    Clean caring facility restored independence

I had a very positive stay in a clean, safe, family-like facility with kind, professional staff. Nurses, STNAs and the therapy team were attentive and dedicated - PT/OT helped me regain independence and walk home without a walker. My large, comfortable room, home-style meals, fun activities and responsive administration made the visit pleasant. Staff kept my family informed and treated us with compassion; I would recommend this place.

    Summary of reviews

Overall impression: Reviews describe a facility with clear strengths in frontline caregiving and therapy alongside persistent operational and facility-level weaknesses. Many families and former residents highlighted compassionate, courteous nursing and STNA staff and praised the physical and occupational therapy teams for measurable gains in mobility and independence. Multiple accounts also commend end-of-life care delivered with attention to comfort and dignity, and note a welcoming, home-like atmosphere in some units.

Care and clinical reliability: Clinical quality appears mixed. Positive reports emphasize attentive, professional bedside care and timely escalation to higher-level medical services when staff recognize a change in condition. However, a recurring pattern concerns staffing levels and response times: reviewers describe limited clinical coverage on some shifts, delayed wound care and medication administration, and occasional episodes that required emergency transfer. There are also specific mentions of incontinence-care concerns and delays in attending to basic resident needs; these point to weaknesses in staffing allocation and workflow rather than isolated interpersonal issues alone.

Staff and family communication: Staff are frequently described as kind, engaging, and willing to work with families. Several accounts note staff who communicated clearly and involved families in care. At the same time, reviewers reported inconsistent communication from management, long hold times or unreachable phone lines, and slow or absent responses to family inquiries. These communication gaps extend to admission logistics and visitor access, where problems with entry codes, scheduling, and late sign-ins were described.

Dining, activities, and therapy: Therapy is a consistent strength in many narratives: well-trained PT/OT staff, progressive therapy plans, and successful discharge outcomes appear regularly. Activities and social programming are described as engaging in some areas, with parties and external-visitor events contributing positively to the resident experience. Dining impressions are mixed—some reviewers praised home-style meals, while others reported poor food quality and inconsistent meal service.

Facilities and environment: The physical plant elicits divergent impressions. Positive comments reference spacious private rooms with comfortable furnishings, TVs and recliners; other reviewers describe small or basic rooms, an older building in need of updates, worn equipment, and cleanliness issues in bathrooms and common areas. These observations suggest uneven maintenance and variability between units.

Management and operations: Management performance is described unevenly. Some families commend an effective administrative team and coordinated clinical transfers, while others describe poor management communication, slow issue resolution, and inconsistent front-desk responsiveness. Access-control and visitor-management procedures were identified as an operational weakness that affected timeliness and family experience during admissions and visits.

Notable patterns and recommendation guidance: The facility appears to offer strong, compassionate bedside caregiving and effective rehabilitation therapy, which can produce tangible functional improvements. Prospective residents and families should weigh those strengths against recurrent operational issues—notably staffing levels, cleanliness and maintenance, inconsistent dining, and communication challenges. For individuals whose primary need is intensive, reliable nursing coverage or specialized memory care, the reported patterns suggest verifying current staffing ratios, infection-control and housekeeping protocols, and management responsiveness prior to admission. For those prioritizing rehabilitation therapy and supportive bedside staff, the facility may provide meaningful benefits, provided families confirm current operational improvements and oversight mechanisms.

About Divine Rehabilitation and Nursing at Pearlview

Divine Rehabilitation and Nursing at Pearlview sits in a quiet country setting in Brunswick, OH, where folks can find both peace and plenty of care options, because they cover a wide range of needs whether someone's looking for assisted living, independent living, skilled nursing, memory care for Alzheimer's or dementia, post-hospital rehab, hospice, or just short-term or respite stays, and the place is staffed with a dedicated team of physicians, nurses, therapists, social workers, and personal care assistants who pay special attention to person-centered care, making sure everyone gets the kind of support they need for both body and mind, which matters a lot when there's specialized memory programs, Music & Memory certification, alternative therapies like massage or lavender oil, and caregiver training services, all there to help keep people comfortable and supported through every stage of their stay.

The rehab and recovery programs are thorough, using state-of-the-art therapy equipment for physical, occupational, and speech therapies, and they've got discharge planning and home safety assessments for better transitions back to life outside, plus outpatient aftercare and a focus on good medical outcomes as shown by a Platinum Level rating for Medicare & Medicaid Quality Measures and a 4-star rating from the Centers for Medicare and Medicaid, and with emergency admissions 24/7, they're ready when urgent care is needed, but folks also notice how the place feels like home because rooms come with things like kitchenettes, flat-screen TVs, cable, Wi-Fi, and new carpeting, with private and spacious suites and sunrooms or lounges so families can gather, or quieter folks can just enjoy the view.

Meals are home-style with menu choices, laundry and housekeeping are provided, and they offer salon services too, so daily living feels a bit easier, and you'll find safety features such as handicap access and sprinkler systems, plus help with walking or wheelchairs, and support for daily tasks like bathing, dressing, medication, and grooming from patient, trained staff, so even with a medical or memory issue, everyday routines can carry on with less worry. There's transportation for medical visits, a family party room, game and activity spaces, arts and crafts, and full social calendars with wellness programs, so no one needs to feel left out, and the facility accepts Medicare, Medicaid, long-term care insurance, and most other insurance, so coverage isn't hard to manage.

The facility has a strong sense of community, avoids alarms or restraints, and focuses on warmth and compassion in care, all while supporting folks with post-acute care, skilled rehabilitation, and long-term nursing, and because the place keeps up with modern amenities and offers spaces like fitness centers, guest parking, and indoor sitting areas, people tend to stay comfortable and active as their needs change, all day and night, since they operate 24/7 and keep the doors open for those needing skilled or nursing care at any hour.
