    Kind staff, lost items, problems

I have mixed feelings. The rehab/PT team, CNAs and many nurses were incredibly kind, attentive and helped my loved one make real progress in a clean, activity-rich facility. But I also experienced lost belongings (clothes, dentures, partial bridge), missed meds and pain management, staffing shortages, ignored injuries, terrible food, noisy dementia residents with little memory-care support, and evasive management - so despite wonderful frontline staff I can't fully recommend it.

    About Autumn Care of Salisbury

Autumn Care of Salisbury stands at 1505 Bringle Ferry Road in Salisbury, North Carolina, and focuses on caring for seniors through skilled nursing, assisted living, and memory care services, offering both short-term rehabilitation and long-term care, and though they've got a licensed capacity of 97 beds-sometimes noted elsewhere as 68-they aren't taking new patients right now, but those who are there experience a homelike atmosphere with a full schedule of activities and events like Bingo, holiday parties, Singing Christmas Tree concerts, "Laugh for Life" laughter yoga, live music, and even big Super Bowl parties with giveaways and tailgate cookouts, all to keep folks engaged and feeling like part of a real community. Residents receive meals prepared by chefs and meal planners who care about balanced nutrition and quality ingredients, and the facility has indoor common spaces, an on-site chapel for worship and special events, and an outdoor area with a gazebo for fresh air. The staff at Autumn Care of Salisbury aim to treat their residents like family, with care tailored to each individual; there's a dedicated dementia program, specific memory care, and plenty of attention to personalized care plans, including specialized programs designed to help meet each resident's needs for comfort, independence, and health. Seniors can take advantage of rehabilitation equipment, an on-site therapy room for physical, occupational, and speech therapies, and pre-surgery exercise programs, plus the chance to meet with an interdisciplinary therapy team before ever moving in, which makes the transition from hospital to home a little smoother thanks to a structured pre-registration process. Healthcare services cover diabetic care, incontinence care, high acuity care, non-ambulatory care, and ongoing collaboration with physicians to make sure recovery plans are right for each person, and whether someone needs help with personal care, transportation to nearby medical appointments, or wants to take part in devotional activities, staff handle support professionally and with compassion. The team speaks mostly English, though a few staff are fluent in other languages, and clinical leadership contributes to making sure customized service plans keep the focus on person-centered care, where beneficial outcomes matter, and the community is part of Consulate Health Care, who encourage staff to care for residents as though they were their own family. The building holds the designation of Skilled Nursing Facility and Physical Medicine and Rehabilitation Clinic, and folks say Autumn Care tries to make life as comfortable as possible while providing the health support seniors often need, whether for a short rehabilitation stay, a bit of respite, or for someone needing a long-term home. The administrator, Donna Rose, along with dedicated professionals, work with the North Carolina Health Care Facilities Association and strive for high standards in medical and nursing care, but at its heart, Autumn Care keeps daily life easy, social, and safe for senior residents living there.
