    Compassionate staff and excellent therapy

I'm very pleased with the compassionate, around-the-clock care-nurses and one-on-one staff like Keisha and Kate are attentive and treat my loved one with dignity. Rooms (single or shared) include private bathrooms and in-room amenities, and the facility is cleaned daily. Therapy and rehab (PT/OT/Speech) are excellent, and activities like arts & crafts, bingo and haircuts keep residents engaged; cafeteria meals are flexible. Overall a very good, caring place I'd recommend.

    Summary of reviews

Ashland Nursing and Rehabilitation Center presents a mixed profile. Many comments praise front-line caregivers and therapy staff โ€” reviewers frequently highlight compassionate CNAs and nurses and strong physical, occupational and speech therapy services. The facility offers practical features that appeal to families and residents seeking rehabilitation and basic comfort: a single-floor layout, private in-room bathrooms, basic in-room amenities (TV, dresser, air conditioning), cafeteria-style dining, an outdoor courtyard, and a program of activities such as arts and crafts, bingo and haircuts. Some families also note the facility accepts veterans and offers both single and shared room options.

At the same time, a recurring operational pattern is concern about staffing levels and the effects on day-to-day care. Multiple accounts point to chronic understaffing that correlates with delayed assistance, inconsistent bathing and dressing schedules, slow call-button response times, and variability in care quality depending on shift. Several reviewers described uneven supervisory responsiveness when escalations occur; this translates into family frustration about communication and difficulty obtaining timely updates about appointments and care plans.

Sanitation, housekeeping and property-management issues appear across reviews as another consistent theme. Comments reference sanitation and odor concerns in resident rooms and some common areas, inconsistent availability of linens and soaps, missed housekeeping tasks, and instances of misplaced clothing or personal items. Related quality-control issues also appear in dining and medication processes: while the facility provides cafeteria meals and some flexible options, reviewers describe missed or cold meals and unreliable medication timing in certain circumstances.

The physical plant and environment are described as serviceable but dated. Reviewers note an older interior, paint and flooring that suggest deferred maintenance, even as certain clinical spaces (for example, the therapy room) are praised as clean and appropriately sized. Safety and fall-prevention practices are an area of concern for some families; a number of accounts describe safety incidents or falls, which raises questions about oversight and transfer protocols.

For prospective residents and families: weigh the facility's clear strengths in rehabilitation therapy and the presence of caring direct-care staff against recurring operational weaknesses in staffing, housekeeping, medication and meal consistency, and management responsiveness. When evaluating Ashland, consider visiting at different times of day (including evenings/weekends), asking for current staffing ratios and supervision structure, inquiring about housekeeping and linen schedules, reviewing recent inspection results, and discussing individual fall-prevention and medication-administration protocols with management.

About Ashland Nursing and Rehabilitation Center

Ashland Nursing and Rehabilitation Center sits at 906 Thompson Street in Ashland, Virginia, and right now isn't taking new patients, but it's a big place with 190 licensed beds, so there's room for lots of residents when they do, and most of the staff speak English, which helps with clear communication every day. As a skilled nursing and rehabilitation center, Ashland belongs to the Consulate Health Care family and serves as both a place to recover and a place to live for those who need support, so if someone needs care for a longer stretch or needs specialized care after a hospital stay, that's what happens here. They accept both long-term residents and people who need help getting back on their feet, offering skilled nursing, post-acute care, rehabilitation therapy like physical, occupational, and speech therapy, plus care plans shaped for each person since not everyone needs the same thing. The staff, under the direction of Ms. Karen Brandt, work to treat residents as if they're family, and with 88.9 full-time staff, there's a good number of hands to help around the clock since this place never really closes, and care is available 24 hours a day, which gives some comfort.

Since some folks need extra support, the center's got a 30-bed secured unit where people with Alzheimer's or dementia can get specialized care, and that part of the building helps keep everyone safe. When it comes to rooms, there are private options at a daily rate of $365 or semi-private rooms at $334 a day, so there are choices depending on what someone wants or needs, and it's included on Virginia's list of long-term care facilities, which keeps things above board. The staff provides social activities and special clinical programs to keep everyone active, trying to take care of the whole person-body, mind, and spirit-in quiet ways that make sense for each individual. Ashland Nursing and Rehabilitation Center is proprietary, part of the Virginia nursing facilities region, and it quietly does the work of long-term care for folks who need it.
