    Compassionate care and excellent rehab

I'm very pleased - the nurses, STNAs and therapists were skilled and compassionate, and rehab helped my loved one regain mobility. Staff treated us like family; admissions and administrators (Megan and Tina) were knowledgeable and attentive. The facility is clean, welcoming, and the dining and activities are nourishing and engaging, which gave me real peace of mind - I would recommend it.

    Summary of reviews

The reviews reflect a facility with clear strengths in hands‑on caregiving, therapy, and social programming alongside recurring operational weaknesses. Many families and residents described compassionate nursing and aide teams, skilled therapists (physical, occupational, and respiratory) and good rehabilitation outcomes; several accounts highlight an attentive dietary director, appealing dining-room presentation, and an active activities calendar including concerts, holiday events, bingo and resident‑council engagement. The facility also maintains useful on‑site clinical capabilities such as IV therapy, x‑ray and routine lab work, which supports post‑acute recovery for many residents.

At the same time there is a consistent pattern of operational concerns that prospective families should evaluate. Staffing levels and shift coverage appear variable: while some reviewers praised prompt, family‑oriented care, others detailed delays in nursing responsiveness and gaps in clinical monitoring (for example, wound/pressure‑prevention and respiratory observation). Relatedly, communication between physicians and nursing staff and between staff and families is sometimes inconsistent, and reviewers described shortcomings in medication administration, documentation, and timely charting. These issues suggest the facility may experience episodic stresses on its clinical workflows during busy periods.

Dining and activities are often cited as positives — meals are described as well‑plated, nutritious and generous, and the activities department is seen as enriching — but reviewers also note inconsistent meal temperature and flavoring across meals and shifts. Facility condition feedback is mixed: several residents and families describe clean, home‑like common areas and attractive dining; others cite deferred maintenance, aging room amenities, unpleasant odors at times, and issues with alarms or door access. Laundry and personal‑item management (including unlabeled clothing) emerged as another operational weakness that affects resident satisfaction.

Management impressions are divided. Some reviews praise long‑tenured administrators and helpful admissions and maintenance staff who make families feel supported; others raise concerns about administrative communication, abrupt discharge or transfer practices, payroll/employee‑support issues, and isolated incidents of unprofessional staff conduct and privacy breaches. While many positive accounts describe a family‑like culture and staff going above and beyond, the presence of several serious individual accounts of clinical and administrative lapses indicates variability in consistency of care.

For families considering this facility, key inquiries would include current staffing ratios and on‑shift coverage, documented protocols for turning and pressure‑injury prevention, respiratory and tracheostomy monitoring procedures, medication‑administration and charting practices, laundry and personal‑item tracking policies, maintenance and room‑upgrade plans, and the facility's approach to discharge planning and family communication. The facility demonstrates genuine strengths in therapy, social programming, and compassionate caregivers, but prospective residents and families should seek up‑to‑date information on staffing, clinical monitoring, and operational controls to ensure those strengths are reliably delivered.

About New Lebanon Rehabilitation and Healthcare Center

New Lebanon Rehabilitation and Healthcare Center sits at 101 Mills Pl. in New Lebanon, Ohio, and has 120 certified beds with an average of about 90 residents each day, and there's a video tour that shows you all around the facility and helps you get a good idea of the layout and what you might expect if you ever visit or stay there. This nursing home offers both short-term and long-term care options, providing things like skilled nursing, memory care, Post-Acute Rehab, and Assisted Senior Living, with services covering Alzheimer's care, orthopedic rehabilitation, ventilator care, dialysis, mental health recovery, stroke care, pulmonary care, cardiac care, and even respite care for families who need a break. The campus is known for being beautiful and amenity-rich, and there are resources for caregivers as well as an admissions office ready to answer questions and help new residents get settled, plus they take admissions at any hour, day or night, and work with every hospital and most insurance plans, which does make it easier for families going through transitions.

The center is affiliated with Crown Healthcare Group and is tied to one of the largest rehabilitation therapy companies in the nation, and there's a highly skilled team of healthcare professionals on staff, including a dedicated medical staff that uses up-to-date medical technology. Nurse staffing sits at 3.40 nurse hours per resident per day, and the nurse turnover rate is at 55.8%. Residents get access to a range of amenities that aim to make life a bit more comfortable and homelike, and the place keeps a steady focus on providing what they describe as compassionate, quality care. They've got both inpatient and outpatient programs, plus specialized services for different medical and rehabilitation needs.

Now, it's important to know the center has seen a total of 60 deficiencies in inspection reports, with 5 of those linked to infection control, and the government did cite the facility for violating federal standards on preventing the spread of infections, noting issues in October and December of 2024. That history of serious quality issues, including being identified as a Special Focus Facility Candidate, is worth considering, though the government hasn't formally flagged them yet. Reviews average out to 4.3 from 51 reviewers online. Even with past problems, staff at New Lebanon Rehabilitation and Healthcare Center continue to provide care to a mix of residents with varied and specialized needs in a setting that tries to feel warm and supportive, offering programs and care for people with both short-term and long-term needs.
