    Spotless facility, compassionate, responsive, effective

I placed my mother here and have been very pleased. The facility is spotless with private rooms, outdoor patios, good food and daily activities, and the therapy is effective. The nurses, CNAs and staff are professional, compassionate, responsive and often go above and beyond - I feel my loved one is in very good hands and would recommend this place.

    Summary of reviews

Overall impression: Reviews describe Madison Health and Rehabilitation as a generally clean, secure, and welcoming facility with strong rehabilitation services and a culture of resident engagement. Many families praised the facility's physical environment, outdoor patios, pleasant views, and the organization of social events that contribute to a homelike atmosphere. Dining and snack offerings are viewed positively, and residents are described as participating in daily activities that support social interaction and recovery.

Care and staff: Care quality is commonly framed as a strength. CNAs and many nurses receive consistent praise for compassionate, hands‑on care, resident engagement, and professionalism. Several reviews highlight prompt nursing responses and effective therapy that helped residents progress toward discharge. At the same time, there is a clear pattern of variability in staff performance: some reviewers describe delayed morning routines, uneven empathy or responsiveness, and skepticism about certain staff reports or assessments. References to competency checks and active department leadership suggest some investment in staff oversight, but experiences vary across shifts.

Facilities, dining and activities: The facility environment is frequently described as very clean and well maintained, with staff who present professionally. Outdoor spaces, private‑room availability, and common‑area programming (celebrations, activities) are noted as meaningful quality‑of‑life features. Food quality and snack availability are generally seen as positive, and families report that activities contribute to residents’ mood and engagement.

Management and operational patterns: Communication from leadership is sometimes praised—families say they were kept informed about care by department heads and corporate staff—and some reviewers report management that listens to CNAs. Conversely, other reviewers describe management inconsistencies, unclear or seemingly arbitrary visitation and checkout rules, and variable responsiveness when concerns are raised. Staffing consistency emerges as an operational risk: weekend coverage and second‑shift performance are specifically called out as weaker periods. Housekeeping performance is also described as uneven in some accounts.

Takeaway for families: Madison Health and Rehabilitation appears to deliver solid clinical and rehabilitative care in a clean, secure environment with many compassionate direct‑care staff and enriching activities. Prospective residents and families should weigh that generally positive baseline against reported variability in staffing coverage and operational consistency. It is advisable to ask about weekend and night staffing levels, clarify visitation and discharge procedures, and observe housekeeping and shift handoffs during a tour to confirm that the facility’s strongest practices match your expectations and needs.

    About Madison Health and Rehabilitation

Madison Health And Rehabilitation sits out in Mars Hill, North Carolina, at 345 Manor Road, with 100 SNF beds, and it's one of those places that tries to make everything as steady and safe as they can for folks who need more help than they used to, whether for short stays or longer term, and you'll find both private and semi-private rooms there, which gives families different choices. The staff includes a Director of Nursing, Assistant Director of Nursing, Admissions and Marketing, a Business Office Manager, an Activities Director, Accounts Payable, people for general inquiries, and the Therapy department, and the administrator there is Haley Niebes, so you know there's always someone to help steer the ship.

They offer 24-hour skilled nursing care supervised by a physician, and have the proper qualifications for Medicare and Medicaid. You'll find both skilled and intermediate nursing services, and the place can store and give out medicines as needed. They provide therapy services both as an inpatient and outpatient, which means physical, occupational, and speech therapies are all right there if you need them and want to maximize whatever recovery is possible, and there's a dedicated wound care RN on staff for those who need more attention for healing up. The therapy team includes physical, speech, and occupational therapists, and they work together to plan out what's needed for each resident. Therapy services reach beyond just exercises by helping folks reach the best improvement possible, and they personalize treatment plans, so it doesn't feel like one-size-fits-all.

For those worried about support, the community there tries to foster a healing environment, keeping things close-knit and helping everyone feel included, and you'll see that reflected in the resident and family lounge, which is called The Lodge, where people can spend time together. Meals get prepared with quality ingredients, and nutritional care is included in the services, while activities and events are always on the schedule for folks looking for something to do, whether it's just entertainment or something a little more involving. There's a tranquil courtyard with a koi pond and turtles, and residents can meet their visitors at a café or use the gym if they're feeling up for it. Entry is secured with 24-hour visitation allowed, though the doors stay locked outside of 7:00 am to 8:00 pm, which helps keep folks safe after dark. There's even a covered smoking porch for those who still smoke. Medicaid, Medicare, and private payments are all accepted, so there are choices that fit different situations. The facility's also associated with A Place For Mom, so families can find expert advice if they need more information on care options.
