    Caring attentive staff and activities

I stayed here for a temporary rehab stay and was very pleased - the staff truly treated us like family and went above and beyond for my brother. CNAs, CMTs, nurses, therapy and activities assistants were kind, attentive, and engaged residents at meals and in plenty of programs (art, bingo, weekly musicians, ice cream socials, shopping trips). The facility is clean, rollator-friendly and felt safe, with good food and prompt follow-up when we had questions. I'm grateful to Pamela, Vonda, Angie, Tony Bell, Richard and Marcie for their caring, compassionate support and would recommend this caring, resident-centered place.

    Summary of reviews

Reviewer impressions of Delmar Gardens West are highly polarized. Many accounts praise individual caregivers, therapists, and social-work staff for kindness, effective rehabilitation, discharge coordination, and active programming. At the same time, a substantial set of concerns describes operational gaps that significantly affect dependent residents and families. Prospective families should weigh the facility's strengths in short-term rehabilitation and activity offerings against recurring operational weaknesses.

Care quality shows wide variability. Positive reports highlight effective therapy outcomes, compassionate hospice support, and prompt emergency coordination. Conversely, other accounts indicate lapses in medication administration, inconsistent clinical documentation, delays in respiratory and wound-related treatments, and episodic clinical monitoring failures that contributed to hospital transfers or worsening skin integrity. These issues suggest inconsistent clinical oversight and handoff practices at times, particularly for residents with high medical needs.

Staffing and staff conduct are recurring themes. Many reviewers singled out individual aides, therapists, and intake personnel as attentive and professional. However, persistent understaffing and high turnover were described as contributing factors to slow call-light responses, delayed assistance with mobility and toileting needs, and variable care consistency. Several families described concerns about tone and responsiveness from particular staff or supervisors, and others reported billing or administrative frustrations that compounded clinical worries.

Dining and activities present a mixed picture. The facility offers a robust activities program—music, art, bingo, socials, outings, and salon services—that several families valued for maintaining engagement. Food quality and accommodation of special diets received positive remarks from some visitors, but there are also reports of inconsistent meal delivery, missing utensils, and diet-specific meal errors. Dining experience may therefore depend on timing, staffing, and unit-level practices.

Facility condition and environment are described inconsistently. Positive comments note wide aisles, rollator-friendly circulation, and well-run short-term units. Critical observations point to aging infrastructure, worn carpeting, maintenance deficits, localized sanitation concerns (including kitchen and dishwasher cleanliness), and pest-control concerns. Room size and bathroom layout were described as cramped in shared accommodations, and some families raised safety concerns about the parking lot and building maintenance.

Management and systems-level issues are evident in patterns of communication and follow-up. While several reviewers praised specific managers and social workers for responsiveness, others described difficulty reaching leadership, inconsistent responses to complaints, and unresolved billing or medication questions. There are also allegations of missing personal items and falsified documentation that were escalated externally in some cases. These reports indicate variability in administrative accountability and incident-resolution processes.

Overall pattern and recommendation: Delmar Gardens West appears to offer strong rehabilitation programming and meaningful activities, supported by a number of dedicated clinicians and aides. However, the facility also shows recurring operational weaknesses—notably staffing instability, inconsistent clinical monitoring, sanitation and maintenance issues, and communication shortfalls—that disproportionately affect higher-acuity residents. Families considering this facility should conduct a focused, in-person assessment that includes observations of staffing levels on the relevant unit and shift, review of medication- and wound-care protocols, clarification of memory-care placement practices, and verification of incident-reporting and item-security procedures. Understanding which units and which staff deliver the strengths versus where deficits occur will be important for placement decisions.

    About Delmar Gardens West

Delmar Gardens West sits at 13550 S Outer Forty Rd in Chesterfield, Missouri, right in a quiet neighborhood, and folks know it as a place for many different kinds of senior care, so you can find skilled nursing, short-term rehabilitation, memory care for seniors living with Alzheimer's or dementia, and some home care services too, and while it's often listed as a nursing home, you'll also see independent and assisted living apartments, including help with daily needs like getting dressed or bathing, which is good if someone's care needs change down the line because it means there's a whole range of services under one roof and moving again isn't always needed, and there's even daycare options if someone needs a break or just short-term help.

There's staff on site all hours of the day and night-the nursing team provides 24-hour care and the Medical Director is on call if something happens-plus licensed therapy for physical, speech, and occupational needs, and folks can get wound care, diabetic care, help with mobility, and pharmacy services all without leaving the building, and because Delmar Gardens West covers things like laundry, housekeeping, and meals (usually one or two a day for those in independent or assisted living), there's less for residents to worry about, and there's plenty of space inside for social and religious activities, including big activity rooms, social events, a beauty salon, and flat-screen TVs with cable.

Parking's easy and visits are comfortable since the campus is wheelchair accessible, there's air conditioning, and guests can use credit cards; people can also bring pets-both cats and dogs are welcome-so there's flexibility for families, and for those who need to get out, transportation is available, plus religious services and offsite trips are planned sometimes. The facility offers semi-private and private rooms with some nice furnishings and grab bars in the bathrooms to help with safety, and there's an emergency call system if someone needs help fast.

Reviews can be mixed-Delmar Gardens West holds a 2.5 rating from about 35 reviews, so folks looking should keep that in mind. The place gets attention from health agencies, it's associated with the Missouri Health Care Association and connects to programs at WashU Medicine through the Division of Nutritional Science & Obesity Medicine. There's resource guides for long-term care, insurance, and caregiving, and the facility works with sponsors and partners to provide services. They run CNA and CMT / Insulin training along with online education, which helps keep staffing trained and up on quality assurance, so staff members know what they're doing and follow regulations, and the care finder tool might help families figure out what care fits best for their loved ones.

Home care services from Delmar Gardens Private Duty extend attention beyond the main building, with things like medical supplies, hospice, palliative care, and private duty assistance in client homes; meanwhile, there's also a dedicated early childhood education center on the campus, a bit unique for a senior living place, and onsite health, pharmacy, and wound management round things out as options for folks there. All in all, Delmar Gardens West offers a long list of care styles in one location, with basic comforts like meals, activities, accessible rooms, and healthcare, but like with any facility, it's wise for families to visit, ask questions, and decide what fits the person needing care best.
