medicare and Medicaid certified, and as part of their service, they try to meet each resident's needs and wishes in a personal way, but also, they make sure everyone-including folks who are deaf or hard of hearing-can participate fully; they have free auxiliary aids like sign language and oral interpreters, and TTYs, to help with communication. Religious services are available, and if someone needs help with wound care or pain management, that's handled here as well, all supervised by trained and professional physicians. There's even a connection with Wesley Long Nursing Center, which offers laundry, dental, and podiatry services when those are needed, so residents don't have to leave for everyday needs.

What makes Adams Farm different, maybe, is their focus on both medical recovery and a sense of community, where people are encouraged to be active, find comfort, and reach their best possible recovery level, all in an environment that feels less like a hospital and more like a place you'd want to call home, or at least somewhere where you're treated with care and respect, and that's about what you can expect if you spend time at this facility in Jamestown.
