    Supportive staff restored my independence

I had a very positive stay - the staff truly felt like family. Nurses, CNAs and therapists (special thanks to Angie and Brittany) were attentive and professional, getting me back to functional independence; housekeeping kept everything spotless and activities/amenities (ice cream socials, events, internet café) made my days enjoyable. Communication and discharge planning were thorough, and I felt well cared for; I highly recommend this facility.

    Summary of reviews

The reviews present a strongly mixed but actionable picture of Elizabeth City Health & Rehabilitation. On the positive side, the facility is frequently praised for its therapy services and rehabilitation outcomes: physical and occupational therapy teams receive consistent commendation for helping residents regain function and return home. Many families also describe compassionate direct-care staff, including nurses and nurse assistants, who provide attentive, person-centered care; individual staff members and social-services personnel are singled out for above-and-beyond efforts. Several reviews note well-kept, comfortable rooms and common areas, an inviting atmosphere, and engaging activities ranging from arts-and-crafts to special events and daily treats.

Clinical care quality appears uneven. While therapy and some nursing practices are strong, reviewers describe operational lapses that affect day-to-day clinical reliability: delays in medication administration, slow or inconsistent response to call lights and care requests, and gaps in monitoring and escalation processes. A number of families described situations that indicate weaknesses in the facility’s incident-response procedures and in communication to families during clinical events; these concerns include delayed transfers to higher levels of care and inconsistent updates to family members.

Dining and nutrition receive mixed feedback. Positive comments include enjoyable meals and occasional special dinners, while negative comments emphasize inconsistent meal quality, difficulty identifying menu items, and failures to adhere to dietary restrictions (for example, salt- or sugar-controlled diets). Food-service continuity and accuracy of diet orders appear to be an operational area in need of attention.

Facility and environmental strengths are clear in many accounts: rooms described as hotel-like, an overall clean appearance in patient spaces, and amenities such as an internet cafe and active programming that support resident quality of life. However, other reviews raise sanitation and maintenance concerns in certain areas—notably odor concerns in some restrooms and common areas, blocked or repurposed bathing spaces, and intermittent hot-water problems—which suggest uneven maintenance practices across the facility.

Management, administration, and business-office functions are another recurring theme. Several reviewers express frustration with administrative communication, perceived favoritism, inconsistent follow-through on family questions, and billing-office professionalism. There are also more serious operational concerns raised about belongings security, record-keeping transparency, and infection-control/outbreak management; some accounts describe alleged missing items and inadequate documentation practices, which warrant further inquiry by prospective families and regulators.

Overall pattern: experiences at Elizabeth City Health & Rehabilitation range from highly positive—especially around therapy, dedicated individual caregivers, and certain amenities—to materially problematic in operational reliability, communication, and some aspects of environmental maintenance. Prospective residents and families should weigh the facility’s strong rehabilitative capacity and examples of compassionate care against the documented variability in staffing responsiveness, medication timing, dining consistency, administrative communication, and facility maintenance. When considering admission, recommend asking for up-to-date information on staffing ratios, medication-administration protocols, infection-control policies, dining accommodations, property-security procedures, and how the facility communicates clinical incidents to families.

About Elizabeth City Health & Rehabilitation

Elizabeth City Health & Rehabilitation sits on US Highway 17 South in Elizabeth City, North Carolina, and it operates as a for-profit limited liability company linked with Sanstone Health & Rehabilitation, offering both short-term rehabilitation and long-term care. The facility can care for about 135 residents each day and has 170 certified skilled nursing beds. The care team, which includes a Director of Nursing, an Assistant Director of Nursing, and an Activities Director, provides around-the-clock support, and they focus on helping residents with a variety of needs, such as physical therapy, occupational therapy, and speech therapy. The staff gives an average of 4.07 nursing hours per resident per day, but the nurse turnover rate stands at 26.7%. The facility follows tobacco-free rules, and it has wound care and skin care services as well. People stay in either semi-private or private rooms, and there's a rehabilitation suite where families can spend the night close to their loved ones. A gym is used for fitness and therapy, and a beauty parlor, lounges, a café, and an outdoor gazebo offer places to relax or visit. Planned events and activities are held often, making a lively community. While the facility has a record with two infection-related deficiencies, it keeps a standard of care that aims for good rehabilitation results. A dedicated healthcare team works across different areas to make sure residents receive proper help for recovery and long-term wellbeing.
