Pathways to Care

Pathways to Care sits over in Casselberry at 430 Plumosa Ave., and it's a not-for-profit program working with Catholic Charities of Central Florida and part of the broader findhelp social care platform, and what they do is help people in Central Florida who don't have stable housing recover from illness or injury, which is a real concern for homeless seniors and veterans especially, and they've got a 34-bed licensed assisted living facility where about 200 people each year get medical respite care and case management, plus there's a special service called Pathways Step2 that offers transitional housing and helps people work toward independence after they leave the hospital, and the way folks get in is through a referral process that includes local hospitals, the VA, HOPWA, other social service groups, or sometimes faith organizations and governments that know about someone in need, but there's even more because they run programs for mental health, behavioral health, and senior wellness alongside immigration services and broader social care support, and you'll see Pathways to Care tied in with national work through the National Institute for Medical Respite Care, so there are standards, directories, toolkits, and things like the Willie J. Mackey National Medical Respite Award, and they also keep up with research, a providers' network, and online resources to make sure the work helps as many folks as possible, and since 2003 this team has focused hard on helping men and women without a safe place to heal, always with steady partnerships in the community, and the campus is busy with outreach, housing help, and programs to support physical and mental health for seniors and other vulnerable people.
