    Clean welcoming community, excellent staff

I toured this community and was impressed. It's clean and welcoming, staff are consistently kind, professional, and helpful, and the rehab/PT care is excellent. Good meals with dietary options, plenty of activities (beauty shop, gardening, church), and solid security make it feel like home - I highly recommend it.

    Summary of reviews

Community Skilled Health Care Centre presents a mixed profile. Strengths frequently cited include accessible location for families, a welcoming atmosphere, and a number of long‑tenured, compassionate staff members. The assisted‑living and short‑term rehab areas receive consistent praise for skilled physical and occupational therapy, effective short‑term rehabilitation outcomes, and a restaurant‑style dining room that offers dietary customization (including diabetic and low‑cholesterol options). Residents and families commonly note clean, well‑maintained apartments, active programming (music, puzzles, church service, gardening, beauty shop), on‑site amenities, and ongoing remodeling that has refreshed parts of the facility.

However, a notable pattern of operational weaknesses emerges across reviews. Nursing‑level care is described as inconsistent, with recurring concerns about understaffing and slow response times to resident needs. Several accounts indicate medication‑administration and clinical‑knowledge gaps—examples include difficulties managing respiratory devices and delays or errors in medication delivery—that point to uneven clinical training and supervision. There are also reported deficiencies in end‑of‑life and hospice communication and coordination, which have heightened family concern following serious medical escalations.

Facility and management issues are also evident. While parts of the property have been updated, other areas show aging infrastructure, water damage, and worn furnishings; space constraints and mixed resident-grouping lead to limited privacy in some rooms. Sanitation and infection‑control inconsistencies are noted for particular units, and some families describe inconsistent accommodation of specialized dietary needs. Finally, several reviews express concerns about administrative priorities—perceptions of an emphasis on occupancy and finances—and there are isolated allegations of financial misconduct that would warrant further inquiry by prospective residents and families.

For prospective residents and their families, the facility appears to offer strong rehabilitative services and a range of social amenities, with many staff members who are caring and familiar to residents. At the same time, decision‑makers should weigh reports of inconsistent nursing care, staffing variability, clinical and medication‑management gaps, and intermittent facility maintenance issues. Visiting in person, asking specific questions about staffing ratios, medication safety protocols, infection‑control practices, hospice coordination, and contractual/financial policies, and seeking recent inspection or licensing records would help clarify whether the facility’s strengths align with an individual’s priorities and clinical needs.

About Community Skilled Health Care Centre

Community Skilled Health Care Centre sits over on Mahoning Ave NW in Warren, Ohio, right in a diverse neighborhood where folks from all sorts of backgrounds, including African American, Hispanic, and White, live together, and the place itself is medium-sized, with both Assisted Living and Skilled Nursing areas for those who need different levels of care, so some people come in for long-term help, others for outpatient care, and there's plenty of services like physical, occupational, and speech therapy for rehabilitation, and the staff, who speak English, stay on-call around the clock for any medical needs, especially with their skilled nursing and respiratory therapy ready all the time, and you'll also see options for memory care, especially for those with Alzheimer's or other cognitive troubles, with special programs in place to support them. There are licensed social workers on staff, and the place keeps up with Medicare and Medicaid, so payment's a bit easier to figure for many, and all around the campus, you'll notice things like a garden, library, walking paths, and even movie nights as simple perks, while inside, residents get private rooms, help with things like bathing and getting dressed, medication management, and a 24-hour call system for peace of mind. The two connected buildings, Community Skilled Health Care and Community Commons Assisted Living, work together to cover pretty broad ground on both medical and daily needs, and alongside the nursing and therapy services, there's podiatry, dentistry, optometry, and optician services offered on-site, so it's not a scramble to get everyday health basics taken care of. For daily living, you'll find resident-run activities, planned events every day, and support for housekeeping, laundry, maintenance, and even transportation, which helps folks stay active and tied into the community, and Religion's cared for, too, with religious centers nearby, plus physicians, hospitals, pharmacies, and cafes close if folks want to venture out. Community Skilled Health Care Centre, licensed by the Ohio Department of Health and marked as a four-star, non-profit skilled nursing facility, does everything to keep things community-oriented and supportive, aiming for a place where everyone gets the care, social life, and safety they need, but right now, they aren't accepting new patients.
