    Attentive staff, excellent therapy, immaculate

I had a very positive experience - attentive, caring staff and nurses, stellar therapists with daily PT/OT, and engaging, varied activities that kept my loved one happy and socially connected. The facility is immaculate and modern with lovely sun rooms, patio and grounds, excellent meals (in-room or dining), responsive management/housekeeping/maintenance, and a dog-friendly policy; I would choose Augustana again.

    Summary of reviews

The review set presents a mixed but informative picture of Apple Valley Village Health Care Center. Many reviewers praise frontline staff — nurses, aides and therapists — for compassionate, personalized care and for producing measurable rehabilitation gains. The therapy program and activity offerings are frequently cited as strengths: residents commonly receive physical and occupational therapy, participate in a variety of activities, and benefit from communal spaces such as sun rooms, patios and a gift shop. The facility's appearance and upkeep receive positive comments as well; several families note a modern, clean environment, attractive grounds, convenient parking and generally responsive housekeeping and maintenance.

At the same time, the reviews reveal several operational weaknesses that prospective residents and families should consider. Staffing consistency is a recurrent concern: reviewers describe limited coverage on night and weekend shifts and longer-than-expected waits for assistance, often tied to delayed response to call lights. Related safety themes include gaps in fall-risk monitoring and transfer-safety practices. Multiple accounts also point to inconsistent medication administration and pain-management practices, as well as delays when acute clinical escalation is needed (for example, imaging, IV administration, or hospital transfer). These process gaps are sometimes framed by reviewers as failures of timely clinical oversight; comments indicate limited physician availability or delayed physician follow-up in certain cases.

Dining and nutrition are another mixed area. Many reviewers compliment meal variety, accommodating dining staff and the availability of in-room service, but others describe inconsistent adherence to dietary restrictions and variability in meal quality. Financial and administrative issues surface as well: reviewers cite billing opacity, high pharmacy charges, and difficulty obtaining itemized statements. Property-security concerns appear in a small number of accounts as allegations of missing belongings, which suggests a need to ask about inventory and property-control procedures.

Communication and management impressions are polarized. Several families commend attentive management and proactive staff who engage with families; conversely, others report poor communication, canceled appointments, or perceived unresponsiveness from physicians and administrative personnel. Cleanliness and sanitation are generally praised in many reviews, but a subset raise sanitation or odor concerns in particular areas and inconsistent bathing or incontinence-care processes. This mixed feedback indicates variable performance across shifts, units or individual staff members rather than uniformly consistent practice.

In summary, Apple Valley Village Health Care Center shows clear strengths in therapy, activities, communal amenities, and many compassionate front-line staff members. However, the facility also exhibits operational vulnerabilities in staffing consistency, timely clinical escalation, medication and pain-management practices, communication, and certain administrative controls. Families considering this facility should weigh rehabilitation and social strengths against these operational issues and ask targeted questions about staffing levels (day/night/weekend), call-light response metrics, physician availability and escalation protocols, dietary accommodation processes, billing transparency, and property-control procedures during their tour and intake discussions.

About Apple Valley Village Health Care Center

Apple Valley Village Health Care Center sits at 14650 Garrett Ave in Apple Valley, Minnesota, and offers a wide range of senior care options, so you'll see independent living, assisted living, care suites, memory care for dementia, skilled nursing for either short-term rehab or ongoing long-term care, as well as hospice, adult day services, and spiritual care, all under one roof, which makes transitions a bit simpler for a lot of folks who don't want to move between many places as needs change. The staff includes physicians, nurse practitioners, licensed nurses, nursing assistants, therapists, social workers, a registered dietician, and a full-time chaplain, and there's always a registered nurse on site 24 hours daily, supporting round-the-clock skilled nursing and personal care for residents, especially those with high health needs or advanced conditions.

There's a strong spiritual life program here, offering regular chapel services, biblical classes, personal prayer, counseling, and prayer groups, and the chapel holds non-denominational services for people from any background, plus Grace Hospice provides comfort-minded care for residents in need with support from chaplains. The place is well equipped when it comes to health services, so you've got a primary care clinic, audiologist, optometrist, dentist, psychologist, pharmacy, X-ray, and lab services available on site. For those needing complex care, staff can provide pulmonary and cardiac rehabilitation, advanced wound care, and manage specialized devices like CADD pumps, PICC lines, and ventilators, and they handle IV and antibiotic treatments too. There's also therapy for all kinds of recovery, such as occupational, physical, speech, music, recreational, and even pet therapy.

The living spaces are designed for comfort, offering private and semi-private rooms, sometimes with kitchenettes and private baths, and you'll find community spaces where people can gather. There's a restaurant-style dining room serving three fresh meals each day with a few choices and daily specials, and snacks available throughout the day. The Deli/Convenience Store makes it easy to grab coffee, quick meals, groceries, and small household items, and having that kind of store on site is very handy. Residents can enjoy amenities like a beauty salon and barber shop, nail salon, billiards and game room, satellite TV, wireless internet, housekeeping, laundry, and plenty of places to relax. For those focused on fitness, there's a special exercise room with equipment designed for older adults, daily exercise classes, and a heated therapeutic pool used both for classes and relaxation, which is quite popular.

Social life gets plenty of attention at Apple Valley Village, so folks can join in organized outings with bus transportation to shopping, dining, attractions, and other entertainment, or stay in and take part in events, guest speakers, travel and education seminars, and regular entertainment or games. Outdoors, there are walking paths, a gazebo, patios, gardens, a pond with a fountain-you'll see residents strolling around or just relaxing when the weather's nice. The community tries to provide a holistic approach, focusing on the social, emotional, spiritual, and physical health of everyone, and staff organize life-enriching activities and therapeutic recreation every week. Adult day services are available for those living at home who need daytime help and social time.

This community is part of Cassia, a 501(c)(3) organization, and works in connection with Apple Valley Villa Apartments nearby, and they keep things running smoothly with a Director of Housing, a Director of Marketing, and an Administrator overseeing the day-to-day. All in all, Apple Valley Village Health Care Center aims to make it easier for older adults to find the care they need, whether that's independent living or advanced skilled nursing, in a place with caring staff, good food, and spaces both indoors and out where people can feel comfortable and at home.
