    Excellent long-term care and rehabilitation

I stayed here long-term and saw real improvement - pain relief, strong rehab, and attentive follow-up on medical issues. The staff were consistently caring and professional (Nurse Kathleen, Grant, Roxanne and SWs like Ms. Afua/Mamadu stood out), the food and recreation felt like family, rooms were bright and clean, and new management has brought positive changes - overall a very positive experience I'd recommend.

    Summary of reviews

Reviews of Riverdale Rehabilitation and Nursing Care Center present a highly mixed picture, with clear examples of both strengths and serious operational weaknesses. Positive accounts highlight compassionate bedside care from specific nurses and CNAs, an active rehabilitation program, coordinated medication and appointment handling, and a recreation program that includes live music and social activities. Several reviewers also praised admissions and social-work staff for being helpful and accessible, and some units or newly renovated areas were described as clean and bright.

At the same time, a substantial portion of feedback identifies persistent sanitation, maintenance, and safety concerns. Descriptions of odor issues, cleaning lapses in resident rooms and bathrooms, HVAC/unit humidity problems, and visible maintenance defects suggest gaps in routine environmental upkeep and infection-control practices. Multiple mentions of pests and pest-control concerns, broken fixtures, ceiling leaks, and general facility wear indicate deferred capital maintenance and cleanliness processes that require attention.

Clinical and caregiving concerns are a recurring theme. While many families noted attentive and compassionate caregivers, others reported inconsistent responsiveness from nursing staff, lapses in personal-care routines (including bathing and clothing management), delayed recognition or treatment of infections and pain, and difficulties accessing or communicating with attending physicians. These items point to gaps in clinical oversight, handoff processes, and care coordination. Safety issues described by reviewers—including falls, injuries, and what families perceived as inadequate monitoring—raise additional questions about staffing levels, training, and resident-supervision practices.

Dining and programming impressions vary. Some residents and families described meals as excellent and recreation as engaging, particularly music offerings, while other accounts flagged poor meal quality or a lack of meaningful activities for certain populations. Rehabilitation services are frequently cited as a positive, but opinions on therapy effectiveness are inconsistent across stays.

Management and organizational themes include reports of weak accountability, variable leadership performance, and communication breakdowns with families. A number of reviewers noted improvement under newer leadership, while others described persistent administrative gaps such as limited access for outside physicians, unclear incident communication, and concerns about regulatory compliance. Language barriers and inconsistent staff training were also noted as factors that affect day-to-day experience.

Overall pattern: the facility appears to be uneven in performance—some units, shifts, or personnel deliver good care and programming, while other areas exhibit measurable operational weaknesses in sanitation, maintenance, staffing consistency, clinical oversight, and family communication. Prospective residents and families should consider an in-person tour focused on unit-level cleanliness and safety, request current infection-control and pest-control logs, inquire about staffing ratios and physician access, and review recent state inspection and complaint histories to assess whether the facility's strengths align with their clinical and safety needs.
