    Impeccably clean welcoming secure community

I moved in recently and I'm very pleased. The facility is beautiful, impeccably clean, peaceful and well kept, with comfortable rooms. Staff at every level are welcoming, kind and knowledgeable - leadership and kitchen staff are excellent. The secured dementia wing and thoughtful activities (crocheting, knitting, board games) make this feel like a hidden treasure I happily recommend.

    Summary of reviews

Reviews for this facility are markedly polarized, with a mix of strong praise and serious operational concerns. On the positive side, multiple accounts emphasize a clean, well-maintained physical environment, secure memory-care accommodations, and a generally peaceful campus. Several reviewers describe staff and leadership as kind, knowledgeable, and responsive, and highlight a range of recreational options (crocheting, knitting, board games, organized events) that contribute to resident engagement. Dining and kitchen staff receive favorable mentions from some families, and room size and general maintenance are frequently described as adequate.

Counterbalancing those positive impressions are a number of substantive care and operational issues. A recurring theme is inconsistent clinical care, especially around skin and wound management and post-surgical follow-up; reviewers also reported unexplained weight loss and other clinical-oversight concerns. Families and reviewers described delays in staff response to resident needs and variability in supervision and staffing patterns, which appear to affect continuity of personal care. Communication gaps with family members about changes in condition are a notable concern.

Sanitation and odor-control are described inconsistently across accounts: some reviews report an immaculate, odor-free environment, while others identify sanitation or odor-control issues in parts of the facility. Dining quality is similarly mixed — kitchen staff are praised by some, but food consistency and satisfaction vary. Reviewers also raised operational concerns such as transportation safety, limited exercise or gym facilities, and the presence of in-room cameras, which prompted privacy and monitoring questions. There are also mentions of an ancillary-fee approach for resident communications (phone charges) that families may want to clarify up front.

Finally, management and workplace culture appear bifurcated in the feedback: several reviewers praise strong executive leadership and organized operations, while others allege negative staff culture and describe retaliatory dynamics; these latter claims have been characterized as allegations and would warrant follow-up. Given the divergent experiences reported, prospective residents and families should plan an in-person tour, ask for current staffing ratios, wound- and dementia-care protocols, recent inspection records, incident-response procedures, and clarity on any ancillary fees. Those steps can help reconcile the polarized reports and assess whether the facility’s strengths align with an individual resident’s clinical and social needs.

About Twin Oaks Nursing Home Inc

Twin Oaks Nursing Home Inc, managed by Ball Healthcare Service, Inc. since July 2003 and owned by Clarence Ball, is a skilled nursing facility in Alabama with 131 certified beds and caring for an average of 121 residents each day, and when you look around you see it offers both semi-private and private rooms designed to help residents feel at home while receiving 24-hour skilled nursing care, but it's also worth knowing that inspection reports show six deficiencies, including violations in infection control and pharmacy services like drug labeling and storage, so some areas need improvement. The nurse turnover rate is 40.3%, which is lower than the state average, though the nurse hours per resident per day at 3.37 fall under the state average of 3.9, and when folks talk about the care, they'll mention the presence of licensed and registered nurses alongside a Medical Director and Nurse Practitioners, ensuring oversight for both routine and specialized care, including cognitive care units, medical sub-acute care, hospice services, ventilator and tracheostomy care, and advanced wound management. They provide occupational, physical, speech, and respiratory therapies; there's outpatient therapy too, plus complex wound care and long-term restorative nursing care, and residents have access to dietary management with appetizing therapeutic diets tailored to different needs, along with support from social services and activities staff, so there's a lot going on to help folks feel engaged and well looked after. The facility accepts long-term care insurance, offers daily meals and dining services, and keeps things running all the time, twenty-four hours a day, every day. When it comes to amenities, folks can use the game and activities room, guest parking, wifi, dining room, fitness center, salon and barbershop, housekeeping, kitchenettes, safety and handicap features, sprinkler system, washers and dryers, cable TV, and even laundry and maintenance help, while you'll also see personal care services like medication support, nurses, podiatry, occupational therapy, dressing and grooming, walking or wheelchair assistance, bathing, and toileting, as well as transportation, and the activity schedule often includes social events, arts and crafts, health and wellness programs, so residents have a variety of options to stay occupied and social. While there isn't much detailed information about other unique services or specific terms, the staff, which numbers around 1,250, benefits from in-house programs and scholarships for continued education, and the facility supports its workers as well as its residents, aiming to provide a safe, secure, and homelike spot for convalescence, though some areas need improvement per inspection reports.
