    Skilled, caring staff; clean, home-like

I had a very positive experience with the new management and genuinely caring, long-tenured staff who treated my father-in-law like family. Skilled therapy, respiratory and dialysis services, quick communication, and a clean, home-like atmosphere helped his recovery. I'm comfortable recommending this facility for rehab and long-term care.

    Summary of reviews

The reviews present a mixed but distinct profile of Warren Nursing & Rehab — a facility with substantive clinical capabilities alongside recurring operational concerns. Strengths cluster around specialty clinical services: the facility operates an on-site dialysis program and a ventilator/respiratory care unit staffed by experienced respiratory therapists who are frequently described as knowledgeable and effective. Rehabilitation and therapy services receive consistent praise for producing measurable recovery outcomes, and several reviewers highlighted successful weaning of trach/vent patients. Reviewers also note team-based clinical coordination between dialysis, respiratory, and therapy staff, which appears to support positive outcomes for residents with complex needs.

Staff-level feedback is polarized. Many families and residents describe attentive, compassionate nurses, aides, and administrators who create a welcoming, home-like environment and offer robust social programming (bingo, arts, outings). Admission and intake staff are often characterized as helpful, and specific administrators and directors were named for advocacy and accessibility. The building and common areas are frequently described as comfortable with ongoing upgrades and modern equipment; some reviewers also praised amenities such as televisions, arcade games, and a fireplace.

Counterbalancing those positives are repeated operational and safety concerns. A number of reviews describe inconsistent staffing levels and turnover that manifest as slow call‑bell responses, delays in medication administration, and variability in personal‑care routines (including bathing and incontinence care). Related clinical process gaps include limited on-site physician availability, uneven family notification following emergencies, and instances that suggest weaknesses in fall-prevention and transfer-safety practices. Transportation and external appointment coordination have also been called out as unreliable in some cases.

Cleanliness and environmental maintenance are another area of mixed feedback. While several reviewers praise a clean, odor‑free environment and ongoing facility improvements, other reviewers raised sanitation and odor concerns in resident rooms and common areas and described inconsistent housekeeping performance. Dining impressions are likewise variable: some reviewers compliment food quality and special presentations, while others note irregular meal timing and limited menu choices at times.

Management and leadership are presented as evolving. Multiple reviewers credit new ownership and an engaged administration with tangible improvements in care quality, staffing, and facility upkeep. At the same time, other accounts describe periods of weak oversight, unresponsiveness from leadership, and limited visible management during shifts. There are also serious allegations by some families regarding the influence of financial considerations on care decisions; these are substantive claims that warrant further inquiry by prospective families.

Recommendation for prospective residents and families: Warren may be a strong option for individuals with specialized needs (dialysis, ventilator/respiratory care, intensive rehab) because of its clinical capabilities and therapy outcomes. When evaluating the facility in person, focus on current staffing ratios, medication-administration processes, call‑bell response times, wound and incontinence-care protocols, infection-control and housekeeping practices, physician coverage, and family-notification procedures for emergencies. Ask about recent quality-improvement initiatives under current management and request documentation of staffing stability and outcomes in specialty units. An on-site visit during a typical weekday and a review of recent inspection or survey findings will help confirm whether the facility’s operational improvements are consistent and sustained.

    About Warren Nursing & Rehab

Warren Nursing & Rehab in Morgandale, Ohio, gives care to people who need special medical treatment, especially those who need dialysis and ventilator support, so if someone has kidney or breathing problems and needs these treatments right at the facility, they've got nurses and therapists and even renal specialists who help manage that. The place can now treat up to 30 dialysis patients daily, using advanced NxStage machines, which is more than before, and they've got a Moderate Complexity Laboratory in-house to help with medical tests. There's a special unit for people on ventilators, including folks who need weaning, and they'll accept patients from out of state-even as far away as Florida. Staff provide hemodialysis within the nursing and rehab center, and they're partnered with Dialyze Direct for staff-assisted treatments, which means residents don't need to go to a clinic somewhere else, and that can really help family caregivers by making things easier.

Ventilator care and vent-dialysis are both available around the clock, and there's a focus on keeping everyone safe and comfortable during treatments. Onsite services also include full respiratory therapy and pulmonary rehabilitation, so if someone needs support breathing or therapy after hospital stays, everything's set up here to help with recovery. The facility uses interdisciplinary teams-nurses, dietitians, social workers, and therapists all work together-for care planning and close monitoring by nephrologists and renal nurses, so treatments match the patient's exact needs. Memory care services connect with onsite hemodialysis for those dealing with dementia or cognitive changes, making it easier to manage both challenges at once.

There's wound care, specialized rehab, and activities to help keep minds active and bodies moving, and they're set up for recovery during short-term stays, too. With 24/7 care, patients get support whether they're there for the long-term or just while healing after a hospital stay. The center expanded its services with a ribbon-cutting event in July 2024, and is now one of Ohio's largest focused on dialysis and ventilator care. People who've left reviews give it about a 3.7 out of 5 rating based on 23 reviews. The main goal is to improve quality of life for each person through careful, on-site care and support.
