    Welcoming staff and effective rehab

I'm pleased with my experience - the administrator and new staff have been welcoming and attentive. Nurses and aides (Tasha, Jamie, Jackie, Joyce, Keisha) provided compassionate, professional care and short-term rehab was effective. The building is clean and home-like, activities are creative and well-run, residents seem happier, and I'm encouraged by recent improvements and look forward to continued progress.

    Summary of reviews

Reviews for Durham Nursing & Rehabilitation Center present a mixed and polarized picture. Many families and visitors praised individual caregivers — including nurses, CNAs, and therapy staff — for compassionate, attentive support and effective short-term rehabilitation outcomes. The activities program, cited for card games, coffee socials and other engaging options, is frequently described as energetic and resident-focused. Hospice and end-of-life services were also noted as supportive where used.

At the same time, a consistent set of operational concerns appears across reviews. Staffing levels and scheduling reliability were prominent issues; reviewers described chronic shortages and turnover that contributed to delays in attending to residents and inconsistent coverage across shifts. Several comments indicated that frontline staff feel undervalued, and that supervision and conduct oversight are uneven, creating variability in how care is delivered.

Facility-environment issues were another recurring theme. Housekeeping and some short-term rehab areas received positive comments, but other areas raised sanitation and pest-control concerns as well as maintenance problems such as lighting failures and ceiling leaks. Reviewers also described problems with laundry and management of residents’ personal belongings. Together these issues point to uneven environmental oversight and maintenance practices.

Clinical- and process-related gaps were also reported. Families raised concerns about continence-management practices and device-related infection risk, inconsistent medication handling, and problems with discharge paperwork. Communication weaknesses compounded clinical worries: reviewers described delays or lack of timely family notifications during infection-control events and difficulty reaching staff by phone.

Management impressions are mixed. Some reviewers noted a new administrator and recent hires with a hopeful tone about improvements, while others described organizational disarray and a director-of-nursing role perceived as unsupportive. Notable patterns are the facility’s clear pockets of strength — committed caregivers, strong therapy and activities — alongside systemic operational weaknesses. Prospective residents and families should weigh the demonstrated strengths in direct care and programming against recurring concerns about staffing reliability, environmental maintenance, clinical-process consistency, and communication, and consider in-person visits and direct questions to leadership about current corrective actions and staffing levels before deciding.

About Durham Nursing & Rehabilitation Center

Durham Nursing & Rehabilitation Center sits in Durham, North Carolina, where it helps seniors with both short-term rehabilitation and long-term stays, and the place has 126 certified beds along with an average of 85 residents each day. The center has two administrators, Mrs. Michelle Baldwin and Ms. Sharla A. Haugh, who oversee the nursing home, and the facility holds license number NH0136 from the state. The staff provide 24-hour skilled nursing care, and they offer therapy services, wound care, psychological help, and even things like hospice, palliative, and respite care for different needs, and you'll also find registered dietician services as well as respiratory and trach care programs for folks who need that extra support. The property includes private rooms, community amenities, beauty and salon services, and even complimentary transportation with shuttle services, so residents get to medical appointments and errands. The center is affiliated with Maximus Healthcare Group, Kindred Healthcare, and the North Carolina Health Care Facilities Association, and it's tied to a larger group that works with hospitals, pharmacies, and special rehab programs, and even has links to LaSalle Healthcare Center. The place generally gets good feedback, with review scores of 9.0 from 45 people and 9.2 from 47 people, most from families and residents themselves. It has a gallery and virtual tour online, but sometimes it's hard to find all the details because some information can be missing due to system errors. For medical support, the nurse staffing stands at 3.31 hours per resident each day, which is a bit below the state average, and nurse turnover is 60.9%, which runs higher than the North Carolina average of 50.9%. Ownership of the center is split among Yisroel Bornstein, Nisson Hirsch, and Yaakov Weitman, who hold different percentages. The facility has had a few deficiencies on reports, such as issues with ensuring residents' assessments are accurate, accommodating preferences, and pharmacy labeling or storage, but each one was marked as no actual harm but with the potential for more than minimal harm, and recent inspections, with the last on January 16, 2025, show staff working to follow rules. The care always focuses on an individualized plan for each resident, aiming for both mental and physical wellness, and there's a team approach with nurses, aides, and therapists involved. Parking and community gathering spaces are part of what you get, and the center tries to nurture a sense of community for everyone living there.
