    Renovated facility, responsive caring staff

I'm very pleased with the positive changes under new ownership - the building is bright, spotless and well renovated. Caring, professional staff (Elizabeth in admissions, nurses, therapists and housekeepers) were responsive and helped my loved one make real rehab progress. Rooms, dining and outdoor spaces are comfortable and accessible, and leadership listens - I recommend this facility.

    Summary of reviews

The reviews portray a facility with a well-appointed physical environment and a polarized pattern of care quality. Many commenters highlight positive aspects of the building: bright, spacious rooms with internet and wall-mounted televisions, accessible bathrooms with large showers and grab rails, private dining rooms, a pleasant dining area, outdoor spaces, and active programming. Admissions and parts of the therapy team receive frequent praise, and multiple accounts credit the rehabilitation staff with measurable functional improvement for residents.

At the same time, a consistent thread across reviews points to operational weaknesses that affect clinical care and resident safety. The most recurrent concerns relate to staffingโ€”both the absolute levels and turnoverโ€”which reviewers linked to delays in attending to residents, unanswered call lights, and inconsistent delivery of scheduled services. Medication-administration problems (timing errors, delays, and tracking issues) and gaps in following physician orders are described often enough to suggest systemic process weaknesses rather than isolated events. Several reviews also describe missed or inconsistent wound-care and personal-care routines.

Dining and dietary operations receive mixed feedback. The facility offers private dining and a well-appointed common dining area, and some reviewers praised meals; others described inconsistent food quality, meal-ticketing errors, and difficulties with special diets or meal substitutions. Communication issues extend beyond meals: families report poor after-hours responsiveness, delayed or missing updates, and uneven engagement from nursing leadership. There are also recurring cleanliness and pest-control concerns in certain units or rooms, even though other areas are described as clean and well-maintained.

Staff behavior and culture appear variable by shift and unit. Many reviewers singled out compassionate, attentive nurse aides, therapists, and some nursing staff by name; others described inattentive behaviors, distractions during shifts, and lapses in monitoring. Leadership and management stability are areas of divergence: some accounts reference positive changes under new ownership and zero state citations, while others cite unresolved complaints, inconsistent management presence, and employee-relations issues.

Notable patterns: experiences appear highly heterogeneousโ€”some families describe successful rehab stays and respectful, timely care, while others describe significant clinical concerns including hospitalization following infections, dehydration and nutritional decline, and serious communication breakdowns (including concerns following a resident's death). These patterns suggest that care quality may depend heavily on unit staffing, shift, and which clinicians are involved. Prospective residents and families may benefit from asking targeted questions about current staffing ratios, medication-safety protocols, wound-care routines, call-bell response metrics, and recent regulatory findings, and from observing care during multiple times of day to assess consistency.

    About Legacy Miamisburg

Legacy Miamisburg sits at 450 Oak Ridge Blvd in Miamisburg, Ohio, and offers a mix of services for seniors including assisted living, Alzheimer's care, skilled nursing, short-term rehabilitation, and respite care, so people can find different levels of support depending on what they need at the time, and they do have English-speaking staff, which makes things easier for many people. The place is run by Legacy Miamisburg Operating Company LLC and has around-the-clock medical support, a clinical and therapy team that focuses on tailored care, recovery, and general well-being, and they try to help seniors stay as independent as possible while also making sure there's always help close by for bathing, dressing, transfers, and medication management. There's a skilled nursing facility for people who need ongoing medical care or are recovering from something, and they offer short-term and long-term stays along with outpatient rehabilitation if someone just needs to get their strength back between hospital and home, and therapists use specialized treatment and equipment to help in these cases.

The community provides designated memory care services for people with Alzheimer's and dementia, including 24-hour supervision, memory-enhancing activities, and support programs, and the staff is trained to help with unique memory challenges. There's a special MedBridge unit designed for short-stay rehabilitation, so people recovering from a stay in the hospital can focus on getting better in a comfortable spot, and private rooms are available as well as semi-private options depending on what the resident prefers, offering some choices for living arrangements along with things like private bathrooms and kitchenettes in the rooms, which helps people feel a little more at home.

Residents can join daily activities like arts and crafts, gardening sessions, games, and community or resident-run programs, and there's always scheduled things to do so people aren't just sitting around with nothing to fill their day. The dining services include restaurant-style meals with options for different dietary needs, so people with diabetes or other food restrictions can feel safe eating what's prepared, and meal prep is handled by kitchen staff so people don't have to worry about cooking if they don't want to. For keeping clean, housekeeping and linen services are part of what they offer, and rooms are kept tidy and well-kept, allowing residents to focus on their interests instead of chores. Residents have access to individual phones to stay in touch with family and staff, and there's a 24-hour call system in place for emergencies, so people shouldn't feel isolated if something happens during the day or night.

Amenities around the community include a dining room, beauty salon, library, computer center, fitness room, gaming room, garden and outdoor areas, as well as a wellness center for various health and exercise needs, and there's air-conditioning in rooms, which can make things more comfortable during hot weather. There's a new transportation bus for outings and appointments, and community-operated or arranged transportation options make it easier to get to where you need to go whether for medical care, shopping, or other local trips, and there does seem to be exterior parking even if details aren't spelled out. There's a smoking area for those who need it and a few apartments, townhomes, and condos in the area that are pet friendly, with various rental options ranging from one to four bedrooms, although exact apartment features and accessibility aren't described. Legal protections against discrimination are in place according to federal and state law, including LGBTQ and source of income protections, so that's something that can help reassure folks about their rights.

Legacy Miamisburg keeps a warm and welcoming atmosphere, has clean and spacious accommodations, and tries to keep residents comfortable and entertained with a variety of programs, and the team-consisting of dedicated nursing staff, nurse practitioners, and an attending physician-focuses on delivering personal care in a safe, respectful place. They haven't reported their current staff availability but do work to provide individualized support and help with daily living, and they're open for scheduled appointments if someone's interested in knowing more or looking at available features.
