    Attentive staff clean facility recommended

I'm very pleased with my experience - attentive, friendly nurses, aides and staff; spotless, bright rooms and beautiful grounds; engaging activities and strong PT/rehab that helped my loved one improve. Meals and family communication were good, and my relative felt safe, comfortable and happy. I would recommend this facility.

    Summary of reviews

The reviews of EverGreen Commons Rehabilitation and Nursing Center present a highly mixed picture. Positive accounts consistently highlight compassionate direct-care staff, effective rehabilitation services, and a range of social and wellness offerings. Families and residents frequently praise the therapy teams for measurable functional gains, the availability of activities (music programs, church services, salon), well-kept common areas and grounds, and comfortable rooms. Several reviewers also noted strong infection-control practices during COVID, accessible social work and admissions staff, and welcoming family policies that support involvement in care and discharge planning.

At the same time, a substantial number of reviews point to operational weaknesses that materially affect day-to-day care. Chronic understaffing and reliance on agency staff are recurring themes; these staffing gaps are associated with delayed responses to call bells, inconsistent assistance with toileting and bathing, and prolonged waits for pain or other PRN medications. Multiple reviewers described inconsistent supervision on some shifts — particularly nights — and variability in care quality between different units or floors. Together these items suggest uneven staffing models and variable adherence to routine care processes.

Clinical safety and process gaps also emerge repeatedly. Concerns about medication management and documentation appear across reviews (medication errors, medicines administered or altered without clear family communication, and medications handed to families at discharge). Several families described problems with discharge planning and clinical oversight, including disagreements over readiness for discharge and continuity of care after transfer. Billing and financial-accountability issues — unitemized charges or disputed balances — were noted frequently enough to be a pattern-level concern. Security and property-management gaps (missing items, questions about camera coverage) were raised as well.

Dining and housekeeping show a mixed record. Many reviewers complimented the food and dining staff and described pleasant meal experiences and accommodating dietary staff; others reported stale or cold meals, limited dinner options, and interruptions in meal-service continuity. Housekeeping and laundry are generally praised for well-maintained common areas, but room-level cleanliness, linen turnaround, and odor concerns (often tied to incontinence care) were reported in multiple accounts, pointing to inconsistencies in environmental services or workload-driven lapses.

Notable patterns include a clear polarization of experiences: some residents and families describe consistently high-quality, compassionate care and excellent rehabilitation outcomes, while others describe operational failures that pose safety and dignity risks. Praise for specific managers and staff members is common, indicating pockets of strong leadership and high-performing teams; conversely, critiques about management responsiveness, staff morale, and HR practices (including turnover and disciplinary approaches) suggest organizational stressors that can degrade consistency.

For prospective residents and families: EverGreen Commons demonstrates strong rehabilitation capabilities and a range of social supports, and many find the personal care and therapy excellent. At the same time, families should directly assess the facility’s staffing levels for the intended unit and shifts, ask about agency-nurse use and staff turnover, review medication-management and discharge protocols, inquire about night supervision and security measures, and observe mealtimes and housekeeping during a visit. Request specific answers about billing practices, itemized charges, and complaint/incident escalation policies. Touring multiple units and speaking with the unit manager and therapy staff can help clarify whether a particular unit’s strengths align with an individual resident’s clinical and supervision needs.

    About EverGreen Commons

EverGreen Commons Rehabilitation and Nursing Center sits at 1070 Luther Rd in East Greenbush, New York, where you'll find it surrounded by landscaped park-like grounds with walking paths and even a playground for young visitors, so folks can enjoy their time outdoors or just watch the seasons pass if that's what they like. The facility has 240 licensed beds and provides both short-term rehabilitation and long-term skilled nursing care, so people can recover after an illness or surgery or stay longer if they need more help with daily living. There's a secure care unit for those who need an Alzheimer's or dementia program, and trained nurses and healthcare professionals are on staff to manage specialized care and support, while housekeepers and a laundry director keep things clean, so everyone's comfortable.

Staff members often get recognized for being helpful, joyful, and kind, which can make EverGreen Commons feel welcoming and friendly, almost like a small community of its own, and there's a focus on both physical and mental health, since emotional well-being matters just as much as medical needs, so activities and recreation therapy are a daily part of life here. The place also offers various therapies, including physical therapy and pet therapy, to help with recovery and brighten up the day, and there's restaurant-style dining where nutritious meals are made from good ingredients, with therapeutic diets available when someone needs something special, and guest and family visits are welcome.

Respite care is offered for caregivers who need a break, and there are resident and guest services, which include laundry and housekeeping, all taken care of by the professional team. EverGreen Commons is privately owned and has been a member of the Rensselaer County Regional Chamber of Commerce since 2016. The community scores 3.7 based on 37 reviews, and 89% of those are five stars, so most people seem pretty satisfied, though like anywhere, not everything is for everyone. The place is managed by Emily Giordano and continues to provide a range of inpatient health care and rehab programs-physical, social, emotional, and spiritual-because everyone's needs change as time goes on, and it's important that a place like this tries to meet those needs as best it can.
