    Spotless rooms, caring staff, recovery

I was very pleased with my stay - spacious, spotless rooms with private showers, lovely gardens and well-appointed common areas. Therapy and rehab began right away and staff (nurses, CNAs and therapists) were attentive, introduced themselves, and answered call lights promptly. Meals were fresh and nutritious with daily choices and snacks; concierge, activities and outpatient rehab made recovery convenient. Overall a caring, professional place that helped my mobility and confidence - I'd recommend it.

    Summary of reviews

Reviews of Lexington Health and Rehabilitation Center are highly polarized, with distinct patterns of positive clinical rehabilitation outcomes and notable operational weaknesses. The facility's therapy program (physical and occupational therapy) is frequently cited as a core strength: reviewers describe rapid functional gains, effective therapy staff, and good therapy outcomes that in many cases facilitated return home. Several families praised specific therapists and rehab teams for individualized plans and measurable improvements.

Staffing and direct-care performance present a mixed picture. Many reviews highlight compassionate CNAs and nurses who provide attentive bedside care and supportive communication; named individuals and unit managers were singled out for going above and beyond. At the same time, other accounts describe inconsistent nursing responsiveness, delayed call-button answers, and uneven personal-care assistance, which families experienced as delays in toileting, bathing, or timely medication administration. These contrasts suggest variability in staff performance across shifts and units rather than a uniformly consistent standard of care.

Medication management, clinical coordination, and safety processes emerged as consistent areas of concern. Multiple accounts reference missed or delayed medications, medication-administration errors, and gaps in pharmacy/medication controls that in some cases required hospital readmission. Family communication about clinical events — including medication changes, falls, or clinician visits — was also described as unreliable in many reports, with difficulties reaching staff by phone and inconsistent documentation of doctor interactions.

Dining and nutrition experiences vary considerably. Positive reports note nutritious meals, adequate portions, and accommodating kitchen staff; however, a sizable number of reviews describe inconsistent meal temperature, limited variety, small portions, and interruptions in meal service. Food-service continuity and menu consistency appear to fluctuate by unit and shift, indicating a need for stronger kitchen processes to ensure reliable nutrition delivery.

Facilities, housekeeping, and maintenance are areas of conflicting feedback. Some units and recently renovated rooms are described as clean, modern, and well-kept, with responsive housekeeping. Conversely, other reviews raise sanitation concerns, pest-control concerns, mold-related observations, and structural maintenance issues such as peeling surfaces and aging fixtures. These differences suggest uneven environmental standards across the campus and variability in housekeeping/maintenance performance.

Management and organizational themes include praise for specific leaders and unit managers who are described as effective advocates, responsive, and hands-on. Simultaneously, reviewers frequently cite inconsistent administrative communication, turnover in leadership, and perceived lack of transparency around billing and discharge procedures. There are also serious allegations in a minority of reviews relating to theft and financial exploitation; these are described as allegations and would warrant formal investigation and verification by regulators.

Activities and social programming are generally seen positively when staffed and scheduled, with several reviewers noting meaningful engagement, outings, and therapeutic activities. Yet availability and consistency of programming are uneven in some units, corresponding to staffing levels and unit focus (short-term rehab versus long-term care).

Overall, prospective residents and families should weigh the facility's demonstrated strengths in rehabilitative care and several named staff members who deliver high-quality, compassionate service against operational inconsistencies: variable nursing responsiveness, medication-management risks, uneven cleanliness and maintenance, and administrative communication challenges. Visiting in person, meeting therapy and nursing leadership, and reviewing current staffing levels, medication-safety policies, and recent regulatory findings would help families clarify whether the facility's strengths align with a specific resident's needs.

About Lexington Health and Rehabilitation Center

Lexington Health and Rehabilitation Center sits on well-kept grounds in Kenneth City, close to St. Petersburg, Florida, not far from top hospitals like St. Petersburg General and Northside Hospital, and it offers a mix of private suites and shared rooms so people have a few options, and those suites come with air conditioning and WiFi, which are nice for comfort, and the place seems to offer a lot for folks who need different levels of care, since it has nursing home services, skilled nursing, short-term rehabilitation for people coming from the hospital, and even specialized units for orthopedic and neurological recovery, which can make things easier if someone's coming back from surgery or needs specific therapies, and they don't stop there either, since they've got a full rehab gym, a therapy lounge, a mobility garden for working on walking or balance, and therapy services that cover physical, occupational, and speech therapy, plus there's a focus on palliative and respiratory care and wound healing, which shows they try to cover a wide range of health needs.

There's a Rapid Recovery Rehab Unit to help with getting strength back, and people there can expect activities and social opportunities every day, and the staff seems pretty committed to keeping folks active whether it's inside or out since Florida weather is good for that, plus they've got places to relax like the common areas, a barber shop, and a beauty salon, and from the looks of things, they've designed their programs so that people can make real progress in their recovery, and there's support for both short-term guests and those who'll stay longer, with an emphasis on making care plans that fit the person rather than a one-size-fits-all approach, and they've got a skilled team of doctors and therapists on hand, which probably helps with the level of medical services they can offer, including high acuity care for those who need more attention, and overall, the environment is described as friendly and welcoming, focusing on connection, safety, and measurable progress, and people can choose from studio and companion layouts depending on how much privacy or support they want.

The Center provides respite care for short stays, memory care, assisted living, skilled nursing, and intermediate care, so it's set up to handle both lighter needs and more complex cases with genuine concern for residents, but honestly, while the place has some nice features and programs, the public ratings are moderate, with a 6.7 out of 10 from the community and a two-star rating from the Centers for Medicare & Medicaid Services for its nursing care, and that means potential residents and families should look at those scores, tour the place if they can, and decide if it fits what they're looking for. Many people feel the care is personalized and the rehab results are good, especially when it comes to wound healing and helping people return home, and there are plenty of on-site and off-site activities aimed at keeping people engaged, but as with any facility, it's important to look at those ratings, visit, and ask questions about staff, programs, or anything else that matters to you.
