    Compassionate staff, strong leadership, rehab

I'm very pleased with the compassionate, skilled staff and strong leadership from Rich and the DON-admissions and Crystal were wonderful to work with. The facility is clean, family-like, offers activities, salon and rehab that improved mobility and got my loved one home; staff went the extra mile and handled appointments reliably.

    Summary of reviews

Reviewers described highly mixed experiences at Greenhaven Health & Rehab, with clear areas of strength alongside recurring operational concerns. Positive accounts emphasize compassionate direct care, effective rehabilitation services, and supportive admissions and therapy staff. Several families noted that therapists and clinical teams helped residents regain mobility and successfully return home, and some praised amenities such as an on-site hair salon, activity programming, transportation to appointments, and a generally welcoming atmosphere in parts of the facility.

Conversely, a recurring theme is variability in basic care delivery tied to staffing and training. Many accounts describe limited staffing levels that result in delayed responses to calls for assistance, postponed toileting and bathing, and late or cold meals. These delays are framed as systemicโ€”rooted in staffing shortages and schedulingโ€”rather than isolated incidents. Reviewers also raised concerns about inconsistent clinical competency among nursing staff, including medication-management problems (for example, medications discarded without family notification) and instances that reviewers perceived as dismissive or unprofessional behavior from certain caregivers or supervisors.

Communication and family engagement are additional areas of concern. Multiple reports note difficulty reaching the front desk or nursing station by phone and inconsistent follow-up when family members attempt to check on residents. Leadership impressions are mixed: some families praised visible improvements under certain leaders, while others described management as disorganized or unresponsive and cited confrontational interactions. There are also notable allegations regarding missing personal belongings and possible mishandling of laundry and personal items; these claims are serious and warrant direct inquiry during any placement decision.

Facility operations and environment were described inconsistently. Positive remarks include clean, well-maintained rooms and lack of odor in some units, but other reviewers reported sanitation and odor concerns in common areas and a general need for building maintenance. Dining quality was unevenโ€”some residents experienced acceptable meals and improved mobility supported by good nutrition, while others described cold meals and schedule lapses. Activity offerings and community outings were a consistent positive when available, contributing to social engagement for residents.

Finally, some reviewers raised safety-related issues such as perceived gaps in emergency-response capability and instances that suggested inadequate urgent clinical oversight. Given the mixed feedback, prospective residents and families should verify current staffing ratios, review medication-handling procedures, confirm processes for tracking personal belongings, observe mealtime service, and speak directly with therapy and nursing leadership. The facility appears capable of delivering good rehabilitative outcomes and compassionate care in some cases, but persistent operational weaknesses make experiences variable and call for careful, individualized evaluation before placement.
