    Respectful staff, clean facility, excellent

I was very pleased with the respectful, attentive and caring staff - nurses, CNAs and therapists who went above and beyond - and the clean, well-equipped facility that feels warm and family-like; front-desk and customer-service were communicative and helpful, rehab care was excellent, and I'm grateful for the personalized, compassionate attention.

    Summary of reviews

Riverside Health and Rehabilitation Center elicits strongly polarized impressions. A large body of feedback highlights compassionate, engaged nursing staff, skilled CNAs, and an effective therapy team that frequently supports rapid recovery and successful short-term rehabilitation. Many families and residents praised the facility’s admissions process, proactive front-desk/customer-service liaisons, on-site pulmonary/respiratory capabilities (including a ventilator unit), and a generally clean, well-maintained environment with pleasant common areas and outdoor space. Recreational programming and social activities (bingo, sing-alongs, themed events) are consistently described as meaningful for resident engagement.

At the same time, reviewers identify repeated operational weaknesses that affect quality and safety. Staffing inconsistencies and periods of short staffing were commonly cited, with attendant delays in responding to assistance requests and incontinence-care needs, variable bathing schedules, and longer wait times for bathroom and hygiene assistance. Several comments point to uneven caregiver competence and professionalism, and there are recurring descriptions of communication and coordination gaps between nursing, therapy, social work, dietary, and administrative teams. Medication-administration and discharge processes also surfaced as an area of concern, including omissions and incomplete medication provision at discharge.

Dining and nutrition received mixed evaluations: some reviewers praised meals and occasional special offerings, while others described late, cold, or missing items and limited menu variety. Facility infrastructure and accessibility show strengths and limits — many noted clean rooms, pleasant views, and modern therapy equipment, yet others described small rooms that challenge motorized wheelchair maneuvering, shared-room privacy constraints, elevator outages, and ongoing maintenance needs in certain areas. Sanitation and pest-control concerns were raised for select units, and several reviewers specifically cited issues with wound care or infection-control practices.

There are also a number of serious, singular allegations within the feedback — including medication incidents, claims of theft, and concerns following a resident’s death. These are not consistent across the entire dataset but are sufficiently severe that prospective residents and families should inquire directly about incident reporting, safeguards, and any regulatory actions when evaluating the facility.

In summary, Riverside presents clear strengths in rehabilitative care, respiratory services, and a welcoming, family-oriented culture maintained by many dedicated staff members. However, persistent operational challenges — particularly around staffing consistency, responsiveness, medication and discharge processes, dining reliability, accessibility, and select sanitation/pest-control issues — create variability in the resident experience. Prospective residents and families would benefit from targeted questions at tour and intake about staffing levels and ratios, call-response times, medication management and discharge protocols, wound-care policies, pest-control measures, room accessibility, and how the facility documents and addresses serious incidents.
