    Compassionate staff, spotless, family-like care

I entrust my grandmother here and have peace of mind - the long-tenured, kind staff treat residents like family and go above and beyond. Nursing and therapy (hip therapy included) are compassionate and hands-on, activities and dietary accommodations keep residents engaged, and the facility is spotless; supportive management and genuine teamwork make this a place I happily recommend.

    Summary of reviews

Reviews present a mixed but clear pattern: the facility is frequently praised for hands-on, compassionate caregiving by long-tenured staff, solid therapy offerings, and an environment that many families describe as reassuring. Multiple accounts emphasize staff who form personal, family-like relationships with residents, engage them in daily activities and spiritual services, and make dietary accommodations when needed. Facility upkeep and cleanliness are commonly described as strengths in many observations, contributing to an overall sense of order and safety for some families.

At the same time, recurring operational issues appear across reviews. Staffing levels and staffing quality are inconsistent: while many employees are described as dedicated and experienced, others are characterized by problematic interpersonal tone or rough conduct. These differences can produce variable day-to-day experiences for residents, particularly during busy shifts. Interdepartmental communication gaps and inconsistent clinical responsiveness are cited as contributors to missed or delayed care interventions and uneven therapy follow-through.

Dining and activities are areas of both strength and variability. Several reviewers note meaningful recreational programming and spiritual services alongside individualized dietary accommodations. However, food quality and meal satisfaction are noted as inconsistent, with some families satisfied and others dissatisfied with menu or preparation standards.

Facility maintenance and housekeeping are generally identified as positives, with multiple comments about clean, well-kept common areas and a pleasant environment; nonetheless, some reviews point to uneven housekeeping performance across units. Therapy and rehabilitation servicesโ€”such as hip therapyโ€”are highlighted as effective in many cases, though a subset of reviews describe variable outcomes and incomplete therapy follow-through.

Management and leadership emerge as a polarizing theme. Some reviewers describe supportive supervisors and a positive workplace culture, while others raise concerns about management practices that affect staff morale, transparency, and incident handling. There are also a few serious allegations about documentation and communication around clinical incidents, including concerns following a resident's death; these matters suggest families should seek detailed, direct answers about clinical oversight and incident-reporting procedures when evaluating the facility.

Overall, prospective residents and families can expect a facility with strong, compassionate caregivers and useful therapy and activity programming, balanced against operational variability in staffing, communication, management practices, and some service areas (food, housekeeping, medication oversight). Visitors should meet the care team, ask about staffing patterns for specific units and shifts, review medication-management and incident-reporting protocols, and observe meal and activity periods to gauge consistency before making placement decisions.

    About Salyersville Nursing & Rehab

Salyersville Nursing & Rehab sits in Salyersville, Kentucky, and offers skilled nursing, long-term care, and various rehabilitation therapies, and folks will find that physical, occupational, speech, and respiratory therapy are all available right on site, since they've got licensed nurses, therapists, and trained health care aides around the clock, with respiratory therapists around any time too, and doctors come in regularly for visits, so you'll always get checked out if you need it. The place takes care of people who need wound care or memory support, helps with daily living, and keeps care plans focused on each person. There's a dedicated program for cutting down on hospital readmissions, plus disease management and early intervention, and people get help with vision and dental care, nutritional and dietary counseling, medication management, and specialty services like respite care too. The staff helps residents regain their abilities after surgery or illness, using new equipment and approaches to rehab, aiming to make folks comfortable and help them get going again, all while keeping things going non-stop, day and night. The rooms are set up for comfort, there are outdoor areas for relaxing, and they try to keep people active with social events, regular activities, and ways to get involved in the community. Housekeeping and laundry are handled, and residents can count on support with whatever daily activities they can't do on their own, so it does cover a lot of needs under one roof. The place holds up to 142 residents, and usually there are about 115 folks staying, so it's a busy place with a mix of different needs. Salyersville Nursing & Rehab has been operated by Extendicare, a company that runs a lot of these facilities around the country, with management by Moshe Kelman and Thomas Tackett, and there's an ownership group including Benjamin Landa, Goldie Platschek, Alexander Platschek, and David Rubenstein. The nurse turnover rate is 28.4% over a year, which is better than what most places in the state see, but nurse staffing hours per person are lower than the Kentucky average. Deficiencies have shown up-there were 16 in recent inspection reports-with issues like keeping equipment in good order and handling nutrition safely, though reports say there was no actual harm, still there was the risk for harm, and inspections have been delayed with the last full check happening over two years ago. While Salyersville Nursing & Rehab tries to offer compassionate, tailored care and lots of support through individual programs, it's good to know about the inspection and staffing points before choosing, and overall, it's a large and busy facility covering most skilled nursing and rehab needs for older adults or others needing recovery in the area.
