    Compassionate attentive staff, homey community

I've had a very positive experience here - staff are kind, attentive, and work together to provide excellent, compassionate care. Communication is prompt and helpful, dining is good (my wife even started eating again while still on a feeding tube), and the social worker, maintenance, and dietary teams went above and beyond. The place feels homey with a warm, small-town community, and I would recommend it.

    Summary of reviews

Reviews of Pleasant Hill Health and Rehabilitation Center present a mixed and polarized picture. Many families describe instances of attentive, caring frontline staff — including aides, nurses, therapy personnel, dietary workers, and a social worker who provided notable support — and praise a home-like atmosphere and strong teamwork among long-tenured employees. Some reviewers specifically highlighted responsive communication, helpful maintenance, pleasant outdoor areas, and staff members singled out for excellent service.

Concurrently, a substantial set of concerns recurs across other reviews. Operational themes include inconsistent personal-care routines (including bathing and incontinence-related care), weaknesses in medication administration and documentation, and low staffing ratios on the memory/dementia unit. Housekeeping and sanitation management problems were frequently cited alongside pest-control concerns and reports of missing personal items, indicating gaps in environmental services and asset controls. Activity programming also appears inconsistent: advertised calendars and memory-care engagement were often described as not delivered.

Clinical oversight and management are central areas of variability. Multiple accounts indicate high staff and administrative turnover, inconsistent follow-up on clinical issues and test results, and gaps in coordination with hospitals and external providers. Several families reported difficulties obtaining medical records or encountering unhelpful interactions with leadership; there are also serious, individual claims that include intimidating behavior and alleged misconduct, which warrant independent verification. At the same time, some reviewers reported recent improvements under new administration and continued examples of professional, empathetic nursing leadership.

For prospective residents and families, the pattern suggests uneven performance across units and shifts. Memory-care areas were more often associated with staffing and activity deficits, while other units were described as providing attentive care. Key items to verify during a visit include current staffing levels and ratios (especially in memory/dementia care), housekeeping and pest-control protocols, medication-administration procedures and audit practices, avenues for accessing records and escalating concerns, and the facility’s activity schedule for cognitively impaired residents. Reviewing the latest state survey findings and asking for recent corrective-action updates can help clarify whether systemic issues have been addressed.
