    Compassionate attentive memory care facility

I placed my loved one here and am very pleased - compassionate, attentive staff provide person-centered memory care, nurses/CNAs are excellent, the facility is clean and comfortable with balanced meals and engaging activities, and staff went above and beyond during move-in; I'd recommend it.

    Summary of reviews

Overall impression: Harvest Acres Nursing and Rehab presents as a small, community-oriented facility with many strengths around day-to-day caregiving, memory-care programming, and resident quality of life. Reviewers emphasize compassionate, person-centered care delivered by nurses and CNAs who are attentive and engaged. The facility projects a home-like atmosphere and many families described a welcoming, family-style environment that supports residentsโ€™ comfort and social connection.

Care and staff: Clinical care is framed positively in multiple accounts, particularly for residents with dementia โ€” staff appear to be adopting a clearer memory-care direction and implementing calming, engagement-focused approaches. Many reviewers praised staff kindness, responsiveness, and a sense of pride in their work. At the same time, reviewers also raised operational concerns: some accounts describe inexperienced staff members, signs of turnover, and occasions where staff communication or conduct created frustration for families. Prospective families should ask about staffing ratios, staff training programs (especially dementia training), and shift-to-shift continuity when evaluating the facility.

Dining and activities: Dining impressions are mixed. Several reviews note balanced, healthy meals and an effort to provide treats and resident-centered choices, while others characterize some menu items as unpalatable. The facility appears to offer a robust calendar of activities, creative memory-loss programming, and social events that contribute to engagement and resident satisfaction. Visiting during meal service and an activity period is recommended to directly assess dining quality and program tone.

Facilities and management: The physical plant is consistently described as clean and well maintained both inside and out, with dedicated environmental staff supporting sanitation and upkeep. Management is portrayed as responsive by many families, timely in addressing concerns, and pursuing a renewed dementia-focused direction. However, the presence of past regulatory citations and reports of operational variability across shifts suggest areas where administrative oversight and consistency may need attention. Before committing, families may wish to review recent inspection reports, ask about corrective actions taken, and request references from current families to gauge ongoing consistency in care delivery.

About Harvest Acres Nursing and Rehab

Harvest Acres Nursing and Rehab stands in Keota, IA, as a for-profit nursing facility offering both long-term care and skilled nursing, and the place takes Medicare and Medicaid if folks need that. The building holds up to 35 beds and has an average of about 26 residents each day, so it's not too big and not too noisy, either. The staff here provides specialized health care, including skilled rehab services, wound care, and support for those with complex needs, and there's always a nurse on duty all hours of the day. Even though the nursing staff is described as compassionate and highly qualified, staff levels run below the state average, and nurse turnover is higher than usual at 56.5% compared to the state's 46.6%. Residents get about 2.89 hours with a nurse each day when the state average is closer to 3.8.

Quality measures at Harvest Acres rate above average, but the facility's health inspections are much below average, and it's listed as a Special Focus Facility Candidate because of serious quality history, including 43 inspection deficiencies, five about infection control, and some citations for not responding or reporting possible abuse, and not protecting residents from neglect or exploitation. The overall government rating sits at much below average.

The facility fits several types of needs, including folks needing long-term nursing care, folks needing memory care, and those needing short-term rehab to help them get well and go home. Harvest Acres handles physical, occupational, and speech therapy, tailoring each plan to help folks regain their abilities, and there's a focus on making transitions home when possible. They offer hospice and respite care, too, with support for comfort, pain management, and dignity at the end of life. For memory care, there's a secure wing for people with Alzheimer's or dementia, with safety features, small group activities, and one-on-one attention. Amenities include private rehab suites, private dining, lounges, a beauty salon, linen service, and housekeeping. The building offers respiratory support, such as oxygen therapy, CPAP, and Bi-PAP, and certified nurses handle wound care on-site.

Harvest Acres operates Monday through Friday from 10:00 to 17:00. Ownership is tied to Campbell Street Services, but further details aren't provided. The facility's motto is "Residents First," and the focus lies in maintaining privacy, dignity, and comfort for those with different levels of care needs, even if the facility's inspection and staffing issues should be noted when making decisions.
