    Compassionate attentive care for parents

I placed my mom and dad at Delmar Gardens and feel at ease - the nurses, therapists and caregivers are kind, patient and genuinely caring across rehab, daily care and hospice. The facility is clean, welcoming and lively with plenty of activities and memory-care support; admin (Gabe and the Grossberg family) and staff like bus driver Kenny went out of their way to help. I'm very happy with the compassionate, dedicated team and would recommend them.

    Summary of reviews

Delmar Gardens of O'Fallon elicits strongly mixed impressions. Many comments praise individual caregivers—CNAs, nurses and some therapy staff are described as caring, attentive and capable—and the facility offers memory-care programming, on-site salon services, social activity options and attractive grounds. Several families reported positive rehabilitation outcomes and a welcoming, home-like environment in parts of the campus.

At the same time, a substantial set of concerns recur across reviews and point to operational inconsistencies. Care-quality issues include missed or delayed medication administration, inconsistent bathing and repositioning schedules, and examples of inadequate monitoring after falls. Reviewers also describe problems with end-of-life support and pain management in some cases. These concerns align with reports of limited clinical oversight and gaps in fall-prevention and transfer safety procedures.

Staffing and staff conduct are major themes. Short-staffing—especially overnight and during busy periods—was frequently cited and linked to delayed responses to call bells, longer waits for assistance with toileting and dressing, and incomplete rounds. Family members described both compassionate caregivers and episodes of problematic communication or confrontational tone from other staff. There are also comments suggesting problematic human-resources practices that affect morale and continuity of care.

Sanitation, housekeeping and facility maintenance show uneven performance. Many reviewers described timely housekeeping and a clean appearance in some areas, while others noted sanitation concerns, delayed trash and linen removal, pest-control concerns in certain units, and worn room furniture or delayed maintenance responses (for example, slow TV or equipment repairs).

Dining and therapy receive mixed feedback. Some residents and families praise the therapy team and note helpful activities and social programs; others experience inconsistent therapy follow-through, difficulty scheduling ancillary services (podiatry, salon), and variable meal quality with complaints about cold or reheated food and slow service.

Management, communication and billing emerged as additional patterns. Several families reported late notifications during clinical decline, lack of timely outreach after incidents, difficulty obtaining clear billing or insurance explanations, and disagreements over length-of-stay charges. There are also mentions of a formal complaint and contact with state health authorities in at least one case, as well as serious allegations involving staff conduct, resident safety incidents, and potential theft.

Taken together, the facility appears to deliver good care in many individual instances—notably where staffing and clinical oversight are present—but exhibits inconsistent performance across staffing, sanitation, clinical oversight, and administrative transparency. Prospective residents and families should consider on-site observation, ask about current staffing ratios (including nights), medication and fall-prevention protocols, housekeeping schedules, infection-control and pest-management practices, therapy plans and documentation of hospice/end-of-life procedures, and should review recent state inspection or complaint records to better understand how the facility addresses the recurring concerns described above.

    About Delmar Gardens of O'Fallon

Delmar Gardens of O'Fallon sits in Dardenne Prairie, MO, and has 45 certified beds available as of June 2025 for residents who need different levels of care, so you'll find options for independent living, assisted living, skilled nursing, memory care, and short-term rehabilitation all under one roof, and since it's a Continuing Care Retirement Community, residents can move smoothly between these services without leaving the community. The facility accepts both Medicare and Medicaid, which helps with affordability for many families, and they organize both resident and family councils that meet regularly to talk over concerns and work on quality and care improvements, which helps keep everyone involved in decisions. You'll notice they have furnished apartments with amenities like private baths, air conditioning, kitchenettes, cable TV, telephones, free Wi-Fi, and some rooms look out over garden spaces, plus everything is wheelchair accessible, and there are secured memory care units called Serenity Suites for those with Alzheimer's or other cognitive needs, which also have a wander alert system for safety and offer daily programming designed just for memory care residents.

When it comes to day-to-day living, staff provide help with activities like bathing, dressing, meals, grooming, toileting, transfers, and medication management, plus there's 24-hour supervision, an emergency call system, and pharmacy services on-site if needed. You'll find skilled care from a staff that includes a Medical Director, registered nurses, licensed practical nurses, certified nursing assistants, and expert therapists, all of whom go through background and safety checks, and there's even a specialty wound care program and MedResources for specialized medical needs. Residents and families can use respite care or transitional stays if recovery or a break is needed after hospitalization, and there's also home care services like medical supplies, home healthcare, private duty, hospice, and palliative support for those wanting to age in place a while longer or needing extra help.

The campus has its own restaurant-style dining room with meals sensitive to allergies and special diets, including diabetes menus, and housekeeping, laundry, move-in coordination, and help with transportation or parking are handled by staff. For recreation, there's a fitness center, library, movie theater, activity and arts room, spa, billiards room, gardens, walking paths, and plenty of grab bars in bathrooms for safety, while the day care and early childhood education center run special programs involving children and residents together, encouraging friendship and respect across generations. Social life stays active with movie nights, music programs, game rooms, religious services, and resident-led activities, and regularly scheduled daily events keep everyone involved if they're interested, so even on days when someone wants to stay in, rooms come with cable TV and high-speed internet, and the beauty salon and barbershop are right on site for convenience.

The facility offers spacious apartments with both private and semi-private room options, and the floorplan is designed to be obstacle-free for easier movement. Community-sponsored programs and outdoor activities make use of the garden space and walking paths, offering ways to meet neighbors or just spend time outside, and there are private areas where families can visit. Staff will also travel out to a client's location if the need for home care comes up, and the center provides pharmacy support, wound care, memory care, and therapy programs, so people can get comprehensive help as needs change over time, all while maintaining a culture that values heartfelt care.
