    Warm, clean facility; excellent care

I placed my loved one here and have been very pleased - the staff are kind, professional and attentive, the administrator is hands-on, and care/rehab have been excellent. The facility is clean, warm and well-maintained with engaging activities and clear communication; I feel confident and reassured. A few small improvements would make it outstanding, but I would still highly recommend it.

    Summary of reviews

Reviewers describe a facility with clear strengths in interpersonal care, rehabilitation services, and the physical environment, alongside recurring operational weaknesses that families should evaluate during a tour.

Care and staff: Many accounts praise compassionate, respectful interactions and note a knowledgeable clinical and therapy staff that delivers strong rehabilitation outcomes. Nursing and aide teams are often described as attentive and professional, and several reviewers expressed gratitude for supportive, patient-centered care—particularly for short-term rehab and end-of-life support. At the same time, there is a pattern of variability in staff conduct and availability. Reviewers raised concerns about inconsistent staffing levels (notably at night), which reviewers link to delayed responses and safety risks during transfers or assisted sitting/feeding. Families should inquire about staffing ratios, overnight coverage, and transfer-safety protocols.

Dining and nutrition: The facility receives mixed feedback on dining. Some families appreciate varied menus and effective accommodation for food allergies, while others noted that therapeutic diets (for example, heart-healthy plans) were not consistently implemented as expected. There are also mentions of feeding-safety practices that warrant clarification. Prospective residents and families should ask how therapeutic diets are documented and monitored, how feeding assistance is trained and supervised, and how kitchen–nursing communication is handled.

Activities and facilities: The physical environment is frequently described as clean, bright, and calming, with pleasant views, accessible outdoor space, and communal rooms that support engagement. Activities such as hymn singing and organized social programs are highlighted positively. However, there are operational concerns related to sanitation and insect control in some rooms and dining-adjacent areas, as well as corridor clutter (wheelchairs in hallways) that can impede safe egress and movement. Clarify housekeeping schedules, pest-control practices, and corridor-storage policies during a visit.

Management and operations: Leadership visibility receives both positive and critical remarks. Several reviewers single out hands-on administrators and engaged leadership for facilitating strong care and communication; others describe inconsistent administrative responsiveness or unmet commitments (for example, promised clinical follow-up). Communication strengths include effective family updates during successful transitions, but there are also reports of discharge-communication gaps and incomplete follow-through on scheduled appointments. Additionally, issues such as lost or misassigned personal items and laundry-handling concerns suggest a need to review personal-belongings protocols and clothing labeling procedures.

Notable patterns and recommendations: Overall, the facility appears to deliver high-quality rehabilitation and demonstrates many strengths in staff compassion, therapy expertise, and the physical environment. Concurrently, recurring themes—staffing variability (especially overnight), inconsistent cleanliness/sanitation in some areas, lapses in therapeutic-diet implementation, transfer/feeding-safety practice gaps, and personal-belongings security—warrant attention. Prospective residents and families should tour during different times (including evenings), ask for care-staffing ratios, request documentation of therapeutic-diet plans and infection-control measures, observe housekeeping practices, and clarify discharge and appointment-follow-up procedures. These targeted questions will help determine whether the facility’s operational practices align with a family’s safety and care expectations.

    About Rolling Hills Healthcare Center

Rolling Hills Healthcare Center sits on a large, 43-acre country property in Annandale, New Jersey, not far from I-78 and several major roads in Hunterdon County, and the setting's peaceful with lots of green views, which many people say helps them feel calm and recover easier. The center provides skilled nursing care, long-term care, hospice care, respite care, and a full set of rehabilitation and therapy programs, including fast-track therapy after surgeries and for people who've had strokes or joint replacements like hips and knees, and does a lot for folks needing post-surgical therapy, wound care, cardiac care, IV therapy, tracheotomy, and gastrostomy care. The staff helps people transfer smoothly from the hospital and takes Medicare, Medicaid, many insurances, and private pay, with admissions possible every day of the week, and there's a Director of Admissions who helps with insurance and explains benefits, which can be helpful during stressful times. Residents can get help with daily activities if they need it, and family caregivers can also get support and advice about long-term care planning and resources when they're trying to figure everything out, and Rolling Hills offers respite programs to give caregivers a break, too. There are also specialized programs for people with Alzheimer's and dementia, as well as assisted living communities for those needing some help but who still want to live as independently as they can, plus adult day health centers that give people a chance to spend time in a social setting during the day. Some parts of the facility are set up for psychiatric care and behavioral health with outpatient options and services for adults coming out of hospital stays, and the whole system is tied into the Ocean Healthcare Network, so that helps them coordinate care if people need different services or move around between levels of care. The center's staff members are known for being friendly, caring, and sincere, and the place is kept clean and comfortable with good meals and simple amenities, and it feels small, friendly, and professional instead of big and crowded, which a lot of families like. You'll also find special programs like HeartSMART, Triple Care™, and the Center for Pulmonary Excellence for people with heart and lung conditions, which are available to support more complex needs. Rolling Hills works closely with local hospitals and doctors and helps people get back on their feet with both short-term and long-term rehab, whether it's from Hunterdon Medical Center, RWJBarnabas Somerset Campus, or St. Luke's Warren Campus. There's always a focus on helping people heal, stay safe, and live with dignity, and though supervision is always in place, it's important for families to stay involved and ask questions about daily care if they have any concerns. The center offers a wide range of care services, from rehab to extended or respite stays and everything from home-based nursing to hospice, all while trying to keep things warm, welcoming, and accessible for people not only in Annandale, but also through the wider New Jersey area.
