I'm very pleased with Park Gardens - the staff are consistently kind, caring and professional, nurses, aides and therapists communicate proactively and supported my loved one's recovery, and the social worker was extremely helpful. The facility feels warm, clean and safe, with good food and activities; I feel my family member is well cared for and I highly recommend it.

    Summary of reviews

Park Gardens elicits sharply divided impressions. Many family members and residents praise the clinical and rehabilitative care, especially the hands-on attention from nurses, aides, and therapy staff, as well as the social-worker support and an active activities program. At the same time, other families describe operational and environmental shortcomings that meaningfully affect the resident experience.

Care and staff: There is clear strength in direct caregiving for numerous residents — reviewers highlight attentive nurses and aides, knowledgeable charge nurses, effective PT/OT services, and a social worker who communicates and advocates for families. Multiple accounts describe compassionate, empathetic interactions and staff who help residents feel safe and respected. Conversely, there are recurring concerns about inconsistent staff conduct and professionalism, including complaints about rude or dismissive behavior and delays in responding to calls. Staffing levels and RN coverage are also noted as uneven, which reviewers link to slower response times and gaps in supervision.

Dining and therapy: Therapy services (physical and occupational) receive consistently positive remarks for supporting recovery and mobility. The kitchen is credited with accommodating special dietary needs and requests; however, opinions on meal quality vary, with some families satisfied and others less positive. Overall, rehabilitative services appear to be a reliable element of the facility’s offerings.

Activities and social support: The activities program and its director receive praise for providing engaging opportunities and contributing to a pleasant day-to-day atmosphere. Social work services are a prominently positive feature, with families valuing proactive outreach, care coordination, and updates about health and progress.

Facilities and cleanliness: A notable pattern of concern centers on facility upkeep. Several accounts reference sanitation and odor concerns in common areas and occasional lapses in cleanliness. Capacity pressures, including crowded rooms and reports of residents being unattended in common corridors, are mentioned and suggest operational strain in some units. These environmental issues appear to be a primary area where experiences diverge sharply.

Management and communication: Experiences with management and administrative communication are mixed. Some families describe proactive, clear updates and easy access to staff, while others report phone-response failures, long hold times, and trouble reaching a manager. These communication inconsistencies compound other operational concerns and can leave families feeling uncertain about care continuity.

Notable patterns and guidance: The facility’s strengths cluster around direct caregiving, therapy, activities, and social-work outreach. The most consistent operational weaknesses are cleanliness/odor management, staffing levels and RN coverage, and communication responsiveness. Prospective residents and families should weigh the positive clinical and rehabilitative capabilities against these environmental and operational concerns. If considering Park Gardens, families may wish to ask about current staffing ratios, housekeeping schedules, infection-control and odor-management practices, medication-management protocols, and the facility’s policies for family communication and resident leave to better understand how the facility is addressing the issues noted by some reviewers.

About Park Gardens Rehabilitation & Nursing Center

Park Gardens Rehabilitation & Nursing Center sits right in Riverdale, NY, where it helps folks with rehab and nursing care in a pretty straightforward way, being part of a bigger Continuing Care Retirement Community so people can stay as their needs change, and, well, they do have both long-term nursing care and short-term rehab for those coming out of the hospital, plus 200 certified beds, though about 17 were open in June 2025, and there's usually 183 residents each day. The staff helps with activities of daily living like bathing, dressing, and moving around, and they offer medication management, skilled wound care, IV therapy, and a good range of medical services including dentistry, psychology, and ophthalmology, while the nurse staffing runs about 2.99 hours per resident each day and they've got care coverage all day and night, though the nurse turnover rate is up at 22.6%, which can mean some different faces over time. The center accepts both Medicare and Medicaid, has bilingual caregivers (but mostly speaks English), and they offer a steady list of activities-movie nights, music, fitness and wellness programs, a game room, library, garden walkways, and resident-run groups-trying to keep things lively. Residents and families can join organized councils if they want a say in how things go, and you'll find an emergency system and supervising staff round-the-clock. There are some things worth noting, like deficiencies found in reports for not always making complete care plans or meeting new residents' needs right away, and inspectors have noted issues with protecting belongings and infection control, though no actual harm reported with those items, and the inspection process does keep the attention on these matters. The place makes therapy programs to fit individual needs, and tries to create a warm, home-like setting, with visits welcome daily from 10:30 in the morning to 8:30 at night, so family can be part of everyday life whenever possible. You'll find public transportation nearby, and if you need the home health services, those run from 8:30 to 4:30 and are right at 6677 Broadway. Park Gardens does aim to offer a real range of restorative therapies for people with different needs, and while they do their best to provide for folks in their care, they've got some areas needing attention too, which is really something to consider.
