    Compassionate team transformed the facility

I placed my loved one here and have been very impressed - Administrator Ashlee and a caring, hardworking team (Lori Beth, Jackie, Amy, Emilia and many others) have genuinely transformed the place in a few months. The facility is clean and well-kept, residents are engaged and happy, rehab and activities are excellent, and staff consistently go above and beyond with compassionate, person-centered care in a safe, welcoming environment, so I highly recommend it.

    Summary of reviews

Avalon View Health and Wellness elicits a strongly mixed set of impressions. Many reviewers describe a core of compassionate, dedicated caregivers, a robust activities department, and an effective rehabilitation program; these strengths are frequently tied to specific staff members and to visible changes under newer leadership. At the same time, a substantial number of comments identify persistent operational gaps that affect clinical reliability, resident comfort, and family trust.

Care and clinical oversight show a bifurcated pattern. Strengths include individualized therapy, attentive aides in many units, and examples of improved wound care and safer practices after management changes. However, reviewers raise consistent concerns about medication administration and documentation, inconsistent chronic-disease management (wound care, diabetes, hypertension), and variable nursing competency and supervision. These clinical gaps are frequently associated with periods of understaffing and high turnover; several reviewers linked delays or errors to staffing levels and training. Families considering this facility should ask specifically about current nursing coverage, medication-safety protocols, wound-care follow-up, and recent clinical outcomes.

Dining and personal care are areas of variable performance. Some families praise meal quality and kitchen staff, while others report cold meals, reliance on supplemental shakes, and inconsistent presentation. Personal-care delivery (showers, dressing assistance, toileting support) is similarly inconsistent across accounts: some residents received attentive, routine hygiene care, while others experienced delays in scheduled bathing and dressing. Call-light response and assistance timing were also noted as intermittent.

Activities and social engagement are consistently cited as a positive. The facility offers group programming, 1:1 engagement, memory-care stations, and plans for outdoor garden spaces; families and residents frequently describe staff in activities as energetic and resident-centered. These programs appear to be a clear relative strength and a focal point of the facility’s culture change efforts.

Facility condition and operations show improvement trends but remain uneven. Several reviewers describe recent renovations, improved cleanliness, and strong EVS leadership; others cite older rooms, heating/TV issues, odor or sanitation concerns in some areas, and occasional pest-control problems. Maintenance and laundry services receive regular praise, but infrastructure limitations are still present in parts of the building.

Management and regulatory context are important factors. Multiple reviews point to a leadership-driven turnaround: new administrators are credited with better staffing hires, improved survey results, and removal from heightened regulatory scrutiny. Conversely, there are serious, isolated comments about financial mismanagement and alleged misconduct; these allegations have been raised alongside citations and complaints in public records. Communication style and family relations are uneven — some families report transparent, supportive management, while others experienced blocked communication, evasive responses, or confrontational interactions.

Overall pattern: Avalon View demonstrates clear areas of progress — especially in therapy, activities, and visible leadership — but it also exhibits recurring operational weaknesses that can materially affect resident safety and comfort. Prospective residents and families should verify recent inspection reports and quality metrics, tour units at different times of day, review current staffing ratios and medication-safety policies, and ask for documentation of corrective actions for clinical and financial concerns before making placement decisions.

About Avalon View Health and Wellness

Avalon View Health and Wellness sits at 1200 West College Street in Liberty, Missouri, and is a Skilled Nursing Facility with 140 certified beds and an average of 105 residents each day, and while it offers nursing, rehabilitation, long-term care, Memory Care in a 34-bed secured unit, therapies like physical, speech, and occupational, and has amenities like private bedrooms, laundry, housekeeping, residents' lounge, emergency call system, and programs focused on health, wellness, and support for residents with dementia including activities based on natural circadian rhythms, there have been many documented problems and deficiencies, which is something families often want to know about ahead of time. The current management is run by College St Consulting LLC, along with Ramilo Gatapia, William Miller, and administrator Ashlee Siard, with ownership by Vertical Health Services since June 2023, and though the staff provides 24-hour nurse coverage, they have a nurse turnover rate of 55%, and nurse staffing levels are below state and national averages at 2.44 nurse hours per resident per day, and the facility has struggled to keep a licensed nurse in charge on every shift and hasn't always met the eight-hour registered nurse on-duty rule. Deficiencies have included failing to support food and nutrition services with enough staff, medication storage problems, not keeping up with infection control programs (with 7 infection-related citations), not preventing medication errors below five percent, missing dental services for each resident, not always providing a safe, clean, and homelike environment, and not developing and using good policies to stop theft, neglect, and abuse, and these problems have been documented in 122 patient care deficiencies from March 2021 to December 2023, with a total of 103 deficiencies found in reports overall and 20 during the latest inspection in September 2024, resulting in a fine of $14,043. The place has also been fined five times, totaling $288,000, and gotten two government payment suspensions from July 2021 to October 2022, and it's rated one star on Medicare.gov. Residents get individualized treatment programs, medication management, and support with activities of daily living for both short-term and long-term stays, but the facility isn't accepting new patients right now, and there's no information about languages spoken other than English or about special services and amenities outside what's listed, and while Avalon View tries to build a homelike atmosphere and work with skilled therapy teams and organizations like the Alzheimer's Association, the history of self-reported harm events, high complaint counts, and infection and fire safety issues mean it's important for families to look carefully at their needs before making choices.
