    Phenomenal care, attentive therapy staff

I'm very grateful for the excellent care my mom received - amazing therapy and helpful, hardworking therapists who answered questions and helped her make real progress. The CNA and nursing staff were warm, professional, and attentive; rooms were immaculate and the dining area and grounds were lovely. Outings, bingo, music and movies kept her engaged. I highly recommend this facility for its phenomenal care.

    Summary of reviews

The reviews present a mixed picture of West Hills Health & Rehab Center. Positive accounts emphasize friendly direct-care staff, effective therapy teams, and an active social program; other accounts describe operational gaps that materially affected some residents' recovery and daily comfort. There is a clear pattern of variability: several families praised attentive CNAs, therapists who produced measurable progress, well-lit rooms, and a pleasant dining/activities environment, while other families experienced substantive care and environment shortfalls.

Care quality is uneven. Some reviewers described successful rehabilitation outcomes and helpful therapists; conversely, multiple families raised concerns about post-surgical care, pressure injuries, and declines in condition while in the facility. Understaffing and inconsistent staffing across shifts are recurrent themes and are linked in the reviews to delays in attending to residents, missed or untimely medication administration, and insufficient assistance with toileting and personal hygiene. There are also mentions of transfer-related problems that suggest gaps in safe-mobility practices and supervision.

Staffing and communication present a mixed picture as well. Many families found CNAs and some nurses to be kind, responsive, and communicative; however, other reviewers reported that doctors were not readily available, charge nurses varied in attentiveness, and phone access or family communication was sometimes unreliable. Language accommodation and cultural-sensitivity issues were raised, indicating that some residents and family members experienced difficulty communicating with staff or felt cultural needs were not fully addressed.

Dining and activities receive both praise and criticism. The facility offers organized programming, outings, and a pleasant dining area that some families appreciated. At the same time, food quality and meal-service timeliness were described as inconsistent, with reports of meals arriving late or being unsatisfactory. Facilities observations are similarly mixed: portions of the building and certain rooms were described as immaculate and well-maintained, while other areas had sanitation and odor-control concerns and reports of poor indoor air quality near some entry points.

Management and overall operations are an underlying theme tying many concerns together. Reviewers described organizational shortcomings that manifested as inconsistent care practices, variability between units and shifts, and difficulty reaching clinical leadership. Where leadership, staffing, and therapy aligned, families described good outcomes; where those elements were lacking, families reported avoidable problems during recoveries.

For prospective residents and families: West Hills may provide very good direct-care and therapy in some circumstances, particularly when staffing and supervision are stable. At the same time, the facility exhibits operational inconsistencies that warrant careful inquiry — ask about current staffing ratios, medication-administration protocols, toileting and transfer safety practices, physician coverage, language services, and how the facility addresses sanitation and air-quality concerns. Visiting at different times and speaking with therapy staff, charge nurses, and family members of current residents can help assess whether the positive aspects align with your expectations and needs.

About West Hills Health & Rehab Center

West Hills Health & Rehab Center sits on Topanga Canyon Boulevard in Canoga Park, CA, right near the 118 Simi Valley Freeway and the 101 Ventura Freeway, and you'll find the front lobby with a sitting area, a gated facility for safety, and a parking lot for visitors, plus there's an outside garden where residents can spend time. The nursing home has 145 certified beds and most days about 142 people live there, so it stays almost full, and it's seen as a continuing care facility offering rehabilitation, restorative care, transitional care, and support for those who need either short-term help or long-term assisted living and nursing care. The center takes admissions any time of day and has respite stay, hospice care, comfort care, and wound care, and you can expect physical therapy, occupational therapy, speech therapy, and therapeutic rehab services all under one roof, with 24-hour licensed nursing care available, plus they help with discharge planning if someone's ready to go home.

West Hills has a nurse turnover rate of 20.6%, lower than the state average, but their nurse staffing level averages out at 3.71 nurse hours per resident each day, which falls below the state average of 4.5. The team there can talk with you in many languages, including Hebrew, Italian, Mandarin, Japanese, Tagalog, English, Spanish, French, German, and Farsi. Residents get activities and entertainment, phone and cable service, internet access through an internet cafe, guest dining options, and salon services. They also offer tours for families to get a sense of life at the center and to meet both the staff and residents.

The facility is owned by a for-profit corporation and connected to Longwood Management Corporation. In 2013, the business was named Business of the Year and it's talked about as a leader in healthcare services in the area and known for aiming for excellence and personalized care, though records show it's not without its challenges. There've been 100 total deficiencies recorded from inspections, including 10 related to infections, and recent complaints led to deficiency findings in April 2025. Some specific deficiencies include failing to provide enough food or fluids (F0692) and not providing timely, quality laboratory services (F0770), which regulators said didn't cause actual harm but had the potential for more than minimal harm. Despite these issues, West Hills Health & Rehab Center does have a dedicated staff and offers a range of healthcare and support services for older adults and those needing rehabilitation in the Canoga Park area.
