    Compassionate staff and effective therapy

I had a very positive stay at Glen Hill - admissions was smooth and informative, the nurses, CNAs, therapists and social services were compassionate and professional, and therapy genuinely improved my mobility and confidence. The facility is immaculate and home-like, food good, communication with family was clear, and I'm grateful to the caring team and would recommend them.

    Summary of reviews

Autumn Lake Healthcare at Glen Hill elicits a polarized set of experiences: many families and residents praise the facility’s rehabilitation programs, compassionate caregiving, and engaging activities, while others describe operational shortcomings that affected clinical responsiveness and day-to-day comfort. The facility’s rehabilitation services — physical, occupational, and speech therapy — receive consistent positive feedback for intensity, clinician skill, and measurable functional gains. Social services and discharge planning are also frequently cited as helpful, with admissions staff noted for being guiding and informative.

Clinical care and staffing present a mixed picture. Reviewers repeatedly highlight dedicated nurses, therapists, and CNAs who provide compassionate, resident-focused care; at the same time, multiple accounts point to inconsistent staffing coverage, frequent use of agency staff unfamiliar with residents, and slow response to call bells. These operational weaknesses have been associated with delayed medication administration, medication-management errors, and delayed wound or clinical follow-up in some cases. Prospective residents with high medical acuity should ask specific questions about current staffing patterns, medication-safety protocols, and wound-care procedures during a tour.

Dining and activities show similar variability. Activity programming is regularly praised for creativity and breadth (weekend events, themed weeks, special functions), supporting social engagement and quality of life. Dining receives mixed reviews: some families describe good meals and positive experiences, while others report inconsistent meal quality, incorrect meal substitutions relative to dietary requests, and food-served temperature concerns. Clarifying dietary accommodations and meal-delivery processes is advisable.

Facility condition and environment combine strengths and weaknesses. Grounds, outdoor spaces, and many common areas are described as clean and well maintained, contributing to a home-like atmosphere. Conversely, multiple comments note small or dated resident rooms, worn furniture, HVAC/temperature issues in some rooms, and intermittent maintenance problems. Noise from nearby traffic and courtyard exposure is noted by some residents.

Management and operational oversight appear inconsistent across reviewers. Several families praise attentive administrators and a proactive Director of Nursing who addressed concerns; other accounts describe lapses in oversight, delayed vendor payments, supply shortages, and inconsistent front-desk/security coverage. These governance and supply-chain issues have contributed to variable experiences with equipment maintenance, availability of basic supplies, and follow-through on family concerns.

Notable patterns: strengths cluster around rehabilitation, therapy-driven recovery, and staff members who go above-and-beyond; risks center on medication safety, shift staffing reliability, timeliness of clinical interventions (wound care/urgent needs), and inconsistent management follow-through. For families considering Glen Hill, balance the facility’s strong therapy and activity offerings against operational weaknesses by asking targeted questions about current staffing ratios, medication-safety checks, weekend coverage, wound-care protocols, dietary accommodations, and escalation pathways for clinical concerns. Visiting during different shifts, requesting recent quality indicators, and confirming discharge-planning processes can help set realistic expectations and clarify whether the facility matches the prospective resident’s clinical and social needs.

About Autumn Lake Healthcare at Glen Hill

Autumn Lake Healthcare at Glen Hill sits as a licensed healthcare center in the Autumn Lake Healthcare network, located right at Glen Hill, and it offers quite a wide mix of care options for older adults, including everything from Independent and Assisted Living to Memory Care, Long-Term Care, Skilled Nursing, Medicare-Certified Home Health Care, Hospice, Home Care, and Adult Day Services, and they're known for having non-medical home care in the lineup too, which is handy for folks wanting to stay in their own place while getting help. Folks who need stronger support can sometimes get short-term rehab in private suites that come with their own furniture, refrigerator, flat-screen TV with cable, and things like free Wi-Fi and even concierge services, and those rooms also have enhanced dining menus for those needing diet options, and then there are separate spaces for longer stays or for specialty care, such as brain injury or dementia services.

Residents can expect medical care and personal services that cover the basics, like hospital gowns, toiletries, bath and bed linens, weekly laundry, meals, snacks, and safe living spaces that are always clean and kept up to standard, and meals are offered to fit dietary needs or special schedules, with menus reviewed by dieticians and changed as needed. There's telephone and Wi-Fi access and plenty of social and common spaces both indoors and outdoors, making it easier for people to stay in touch or join activities if they want, and the center tries to keep things homelike with cable TV, comfortable furniture, and a refrigerator available in living areas. General counseling is available, and there's a range of support services from case management to social work, discharge planning, audiology, vision and dental care, psychiatric care, and even dietary management.

Rehabilitation is a big area, with therapists working seven days a week on physical, occupational, speech, and orthopedic therapy in a state-of-the-art gym, and care teams use personalized recovery plans managed by board-certified specialists, with a special focus on regaining mobility and independence, and people who need IV therapy or advanced wound care services can usually find that here too. For folks looking for subacute care or brain injury support, there are programs and specialized staff, and there's always someone around to help with emergency alerts 24 hours a day. Autumn Lake Healthcare at Glen Hill is independently owned and operated, with its own licensed care staff and on-site medical support services rather than third-party providers.

The center takes up to 100 residents in certified beds, keeps an average of about 85 residents daily, and it stands affiliated with Genesis Healthcare and the Glen Hill Center, which has received several ACHA Quality Awards. The nurse turnover rate is 40.7%, and they provide about 3.46 nursing hours to each resident per day. The most recent standard inspection by CMS was on May 23, 2023, and records show 25 documented deficiencies (including 2 related to infection control), and ongoing CMS oversight continues for compliance and resident safety. They're set up to support a wide range of insurance options, including Medicare, Medicaid, and most private payments, with a strong focus on delivering licensed health, therapy, counseling, rehabilitation, and daily living help for adults in a setting meant to feel comfortable and safe for all who live there.
